Once the patient has been found by PDS and registered at their new practice a message

is sent to the previous practice via the ‘Spine’, requesting a copy of the patient’s electronic

health record

The previous practice’s system locates the patient’s record and sends an acknowledgement stating whether or not it is able to provide the electronic patient record

The electronic record is then generated and securely sent to the new practice for integration into its GP clinical system. The record includes any attachments to the electronic record e.g. scanned letters.
