Flagyl (Metronidazole Tablets) is used in the prevention of post-operative infections due to anaerobic bacteria, particularly species of Bacteroides and anaerobic streptococci.This medicine is also used in the treatment of septicaemia, bacteraemia, peritonitis, brain abscess, necrotising pneumonia, osteomyelitis, puerperal sepsis, pelvic abscess, pelvic cellulitis, and post-operative wound infections from which pathogenic anaerobes have been isolated.

Other Flagyl indications include Urogenital trichomoniasis in the female (trichomonal vaginitis) and in the male. Bacterial vaginosis (also known as non-specific vaginitis, anaerobic vaginosis or Gardnerella vaginitis), all forms of amoebiasis (intestinal and extraintestinal disease and that of symptomless cyst passers); giardiasis and acute ulcerative gingivitis.

Trichomoniasis:In Female Adult Patients: One-day treatment - two grams of Flagyl, given either as a single dose or in two divided doses of one gram each, given in the same day.

Seven-day course of treatment - 250 mg three times daily for seven consecutive days.

Pregnant patients should not be treated during the first trimester.

Flagyl dosage should be individualized as per the requirement of the individual patient. The dosage of Flagyl tablets (Metronidazole Tablets) for various indications is given below:

Recommended Adults Dosage for Amebiasis: For acute intestinal amebiasis (acute amebic dysentery): 750 mg of Metronidazole orally three times daily for 5 to 10 days.

For amebic liver abscess: 500 mg or 750 mg orally three times daily for 5 to 10 days.

Pediatric patients: 35 to 50 mg/kg/24 hours, divided into three doses, orally for 10 days.

Anaerobic Bacterial Infections: In the treatment of most serious anaerobic infections, FLAGYL I.V. (metronidazole hydrochloride) or FLAGYL I.V. RTU (metronidazole) is usually administered initially.

Flagyl (Metronidazole Tablets) have to be stored at controlled room temperature i.e. from 20C to 25C (68F to 77F) . Keep this as well as all other medicines away from children and pets.

Metronidazole has been shown to be carcinogenic in mice and rats. Unnecessary use of this medicine should be avoided.Cases of encephalopathy and peripheral neuropathy (including optic neuropathy) have been reported with Metronidazole.Encephalopathy has been reported in association with cerebellar toxicity characterized by ataxia, dizziness, and dysarthria.

Peripheral neuropathy, mainly of sensory type has been reported and symptoms include numbness or paresthesia of an extremity.Convulsive seizures have been reported in patients treated with Metronidazole.Cases of Aseptic Meningitis have been reported with metronidazole. Symptoms can occur within hours of dose administration and generally resolve after Metronidazole therapy is discontinued.

Flagyl is contraindicated in patients with a hypersensitivity to Metronidazole or any other component of this medication. In patients with trichomoniasis, Flagyl Tablets are contraindicated during the first trimester of pregnancy. Use of Metronidazole tablets is associated with psychotic reactions in alcoholic patients who were using Disulfiram concurrently.Patients who have taken Disulfiram within the last two weeks should not be administered Metronidazole tablets.

Contraindications

Use of Metronidazole tablets is associated with a Disulfiram-like reaction to alcohol, including abdominal cramps, nausea, vomiting, headaches, and flushing. Stop consumption of alcohol or products containing Propylene Glycol during and for at least three days after therapy with Flagyl tablets.

Common Metronidazole side effects with an incidence rate 1% or more of those treated with the medicine include nausea, diarrhea, weight loss, abdominal pain, vomiting, headache, dizziness, and metallic taste in the mouth.

Infrequent side effects include hypersensitivity reactions (rash, itch, flushing, fever), headache, dizziness, vomiting, glossitis, stomatitis, dark urine, and paraesthesia.

High doses and long-term systemic treatment with Metronidazole are associated with the development of leucopenia, neutropenia, increased risk of peripheral neuropathy, and central nervous system toxicity

Metronidazole has been associated with cancer in animal studies.

Metronidazole Overdose

Single oral doses of Metronidazole tablets, up to 15 gm, have been reported in suicide attempts and accidental overdoses. Symptoms reported include nausea, vomiting, and ataxia.Metronidazole tablets has been studied as a radiation sensitizer in the treatment of malignant tumors. Neurotoxic effects, including seizures and peripheral neuropathy, have been reported after 5 to 7 days of doses of 6 gm to 10.4 gm every other day.

There is no specific antidote for Flagyl overdose. Therefore, treatment of the patient should consist of symptomatic and supportive therapy.

Metronidazole During Pregnancy

Flagyl (Metronidazole Tablets) has been classified by the US FDA as Pregnancy Category B. Animal reproduction studies have failed to demonstrate a risk to the fetus and there have been no adequate and well controlled studies of Flagyl in pregnant women. Use of Flagyl tablets (Metronidazole) for treatment of trichomoniasis in pregnancy should be carefully evaluated because Metronidazole crosses the placental barrier and its effects on the human fetal organogenesis are not known.
