Proposal

• Clearly identify the barrier you are targetting.• Identify a target population.• Validate your choices – real data.• Offer a solution – describe briefly. Specify the

target health behaviours or choices.• Validate your solution – Align to an Ottawa

Charter Action Area, offer real data, tell me why it will work on your population and why it will correct your barrier.

Proposal

• There is a template developed for written responses.

• However, your proposal can be in any form, as long as it addresses the requirements.

Plan of Action.

• You will only be allowed to start on this once I have approved (by signing off) your proposal. Note: I do not need to sign off on a good copy.

• Identifies all the actions that need to occur, who will be responsible, how you will measure that it is complete.

• Have a space for contingency, reflection notes and adaptation.

• I envisage this being a fluid document (meaning it will change), however save each version so changes can be shown.

• No template, I suggest a table in landscape view.

Implementation

• This stage will not occur until I have approved the initial action plan.

• You carry out the tasks in the plan of action, reflecting as you progress.

• Remember to “measure” at each step.• Think about how you will record progress

and action. (ePortfolio)

Appraisal

• At a given point, I will call a complete stop to implementation.

• At this point, we appraise our success by referring to the questions on the task sheet.

• Refer to your reflective blogging and fluid plan of action to assist.
