Join and open the application

Go to templates and select one

Another option …Mailing List

Other options…Guestbook etc. etc.

Make your choice and start editing

to change to your liking

Click on each word of the

template and edit it

Go through the options one

by one

If you need new fields choose them

Save the form

Embed it into your blog or wiki

Limitations
