PenaltiesASI will forfeit 2% of its admin fees for each quarter it misses any of the proposed PG’s. Thedetails of the PG’s will be developed during the contracting phase of the procurement process

Flexible Spending Account COBRA/Billing

Claims – 95% within 3 business days, 99% within 5 business days

QE Notification – 100% within 14 business days

Call Center – 95% within 30 seconds or lesss Call Center – 95% within 30 seconds or lesss

Administrative Fees

Proposed Fee Schedule

BAFOASIFlex is waiving its implementation fee for the City, and all fees are guaranteed for a periodof three (3) years.

Flexible Spending Account COBRA/Billing

Implementation Fee – Waived Implementation Fee – Waived

$3.50 PPPM $20 per QE

$1 for optional debit card 2% surcharge retained

Onsite meetings $250/day plus travel $8 per takeover
