INDEMNIFICATION AGREEMENTWAIVER AND RELEASE OF ALL CLAIMSPERMISSION TO SECURE TREATMENT
Loras College Softball Hitting & Pitching ClinicNovember 6 & 13, 2016
Please read this form carefully and be aware that by participating in the Loras College Softball Hitting & Pitching Clinic on November 6 and 13, 2016 (hereinafter Event) you will be waiving and releasing all claims for injuries, agreeing to indemnify, hold harmless and defend Loras College from all claims arising out of such injuries even if caused by Loras College and authorizing Loras College to obtain emergency healthcare at your expense.
I, on behalf of myself and, on behalf of any child/ward of mine participating in the Event as well as any parent/guardians of such child/ward (hereinafter individually and collectively referred to as “Participant”), acknowledge understanding of the requisite skills and qualifications necessary to properly and safely participate in the Event and hereby agree to assume the full risk of any injuries, including death, damages or loss regardless of severity, which Participant may sustain as a result of, arising out of, connected with, or in any way associated with the Event.
Participant agrees to waive and relinquish all claims Participant may have as a result of the Event against Loras College and its employees and agents and does hereby fully release and discharge Loras College and its employees and agents from any and all claims for injuries, including death, damage or loss which Participant may have or which may accrue to Participant as a result of, or arising out of, connected with, or in any way associated with the Event, even if caused by the negligence of Loras College, its employees or agents.
Participant further agrees to INDEMNIFY AND HOLD HARMLESS AND DEFEND Loras College and its employees and agents from any and all claims for injuries, including death, damages and losses sustained by Participant as a result of, arising out of, connected with, or in any way associated with the Event, even if caused by the negligence of Loras College, its employees or agents..
Participant further understands that Loras College does not carry insurance for injuries sustained by Participant. Therefore, Participant must look to their own health insurance policy for any injuries sustained in connection with or arising out of this Event. Participant’s failure to purchase health insurance coverage does not make Loras College responsible for payment of medical or other expenses.
In the event of an emergency, Participant authorizes Loras College to secure any treatment deemed necessary from any licensed hospital, physician, and/or medical personnel and agrees to be responsible for payment of any and all services rendered. If any provision herein is held invalid or unenforceable for any reason, Participant understands and agrees that the remaining provisions will continue in full force and effect.

• Pitchers must supply their own pitching ball and catcher, and all catchers must complete a waiver! • Grades 4-7 will learn basic pitching mechanics, grips for fastball and changeup• Grades 8-12 will learn basic pitching mechanics, grips for fastball, changeup, curve, rise and drop• If you are attending the pitching camp and hitting camp, please bring your own lunch to eat between sessions.
HITTING CAMP

Campers will be learning offensive fundamentals including hitting, bunting and base running. This camp is designed to prepare campers with the drills and skills to help them improve their offensive abilities in the offseason.

ASHLEY WINTERHEAD COACH / CAMP DIRECTORDuring her softball playing career with the Wartburg College Knights, Winter
was a first team All-Iowa Conference selection in 2008 and a two-time second team pick as a first baseman. In her final season, Winter hit .400 and was in the top five in the IIAC in batting average, slugging percentage, on base percentage, runs and homeruns.
In July 2009, Winter was named the Iowa Conference nominee for the NCAA Woman of the Year Award. In 2011 she was inducted into the Iowa Girls High School Athletic Softball Hall of Fame. She was also named to the Wahlert Catholic High School Athletics Hall of Fame in 2012.
Kevin Schuchmann has been a softball pitching clinician since 1996, providing pitching instruction throughout Northeast Iowa.
Schuchmann is a member of the National Fastpitch Coaches Association and has over 800 career high school wins in 28 years, including a 2007 IHSSA Championship at West Delaware High School and a 2002 IHSSA State Runner-Up finish at Edgewood-Colesburg High School. Eight times in his coaching career, Schuchmann has qualified his team for the IHSSA Softball Tournament.
In 2014, Schuchmann was inducted into the Iowa High School Softball Hall of Fame.
