Your benefits include everything from health care and insurance to a retirement savings plan, paid time off and employee perks and discounts. The programs are designed to offer a variety of options, so you can participate in the plans that are right for you. It’s worth investing a little time to find out about how our programs can help you improve your health, save time and save money!
This brochure summarizes your benefit plans for 2010. The benefits shown here are for comparison purposes only. For information regarding benefits that are not outlined in this brochure and complete plan details, including your Summary Plan Descriptions, visit our benefits website at www.getardentbenefits.com
Eligibility
You are eligible for the benefits described if you are a full-time or regular part-time employee working at least 20 hours per week. Your dependents including your spouse or domestic partner, unmarried children through age 19 and unmarried children under age 25 if full-time students are also eligible for some benefits, including medical, dental and vision.
Your benefit elections
The benefits you choose as a new hire or during Open Enrollment will remain in effect for the plan year (January 1 through December 31).
You may change coverage only due to a Qualified Life Event change and must do so within 31 days of the event. Qualified Life Events include getting married, having a baby and other changes in status.
Health, wellness and insurance benefitsTaking care of you and your family
Ardent offers health and insurance benefits to keep you and your family healthy and provide financial protection in case of a serious illness or injury. With the help of Ardent’s benefit resources, you and your family can pick the plans you need and use them efficiently.
Bravo Wellness Program
The Bravo Wellness Program helps you learn important information about your health and gives you a way to control your medical premiums. If you participated in the Bravo Wellness Program health assessment and screening, you will receive discounts to help offset your 2010 medical premiums. New hires will have the opportunity to participate throughout the year.
The wellness program is offered through Bravo Wellness, a confidential third party and leader in wellness initiatives. If you have questions about the program or are a new hire and want to participate in an upcoming screening event, you can visit the Bravo web site at www.bravowell.com/ardent or call 877-66-BRAVO (877-662-7286).

Hillcrest Healthcare System and Corporate Office medical plans
Hillcrest Healthcare System and Corporate Office employees have two medical plans available through BlueCross and BlueShield of Oklahoma: the Essential Plan and the Core Plan. Benefits will be paid at a higher level when you obtain services at an Ardent facility.** To locate participating providers, visit www.bcbsok.com.

All of Ardent’s medical options are designed to promote good health and provide coverage for health care expenses. We have some welcome news in the face of this challenging economy! In a year when many companies are being forced to scale back benefits or pass along big premium increases to employees, we are delighted to inform you that your payroll deductions for Intuit’s high-quality health benefits will not increase for FY10…for the first time ever at Intuit.
And, to better manage Intuit’s and your costs, we are making some changes with our carriers. You’ll have a new medical network to access for your medical providers, and a separate prescription drug program is being implemented for most plans.
Let’s pay attention to these changes and their implications. Please read this brochure and the annual enrollment updates on the Benefits@Intuit website and Insight so that you fully understand how these changes impact you and your family.

Is your doctor in the new network?
Is your doctor in the new network? We carefully evaluated the Cigna network to ensure your doctors and providers are in the new network. Over 90% of the Aetna providers and over 97% of the facilities used by Intuit employees and families are in the Cigna network. You should still check to make sure your doctors are in the new network by visiting Cigna or UHC online or by calling their customer support line.

Currently in treatment for a serious condition? If your doctor is in the network today but not in your new network, and you are in treatment for a serious condition, you may be eligible for transition of care benefits. Transition of care will let you continue to see your current provider at the in-network benefit level for a set period of time. Serious conditions include cancer, kidney disease and organ transplants, or if you’re in the second or third trimester of pregnancy. flexibility, we offer our hoods with either
an interna l Recirculating Venti lat ion System or a duct-out , chimney-style ex-haust. The choice is yours. Charcoal Filters cleanse and de-odorize the air from the
cooktop before being recirculated.
You also have a choice of either a 600 cfm Internal Blower or an extremely powerful1,100 cfm In-Line Blower that mounts at a distance from the hood. Both options quickly
clear the air in virtually any cooking area.
