Feedback We welcome feedback from anyone using our services.
If you have any suggestions or comments please ask for a ‘Friends &
Family’ card. This can be used to give feedback on the services at the
surgery. You can also leave comments on our website ‘Friends &
Family’ section, or the website suggestion box.
Alternatively you may write to the Practice Manager Vicki if you have
any comments or complaints. All written complaints will be
acknowledged within 48 hours and a full investigation will be
undertaken.
You can also seek advice from NHS England 03003 112233, or
Healthwatch Rutland on 01572 720381.
If you are unhappy with the outcome of your complaint you may write

Patient Participation Group
All patients are eligible to join the Uppingham Surgery Patient
Participation Group (PPG). The PPG meet regularly with the Practice
team to exchange ideas and information. We always welcome new
members to the PPG. Contact details for the PPG members are
available on the PPG display board at the surgery and on the surgery
website.

There is a no smoking policy at the surgery, this applies to the branch
surgeries and the surgery grounds.

About Our Practice Our surgery is located at the edge of the historic market town of
Uppingham. Uppingham Surgery serves the community in Uppingham
and the surrounding countryside.
We operate a main surgery in Uppingham and branch surgeries in
Barrowden and Gretton. We provide GP and nursing appointments
and dispense medication at all sites.
We are also a training practice which means that we host doctors who
are undergoing their specialist GP training and placements.
surgery Access
All our facilities are on the ground floor, at all sites and are accessible
to patients with mobility problems and wheelchair users.
Disabled parking is available
We have a hearing loop system available for those with hearing
difficulties
An interpreter service can be made available for those who do
not speak English
Breast feeding mothers are welcome at the surgery
Chaperone—Please tell us if you would like another person to be
present during a GP or nurse consultation or examination, we can
then arrange this.
If you have any special requirements please let us know.
Home visits are only for the housebound and seriously ill. If you need
a doctor to visit please telephone the surgery before 10am on the day
that you require a visit.

Data Protection As a Practice we take care to ensure that information about you is not
disclosed to anyone who is unauthorised to see it.
Please see our website or contact reception for more details
regarding access to records and any exceptions to the policy. Under
The Data Protection Act 1988 you may access your own medical
records. Forms are available at reception to request copies of your
record.
You may sign a confidentiality consent form if you would like to give
consent for a third party (including relatives) to have access to your
records. Forms are available from Reception.
Patients who have internet access can also view certain aspects of
their medical record online. Please ask a member of staff for more
details.
There are various initiatives for electronic record sharing between
organisations that care for you within the NHS. If you would like more
information please ask for a ‘Medical Record Sharing Information
Pack’, visit our website, or the NHS choices website.
The EU's General Data Protection Regulation (GDPR) will apply from
25 May 2018, when it supersedes the UK Data Protection Act 1998 .
zero tolerance
The surgery operates a zero tolerance policy and will not endure
abusive, violent or unreasonable behaviour. Patients who are
deemed to fall within this category will be removed from the surgery
list and will be required to register with a secure surgery in Leicester.

New Patients our practice boundary
We can only accept patients who fall within our practice boundary.
Please see below. You can use our postcode checker on our website
for further clarification.
However we will consider registering patients who live at least 50
miles from our Practice boundary and who commute to work in our
practice area on a regular basis.
how to register as a patient
If you live in within our practice area you can register by calling in to
any of our sites to collect our registration forms, or by completing
the online form.
After you have completed and submitted the forms you must bring 2
forms of identification (ID) to the surgery within 7 days. One of these
must contain your address and the other to have your photograph.
When registered you will be assigned an allocated GP. Requests to
register with another GP will be considered. Please ask the Patient
Services Team for a ‘Change of GP’ request form.

The Clinical Team are supported by our Patient Services Team
who are often our patients first point of contact in the surgery. Our
Dispensary Team are responsible for dispensing medication to
patients at all four of our surgery sites.

The practice employs a diverse skill mix of clinical staff have been
appointed to ensure that you and your family are seen by the most
appropriate clinician available at the practice.

To provide you with the best possible care we ask that you ring
reception preferably between 8:15am and 11am.
11am onwards are for medical emergencies only.
You can also book a telephone call-back appointment online (see
online services).

Don’t be offended if a Patient Services Team member asks for some
details of your condition. We understand that things are personal, but
if you can tell us what the problem is this enables them to book a
longer appointment if necessary and make sure you are seen by the
most appropriate clinician. All of our staff are bound by patient
confidentiality, which we take extremely seriously. The Patient Services Team may also ask you to confirm your name,
date of birth and address. It is important that you keep us up to date
with any changes to your name, address or telephone number.

How we communicate
SMS - We have a text messaging service to remind you of any
appointments or review dates that are due.
EMAIL - We have an email service to keep you informed of surgery
news.
LETTERS - We may send you out a letter inviting you for a service,
reminding you to book an appointment or just updating you on
surgery news.
If you would like to receive any of the above then please speak to a
member of the Patient Services Team.
It is important that you keep us informed of any change in your
contact details, such as address or telephone number.

test results
Please telephone the surgery on 01572 823 531 after 2.00pm for
blood test and x-ray results, or make a request online via our
website.
If you call the surgery for your test results then our Patient Services
Team will inform you of the result. Our Patient Services Team will not
be able to give out any clinical information such as numeric values but
what they can do is book a telephone call with your GP so they can
discuss your results with you in more detail if required.

Self Management quick advice
Your local pharmacy is the place to go to get any clinical advice for
minor health concerns. They can also advise you on the safe use of
prescription and over-the-counter medicines
As well as helping with common illnesses – like sore throats, coughs,
colds, tummy troubles and aches and pains – pharmacy teams can
also help with stopping smoking, cutting down on alcohol, advice on
safe sex and emergency contraception.
what happens when i get referred?
When a clinician refers you to the hospital our secretary will complete
the referral and send this off. You will either hear directly from the
hospital or receive a letter from us about how to book your
appointment.
If you have any problems or queries about your appointment the
most appropriate people to contact is the hospital you have been
referred to.

Sometimes the charge is because the service is not covered by the
NHS, for example, providing copies of health records or producing
medical reports for insurance companies, solicitors or employers.
patient responsibility If you need to cancel an appointment please give as much notice as
possible so that we may offer the appointment to someone else. You
may cancel your appointment online if it is more than 24 hours before
your appointment time.
The Uppingham Surgery works in partnership with the patients. The
patient has a responsibility to ensure the services provided under the
NHS are used appropriately.

Prescriptions uppingham dispensary opening times
Monday - Friday 8.30am - 6.00pm
Branch Surgery Dispensaries are open during branch surgery hours.
We can dispense medications to any patient who lives more than 1
mile (in a straight line) from Uppingham Surgery.
ordering a repeat prescription
There are various ways that you can order your prescription:
Order using your repeat prescription slip, drop off your repeat
prescription slip in the post box situated outside of the front
entrance or inside just before the dispensary hatch.
You may order repeat prescriptions online (ask a member of
staff for your log in details).
You may order using our telephone system. Call 01572 820123.
Lines are open Monday-Friday, 9am-1pm
You may sign up to a local Pharmacy prescription service if we cannot
dispense for you, or it is more convenient for you.
Please allow 48 hours notice for repeat prescriptions. This allows
time for the doctor to check and sign the prescription.

Online Services about our online services
By registering for the online services you will be able to complete a
variety of different tasks online rather than having to come in to the
practice such as:
Book a call back with a clinician
Cancel your appointment
Request repeat prescriptions
View your medical record information such as test results, medications, allergies and immunisations.
Submit feedback
These online services are as safe as online banking as long as you
keep your passwords secure, and you can access these services 24
hours a day 7 days a week.
how to register
If you would like to register for online services please call into the
surgery or complete the form on our website.
A registration form will need to be completed and your identity
verified face-to-face with proof of identity (passport/driving licence/
bank statement) by a member of the practice team.
Once you have these log in details you are ready to use our online
services.
