An Integrated, Mu lt id isc ip l inary Approach to Treatment

INTRODUCTION AccordingtoarecentcoststudyconductedbyHomeBase(2013),betweenJune1,2011andMay31,2012,34individualsintheCountyofMarinincurredacostof$2,039,463forservicesrelatedtochronicaddictiontoalcohol.Actualcostsare“mostlikelymuchgreater”(HomeBase,2013,p.i).Thevastmajorityofthese34individualsweremale,91percentwerehomeless,andtheaverageagewas47.

Inrecentyears,innovativeprogramsinseveralCaliforniacountieshavesprungupattemptingtoaddressthisissue(HomeBase,2013).SanDiego’sSerialInebriatePrograminparticularachieveda58%reductioninarrestsanda50%reductionintheuseofemergencyandinpatientmedicalservicesforparticipantswhocompletedtheprogram(HomeBase,2013,Appendicesp.5‐6).SanDiegoCountyusesajaildiversionmodelofferingtreatmentinlieuofincarceration(HomeBase,2013).

InJanuary2012,MarinCountyDeputyPublicDefenderChristineO’HanloninitiatedthecreationofasimilarprograminMarinCounty.Theprojectquicklybecameacollaborativeeffortbetweencountyandcommunity‐basedorganizations,leadingtoMarinHealthandHumanServicescommissioningtheHomeBasestudy.

Initsfinalreport,HomeBase(2013)madethefollowingrecommendations:

1. DesignatetheForensicMulti‐DisciplinaryTeam(FMDT)astheBodytoOverseetheContinuingandCoordinatedEfforts

2. ExpandtheCapacityoftheFMDTtoConductOutreachandFollowUp3. DevelopaJailDiversionProgram4. EstablishTargetedScattered‐SitePermanentSupportiveHousing5. CreateaViableMedicalDetoxificationOption

PROJECT OVERVIEW TheMarinChronicAlcoholwithJusticeInvolvement(CAJI)Projectofferstreatmentinlieuofincarceration(jaildiversion)forindividualswhohavesixormorealcohol‐relatedcontactswithina12‐monthperiod.Thecourseoftreatmentissevenmonths.Ahybrid,multidisciplinarymodelcombiningcollaborativecourtstructureswithAssertiveCommunityTreatment(ACT)isutilizedtoprovidehighlyindividualized,intensive,wrap‐aroundservices,support,andsubstance‐abusetreatment.

TheMarinChronicAlcoholwithJusticeInvolvementProjectincludesaninitialthree‐personpilot.Currentfundingis$300,000andestimatedcostsforone,seven‐monthpilotare$149,161.

MissionStatementThemissionoftheMarinCAJIProjectistobreakthecycleofchronicalcoholism,justicesysteminvolvement,andhomelessnessinordertoimprovethequalityoflifeforindividualparticipants,reducecostsassociatedwithchronicalcoholism,andpromotepublichealthandsafety.

MarinCAJIProjectGoals• Reduceoverallcoststothecommunityandthecriminaljusticesystemrelatedtopersons

sufferingfromalcoholismandchronichomelessnesswhocyclethroughemergencycareandthecriminaljusticesystemrepeatedly(hereinafter“participants”)

• Improvethequalityoflifeforindividualssufferingfromchronicalcoholismandhomelessnessbyfurnishingintensivecasemanagement,andassertivecommunitytreatment,usinganintegrated,multidisciplinaryapproach

• Assistparticipantsinmovingtowardsobrietybyofferingevidence‐basedtreatmentasanalternativetorepetitiveincarceration

• Assistparticipantsinobtaininghousing• Improvethecriminaljusticesystem'sabilitytoidentify,assess,evaluateandtreat

personssufferingfromalcoholismandchronichomelessness• Reducerecidivismamongparticipants• Safeguardparticipantsandthecommunitybystructuringinterventionsthatreducethe

numberofpersonssufferingfromalcoholismandchronichomelessnessonthestreet

InadditiontotheabovekeycharacteristicsidentifiedbyHomeBase(2013),thestudyalsodiscussesAssertiveCommunityTreatmentTeams.HomeBasedefinestheseteamsas,“multi‐disciplinaryteamsthatprovidearangeofbehavioralhealthandsupportservicestoclientswithseverementalillnesseswhiletheyareinthecommunity”(2013,Appendices,p.2).

MarinCountyseekstocreateaninnovativehybridoftheAssertiveCommunityTreatment(ACT)Teamapproachbyblendingitwithexistingcollaborativecourtstructuresandapplyingthishybridmodeltothetreatmentofthosesufferingfromchronicalcoholismandjusticeinvolvement.Itdoessobyusinganexistingresource–theMarinCountyForensicMultidisciplinaryTeam(FMDT)–toassistinprovidingwrap‐aroundservices,brainstormingcreativesolutionstoissuesthatarise,andbyhelpingtosupporttheparticipantsinthecommunity.

Otherkeyadditionstothishybridmodelinclude: RecoveryCoaches–workmostcloselywithparticipantstoprovideintensive,

individualizedtreatment,one‐on‐onesupport,andcasemanagement

SocialWorker–whilerecoverycoachesworkdirectlywiththeclienttomaintainsobrietyandprovidesupport,thesocialworkerassistsinslightlybroaderdutieslikesecuringbenefits,andobtainingmedicalandpsychiatricrecordsfordatacollection

IntegratedProgramCoordinator–overseesandcoordinatesallaspectsoftheproject,facilitatingcommunicationbetweenallpartiesonalllevels,monitoringdata,andsupervisingcertainpersonnel(FMDTSocialWorker;FMDTIntern)

PROJECT STRUCTURE – COURT TEAM ThestructureoftheMarinCAJIProjectusestwointerconnectedteams,aCourtTeamwhichconsistsoftheJudicialOfficer,DistrictAttorney,DefenseCounsel,Probation,RecoveryCoach,andProjectCoordinator,andanAssertiveCommunityTreatmentorACTTeamwhichconsistsoftheMarinForensicMultidisciplinaryTeam(FMDT),FMDTSocialWorker,andtheProjectCoordinator.TheCourtTeamconsistsofpartieswhowillattendcourtappearanceswithparticipants:JudicialOfficerTheJudicialOfficerpresidesoverallcourtproceedingsandinteractsdirectlywitheachparticipanttoestablishrapport.TheCourtholdsparticipantsaccountable,andutilizesitspositiontoprovideaffirmation,acknowledgementofprogress,andmotivationtoparticipants.Italsoimposessanctionsorgivesrewards,asappropriate.DistrictAttorneyTheDistrictAttorney(DA)verifiesthresholdeligibilityfortheSerialInebriateProgram(sixormorealcohol‐relatedcontactsina12‐monthperiod)andplaysanintegralroleinmonitoringparticipants’progressthroughouttheprocess.TheDAisalsoamemberoftheFMDT..PublicDefenderIDefenseCounselDefensecounselmeetswithprospectiveparticipants,explainsprojectprocedures,andplaysanintegralroleinmonitoringparticipants’progressthroughouttheirparticipation.ThePublicDefenderisalsoamemberoftheFMDT.ProbationProbationattendscourtproceedings,assistswithmonitoringparticipantswhileinthecommunity,andisalsoamemberoftheFMDT.RecoveryCoachesRecoveryCoachesworkcloselywithparticipantsbothin,andoutsideof,court.Theyareparticipants’closestalliesduringthisprocess,providingintensivepersonalandclinicalsupport.Theproposedbudgetincludestwohoursofcontact,fivedaysperweek,foreachparticipantandhis/herrecoverycoach.RecoverycoachesarepartoftheCourtTeam,reporttotheProgramCoordinators,andattendFMDTmeetingsoncepermonthtobrieftheFMDTonprogress,plans,andissues.ProjectCoordinatorTheProjectCoordinatorattendsCourt,recordsproceedings,participatesindiscussionsregardingsanctionsandrewards(wherenecessary)collectsdata,andworkscloselywiththeJudicialOfficertomonitorprojectprogress,concerns,andpossibleissues.(AdministrationcostsarekepttoaminimumwithProjectCoordinatorutilizingonly6.26%ofthetotalestimatedbudget.)

FMDTSocialWorkerTheFMDTSocialWorkerworkscloselywiththerecoverycoachtoprovidealower‐costsupplementfortasksnotrequiringsubstance‐abusecounselingexpertise.Thisincludes,butisnotlimitedto,helpingparticipantsobtainbenefitslikePublicAssistance,SocialSecurity,andhealthbenefits.Thesocialworkerisalsoresponsibleforcompilingcomprehensivemedical,psychiatric,andtreatmentrecordsforthepurposesoftreatmentanddatacollection.Inaddition,theFMDTSocialWorkerwillassisttheFMDTLeadershipTeamindraftingpoliciesandproceduresandwillberesponsibleforseekingadditionalfundingthroughthecreationofgrantproposals.TheFMDTSocialWorkerreportstotheProjectCoordinator.

PROJECT FLOW – PHASE I

Thecourseoftreatmentinthisprojectisdividedintofourphases:1)Arrest‐Jail‐CaseDisposition;2)DetoxificationandNeedsAssessment;3)InpatientTreatment,and4)OutpatientTreatment.Aftersuccessfulcompletionofoutpatienttreatment,theunderlyingcriminalcasewillbedismissed.

PhaseI‐‐Arrest‐Jail‐CaseDispositionEstimatedTimePeriod–5to10days

General: DuringPhaseI,theproposedparticipantisarrested,identifiedaseligibleforMarinCAJIatarraignmentbytheDA,andissentencedtoaminimumof180dayswithCAJIprobationtermsandconditions.AwaiverofGT/WT(good‐time/work‐time)creditsmaybeapartofhisorherpleaagreement.

MarinCAJIProject,theparticipantwillbeassignedarecoverycoachwhowillactashis/herdedicatedrecoverycoachthroughouttheprocess.Theparticipantwillbereleasedfromcustodytohis/herrecoverycoachandtransportedtoTheVinefordetoxification.Duringthedetoxificationperiod,theparticipants’immediatemedicalandpsychiatricneedsareattendedto,andgroundworkislaidbyboththerecoverycoachandsocialworkerforobtainingallpossiblebenefits.Acomprehensive,individualizedassessmentofeachparticipant’sneeds.isalsoperformed.

hisorherimmediatemedicalandpsychiatricneedsareaddressed,theparticipantisreadytobegin90daysofresidentialorinpatienttreatment.TheparticipantwillalsoappearincourttwicepermonthsotheCourtcanmonitorprogressintreatment.

MicheleR.Berrong,J.D.joinedtheCountyofMarinin2005.MicheleinitiallyworkedasaDeputyPublicDefenderandherassignmentsincludedthecollaborativecourts(AdultDrugCourt,Proposition36Court,andS.T.A.R.Court)mentalhealthconservatorshiplitigation,post‐convictionrelief,misdemeanor,andjuvenilelitigation.MicheleparticipatedinhomelessoutreachinpartnershipwithLegalAidofMarinwhileatthePublicDefender’sOfficeandalsodevelopedtrainingmaterialsonpost‐convictionrelief.SheattendedlawschoolattheUniversityofSanFrancisco.In2009,MichelemovedtotheMarinCountyPublicGuardian’sOfficeinapart‐timecapacitytopursueaMastersDegreeinPsychologywithaDepthEmphasisatSonomaStateUniversity.SheexpectstograduateinAugust2013.MichelehasservedastheLPSConservatorshipInvestigatorforthePublicGuardian’sOfficeforthepastfouryears,workingexclusivelywiththeseverelymentallyillandduallydiagnosed.Forthepasttwoyears,Michelehasco‐chairedandcoordinatedtheMarinCountyForensicMultidisciplinaryTeam,hasbeenapartoftheMarinChronicAlcoholwithJusticeInvolvementProjectsinceitsinception,andwasinstrumentalindraftingtheBusinessPlan.Ms.BerrongwasrecentlyadmittedtotheClinicalPsychologydoctoralprogramattheCaliforniaInstituteforIntegralStudies.Shebeginsdoctoralstudiesinthefallof2013.
