OBJETIVO GENERAL:

Realizar, de acuerdo al consentimiento informado de los padres de familia; acciones preventivas (profilaxis y aplicación de barniz fluorurado (pendientes), toma de radiografías y aplicación de sellantes fosas y fisuras); que contribuyan a disminuir el riesgo de caries y las afecciones gingivales presentes en la cavidad oral de los niños de la Escuela 15 de Septiembre.

Se desarrollará la III fase odontológica, de acuerdo a los requerimientos de privacidad, control de infecciones y bioseguridad mínimos que se deben de tener para este tipo de acciones. Razón por la que será llevado a cabo en la Clínica Odontológica CEO Mandujano (toma de radiografías) y Clínica Odontológica del Centro de Salud de San Francisco (profilaxis, fluoruración y aplicación de sellantes de fosas y fisuras), respectivamente.

JUSTIFICACIÓN: Debido al estado crítico de salud oral que presenta la población evaluada. Se decidió a bien realizar la Fase III del Proyecto, la cual consiste en:


Theworkareawasinstalledinoneoftheclassroomsoftheschool.Supportwasprovidedbytheschoolteachersandmothersofthecommunity,staffofWilliamJewellCollege,supportstaffoftheMunicipalHealthCenter,aSocialServiceDentistryInternandvolunteers.TherequiredsupplieswereprovidedbytheWilliamJewellCollege,accordingtorequestspreviouslyprovided.Oralbrushingwasperformedonallchildren,priortothedentalevaluation.ThiswasmadepossiblewithdonatedtoothbrushesandtoothpasteprovidedbyWilliamJewellCollegeandtheDr.Mandujano’sOdontologyClinic.Likewise,theDentalCliniccreatedtoothbrushholdersusingreusablematerialsincollaborationwithAstridEscobar,studentfromtheUNAH-CURLANursingdepartment,sothateachchildhasabrushandtoothpasteatschool.Thisenablesestablish-mentoforalhygienehabitsafterrecesseachschoolday.Oraldiagnoseswereperformedonadentalclinicalrecliner,withdisposablematerialsforeachpatient,inasuitablespaceandwithadequatelighting.Adentalrecordwascreatedforeachstudent.Attheendoftheday,ameetingwasconvenedwithDr.LoriWetmore,EngineerSarahiZerón,EngineerVilmaCastañeda,EngineerEvangelinaTrejoandDr.LilianaMatuteMandujano.Apreliminaryreportofthestateoforalhealthofchildrenandrecommendednextstepswereelucidated,withthesekeyobservations:

1. RecognitionthatallthechildrenwereevaluatedbyaPediatricDentalSpecialist,withthesupportofthedentalclinicteam.

2. Firststepsshouldincludepreventiveactionsanddecreaseofanycariogenicloadwithdentalprophylaxis(cleanings),applicationoffluoridevarnishandcompletionofradiographs(incasesthatrequireradiographrecords).

3. Step2activitieswillrequireadentalunit,soitwassuggestedthattheybecarriedoutbythestaffoftheDentalClinicoftheVOAE,attheHealthCenterofSanFrancisco,withtheassistanceoftheDentalStaffthatworkthere.

4. Realizationofthetreatmentsdescribedabove,willrequireplanningaccordingtothenumberofstudents.Thiswillrequiretwodays,withAMandPMsessions.

5. AlistofthesuppliesandmaterialsnecessaryforthecompletionofPhaseIIIwasprovided,withtherealizationthatsomematerialshavealreadybeenprovided.

6. Duringthemeeting,someculturalinsightwasprovided.Theparentslackeducationondentalhealth,sotheydonotunderstandtheimportanceofmaintainingthehealthofbabyteeth.Fromtheirperspective,thechildrenwilllosetheseteeth,sotheydonotseetheneedforregulardentalcare.

- Schedulingofdatesforcompletionofpreventiveactions(dentalprophylaxisandapplicationsoffluoridevarnishtoallstudents),bythestaffoftheDentalClinicoftheVOAEattheSanFranciscoHealthCenter,Atlántida

- Somestudentsrequireradiographs,whichcanbecompletedattheSanFranciscoHealthCenter.Requiredoralperspectiveswillberecorded.

- Subsequently,theremainingpreventive,restorativeandcontrolactionswillbecarriedoutinthefollowingphases.

- Itisnecessarytoraiseawarenessamongteachers,parentsandstudentsabouttheimportanceoforalhealthinthedevelopment,growthandmaintenanceofgeneralhealth.Therefore,check-upsshouldbeestablishedovertimetoperformoralcleanings(every3,4or6months)oraccordingtothecariogenicriskofeachpatient.

OBJETIVO GENERAL:





Whilewaitingfortreatment,childrenparticipatedindrawingactivities,painting,crosswordpuzzles,withteachersupervision.Studentswereservedrefreshmentsthatincludedahealthyfruitcocktailsnack.ThechildrenweretransportedfromthecommunityofBocadelTorotothedentalhealthclinic(CESAMO)inSanFranciscomunicipalityinasmallbusprovidedbyCURLA.AttheconclusionofPhase2,ameetingwithEngineersCastañedaandZerónwasaccomplishedtodeterminethenecessarymaterials,budgetandtentativedatesforthePhase3.ConclusionsThetreatmentswereperformedwithaseptictechniqueandbiosecuritytoavoidinfectionsorcontamination.TheseactionsrequiredtheuseofthedentalfacilitiesatCESAMO.Thirtystudentsweretreated,ages3-11yearswithanaverageageof8years,52%weremaleand48%werefemale.Amongthefemalestudents,71%werenothealthyand29%werehealthy.Amongthemalestudents,80%werenothealthyand20%werehealthy.Oraldiagnosesweredeterminedfor30students.

OBJETIVO GENERAL:





Step2:ApplysealantstopitsandfissuresDuringthelastcommunitydevelopmentvisitApril25-27,2018,theobjectivesforthisphaseweredetermined,inaccordancewiththedevelopmentplan.Thelogisticsandadequatepreparationofequipment,instruments,materialsandsuppliesnecessaryforthedevelopmentofphaseIIIweredetermined.OnDecember3,2018,beginningat8:00am,thechildrenweretransportedtotheMandujanoOdontologySpecialtyCentertocompleteoralradiographs.TheHealthCenterinSanFranciscomunicipalityisnotequippedwithradiographequipment.Theradiographswereprovidedwithoutcharge.OnDecember4-5,2018,beginningat6:00am,theneededequipmentwastransportedtotheHealthCenterinSanFrancisco.Thefollowinginterventionswerecompleted:

1. Dentalprophylaxisforallthechildren2. ApplicationoffluoridevarnishforchildrennotcompletedinPhaseII3. Applicationofsealanttopitsandfissuresinaccordancetocariesriskforeachchild4. Eliminationofpartialcariesandcompletionofprovisionalfillingsinthepatientsthat

requiredthistreatmentThenecessarysupplieswereprovidedbyUNAH-CURLA,inaccordancewiththelistsubmittedearlier.Itshouldbenotedthatallsuppliesandmaterialsareinaccordancewithscientificevidenceandareofthebestqualityavailableinthemarket.Radiographswerecompletedforatotalof22children,andatotal16childrenreceivedpreventivecareofsealantandfluoridevarnishapplication,asneeded.Allprophylaxiswascompletedafterremovaloftartar.Atthissametime,fluoridevarnishapplicationwasappliedtothetwopendingpatients.Removalofpartialcariesandcompletionofprovisionalfillingswerecompletedinthreepatients.Duetothehighcariogenicactivitythatwaspresented,continuedtreatmentformanagementofcariesisneededtopreventprogression.Whilethechildrenwaited,activitieswereprovidedthatincludeddrawing,games,andmovies.TheseactivitieswereprovidedbyEngineerVilmaCastañedaandInternGemmyRivera.Inaddition,ahealthysnackwasgiventoeachofthechildrenthatconsistedofpopcorn,wheatbreadsandwiches,Jamaicajuiceandlunch.ThechildrenweretransportedtoandfromBocadelTorotothecentersfortreatmentinthesmallCURLAbus.AttheendofWednesday,December5,InternYeimyRiveraindicatedthatalltheremainingsupplieswouldbeleftwiththeHealthCenterasadonation.Allphasesofthedentalhealthprojectwerefinalizedwiththisthirdphase.Thisthirdphaseproceededasdescribedinthisreport,withdeliveryofallmaterialsandsuppliestoDr.EstherAlejandraCastillo,OdontologyIntern,HealthCenter,SanFranciscoMunicipality.


Itisworthnotingthat:1. Theparentsofeachfamilygavetheirinformedconsentforthechildrentoparticipatein

allthreedentalpreventionphasesoftheproject.2. Allofthetreatmentswereprovidedbyaspecialistinpediatricodontology,withquality

care,followingguidelinesforasepsisandbiosecurity,topreventanytypeofinfectionand/orcontaminationbetweenthepatients.

3. TheactionsthatrequireduseofthedentalunitandradiographequipmentwasaccomplishedbythedentalclinicpersonnelattheHealthCenterinSanFranciscomunicipalityandtheMandujanoOdontologySpecialtyCenter,asdescribedintheplansdevelopedinphase2.

4. Thecompletionsofthetreatmentsdescribedpreviouslyinthisreport,accordingtotherequirementsforthenumberofchildrentreatedoverthreedays,inthemorningandafternoon(similarinformtowhatwasaccomplishedinphase2).

5. AdonationofallthematerialswasprovidedtotheHealthCenterinSanFranciscomunicipality.Allsuppliesandmaterialswereaccounted,asdescribedfortheproject.

6. Maintenanceofpreventivecontrolsforallpatientsisneededatleastevery6months,inordertocompletedentalprophylaxisandapplicationoffluoridevarnish.

7. Somepatientsstillrequirerestorativeandrehabilitativetreatments(dentalfillings,rootcanals,extractions),thuscontinuationoftheprojectissuggested.

Sealantapplicationwasprovidedfor16/30patients.Fluoridevarnishwasappliedto pendingpatientsandemergencycarieswereeliminated.(Fillingcompositewas providedbytheMandujanoDentalSpecialtyCenter.)ConclusionsTwenty-twostudentswereprovidedperiapicalradiographsintheagerangeof4-13years,withanaverageageof9years,41%weremaleand59%werefemale.Atotalof49radiographswerecompletedFortheapplicationofsealanttopitsandfissures,16studentsweretreated.Allofthesestudentsunderwentpreviousdentalprophylaxisandatotalof84sealantswereapplied.Twostudentsreceivedfluoridetherapy.Threestudentsrequiredplacementoftemporaryfillings,duetotheircariogenicstatus(donatedbyCEOMandujano),9fillingswerecompleted.Somerestorationsarestillpending(PhaseIV).Fourpatientsrequirerestorativedentaltreatmenttoavoidlossoftheirpermanentmolars.(ThiswasemphasizedbothtoDr.CastilloandMr.CastañedaandMr.Rivera).Inaddition,onepatientrequirespanoramicradiographybecausehepresentedwithseveredentomaxillarydisharmonyandlackoferuptionofseveralofhisteeth,withobviouslossofspace.
