• Historically, LTC facilities have been siloed.
• In the evolving world the health systems need to be interoperable and interconnected via the electronic health record (EHR) in order to break the silos.
Adoption of HIT within LTPAC Spectrum
• Long-term care facilities provide a wide range of services, both medical and personal care, to individuals who are no longer able to manage independently in the community
• HIE in facilities with EHR is FIVE TIMES better than facilities that do not use an EHR or an HIO.
