Intent to Contribute Statement:We will be trialing a small test of change of handing out "Thank you cards" to discharged patient on
the day of discharge. The cards are in printing right now and we are hoping to start as soon as we
get the cards in. The front of the cards have a picture of our "Caritas committee" who consist of
front line staff and management team. The card are signed by all the providers that took care of the
patient and a general message to show our appreciation and thanking them for allowing us to care
for them. In addition, we also have an insert to the thank you asking the patient if there was
anyone they would like to recognize for excellent care during their stay. They are asked to return
the card or leave it at the nurses station inbox for the caritas committee. The "recognized"
individual will be share to our management group so we can recognize the individual for providing
excellent care to our patient. We hope to achieve higher HCAHPS score and increase recognition
of our employee/staff for excellent care.
