are ready to improve your health, our programs are for you.
Programs are free of charge.
We offer a range of programs that can help you with specific
health issues. We also offer programs that can help you quit
smoking, start exercising, or eat a healthy diet.
Partners and support persons are welcome to register and
attend.
Note: Some class spaces may be held for patients of
local MyHealth Team physicians.

Commit to Quit / Cesser de fumer Five 1 to 1.5-hour sessions Commit to Quit is for people who are planning to quit using tobacco. Most people will be ready to
quit using tobacco after Week 4 of the program.
In this program you will:
Learn about support techniques and aids to quit smoking, including smoking cessation
medications.
Develop a plan to quit and work through your plan.
Learn and practice strategies to deal with recovery symptoms and stay tobacco free for the
long term.

COPD Essentials One 1.5-hour session
This program is for people diagnosed with Chronic Obstructive Pulmonary Disease (COPD), or
people looking for more information on COPD.
In this program you will:
Learn what COPD is and how it is diagnosed.
Learn the essential components of managing COPD, including an introduction to COPD
medications, what flare-ups are and how to manage them, steps to stop using tobacco, and
managing anxiety and stress.
What resources and tools are available and where you can find them.

Exercise HealthSense One 2-hour session This program is designed for those new to, or unaccustomed to, exercise who have at least one
chronic health condition.
In this program you will:
Learn the basics about exercise and chronic conditions, including how your condition and
medications affect what you can do safely.
Learn how to develop a suitable exercise program and what’s available in your community to
support you.
Please come in clothes suitable for light movement.

Get Better Together! / En santé... ensemble!
Six 2.5-hour sessions
Get Better Together is for anyone who wants to gain confidence and learn skills to better manage
their health, especially those with a health condition. Support persons are also welcome to register.
Workshops are led by trained volunteer leaders, most of whom have a chronic health condition.
Workshops are also available in French.
In this program you will:
Learn what you can do to take control of your health.
Learn and practice skills to manage symptoms, reduce fatigue, problem solve and set goals.

Living Well with Pain
Six 2 to 2.5-hour sessions This program is for people living with ongoing pain, or people looking for more information on
living with pain.
In this program you will:
Learn about what causes chronic pain and what keeps it active.
Learn physical and mental coping strategies to help relieve pain and improve daily functioning.
Share and learn with others who are also living with chronic pain.
*Note: This program will be delivered over 4 sessions.
Living Well with Pain is also offered at other locations. Additional programs will be available for
Fall 2018.
Heart Health Essentials
One 2-hour session This program is for people diagnosed with a heart condition or at risk for a heart condition. If you
have a family history of heart disease, have high cholesterol, hypertension, or diabetes, this class
is for you.
In this program you will:
Learn what heart disease is, and know how to manage targets for blood pressure and choles-
terol.
Learn steps and strategies for managing risk factors, and improving heart health starting with
how to set a realistic health goal.
Understand the warning signs of heart attack and stroke.

Cardiac Rehabilitation
Cardiac Rehabilitation is a 16-week program for those who have been diagnosed with any type of
heart condition. This includes persons looking to return to better health after a heart attack or
diagnosis of coronary artery disease, peripheral artery disease or other heart condition.
In this program you will:
Learn the level of activity you can safely handle, and work on an individualized exercise
program.
Develop necessary skills and self-confidence to make lasting behaviour changes.
Improve your diet and lifestyle, reduce cardiac symptoms, and reduce stress and anxiety.
Reduce the risk of another cardiac event and keep an already present heart condition from
getting worse.
A referral from your doctor or nurse practitioner is required. There is a fee of $250 for this program.
Financial assistance may be provided based on individual need and the availability of funds. Some
extended health insurance plans cover all or part of the fee. You can ask your doctor about joining
Cardiac Rehabilitation or contact a program site and we will work with your doctor to request a
referral. To register for an information session about the program, please contact one of the sites
below.

Pulmonary Rehabilitation
Pulmonary Rehabilitation is an 8-week program (16 sessions)
for those diagnosed with lung conditions. This includes chronic
obstructive pulmonary disease (COPD), emphysema, chronic
bronchitis and pulmonary fibrosis.
In this program you will:
Learn the level of activity you can safely handle, and work
on an individualized exercise program to improve your
strength, breathing and sense of wellbeing.
Learn to use medications effectively, prevent infections and
get strategies to help stop smoking.
Reduce respiratory symptoms and the risk of an
exacerbation/“lung attack” to slow the worsening of your
condition.
A referral from your doctor or nurse practitioner is required. You
can ask your doctor about joining Pulmonary Rehabilitation or
contact us through the registration line and we will work with
your doctor to request a referral.
