Objectives
� Review interfaces between Dawn AC and the Brigham and Women’s Hospital
� Outline the benefits seen by the medication interface with EPIC
Brigham and Women’s Anticoagulation
Service (BWH AMS)
� Currently have ~3,300 outpatients
� 10 pharmacist on staff
� ~400 patients per pharmacist
� 4 pharmacy student interns
� ~7,000 INR’s dosed each month
� ~250 new patients each month
� Primarily phone based
Daily Tasks
� Call Labs
� Call “not dones”
� Check the medication tab
� Dose INR’s
� Call dosed patient’s
Possible Obstacles
� Over correction
� Decrease in patient reporting
� List may not be all-inclusive
What’s Ahead?
� Integrate Televox: automated reminder calls
� Incorporate more labs into laboratory interface (i.e. Alere)
� Drug-Drug interaction guidelines
� Adding the detailed interaction for each drug
� Texting and emailing through DAWN
