Excursion Monitoring and Corrective Actions
On May 22, 2015, well SM6-28 was placed on excursion for exceedance of upper control limits. Thewell was removed from excursion status on August 3, 2015.
On May 22, 2015, well SM8-28 was placed on excursion status for exceedance of upper controllimits. The well was removed from excursion status on August 3, 2015.
On May 28, 2015, well SM1 0-21 was placed on excursion status for exceedance of upper controllimits. The well was removed from excursion status on July 29, 2015. This well was purgedcontinuously to help mitigate the chloride plume that had impacted this well.
On June 3, 2015, well SM8-5 was placed on excursion status for exceedance of upper control limits.The well was removed from excursion status on July 28, 2015.
On July 10, 2015, well SM8-2 1 was placed on excursion status for exceedance of upper control limits.The well was removed from excursion status on July 29, 2015.
On August 14, 2015, well SM6-24 was placed on excursion status for exceedance of upper controllimits. The well was removed from excursion status on September 3, 2015.
The Mine Unit 6 and 8 shallow monitor wells were removed from excursion status after the areadried up and the water levels in the wells dropped.
A summary of the weekly excursion indicator parameters and laboratory reports are included inAppendix A and Appendix B respectively.
Mine Unit 6 and 8 Future Water Ouality Monitorin2
In order to get a better understanding of the water chemistry in the English Creek area of Mine Units6 and 8, Crow Butte has implemented the following sampling plan:
1. Collect ten isotopic uranium samples from Mine Unit 6 and 8 wellfields that have notundergone restoration (RO) treatment.
2. Collect seven isotopic uranium samples from Mine Unit 6 and 8 production zone monitorwells that have not been impacted by mining fluids (e.g. no evidence of increasing trends inexcursion indicator parameters).

3. Collect twenty four isotopic uranium samples from Mine Unit 6 and 8 shallow monitoringwell wells that have been on excursion or with increasing excursion indicator parameterstrends.
4. Collect seven isotopic uranium samples from Mine Unit 6 and 8 shallow monitoring wellsthat have not been on excursion and which do not show evidence of increasing or unusualtrends in the excursion indicator parameters.
5. Collect nine isotopic uranium water samples and three uranium and radium sedimentsamples from English Creek. The samples include the headwaters/spring areas, midstream,and downstream along with the downstream impoundments.
6. Collect four quarterly uranium samples from twenty four shallow monitor wells within theEnglish Creek area.
7. Collect four core samples from four different locations within Mine Units 6 and 8. Testingof these samples will be based upon the results of the isotopic uranium sampling.
