As the newly appointed
Chairman of the Hospital, I
strongly advocate the delivery
of quality and sustainable care for
every patient that we serve. With a
rapidly ageing society, there is a need
to look beyond the immediate care of
our patients in the Hospital and ensure
that they are well supported at home
as well. We also need to support and
empower their caregivers who play
a pivotal role in the whole patients
support network system to provide
optimal care for their loved ones.
Capabilities and Capacity
In response to the changing
demographic and health care needs of
our Singapore population, the Hospital
has enhanced its capabilities over the
years in the following areas: dementia
care, stroke rehabilitation and palliative
care as well as home care. The newly
operational Community Health
Centre will also support the primary
healthcare network by providing
Diabetic Retinal Photography and Foot
Screening. The Hospital’s Caregiver

Training Programme has also been
expanded to provide foreign domestic
workers and elderly caregivers with
customised resources and training
programmes.
Hospital Expansion
The Hospital will see the completion
of an Administrative Block, an Annex
Block and the addition of 10 isolation
beds by the end of 2017. This expansion
will also increase the size of our current
rehabilitation gymnasium as well as
expand our training capabilities with
the addition of a larger lecture theatre
and more training rooms.
Caring with Passion
The Hospital has been recognised for
its commitment to excellent patient
care at the Singapore Health Quality
Service Award (SHQSA), which saw
293 staff honoured for their dedication
to quality care and excellent service.
A total of 17 of these staff were also
presented with the STAR award for
going the extra mile to care for our
patients.

I would like to take this opportunity to

Conclusion
I would like to thank my fellow

Ms Mardiana Binte Sabtu, our Senior
Medical Social Worker, was presented
with the 2016 Healthcare Humanity
Award along with Mr Stephen Tan, our
Senior Nurse Manager, who received
the 2016 Nurses’ Merit Award from our
Health Minister, Mr Gan Kim Yong.
These national awards are a testament
of our dedication towards excellent
patient care.
We would also like to thank our
dedicated volunteers, who contributed
their precious time in various ways, be
it befriending our patients, organising
group-based activities such as Bingo,
organising Karaoke sessions or
conducting art and craft workshops.
The Hospital has also introduced
animal assisted-therapy by partnering
Cat-Assisted Therapy Singapore and
Save Our Street Dogs, to enhance
patients’ social and emotional well-
being.

Moral Society is “To Serve
Mankind”. The Ang Mo Kio-
Thye Hua Kwan Hospital is part
of this service to all our people in
Singapore. In the process of helping
anyone who needs the Hospital’s help,
we also ensure that we fully respect
each person’s race, colour, creed and
religion.

Ms Cheah Sheau LanMember
Ms Cheah Sheau Lan is a Chartered Accountant registered with the Institute of Singapore
Chartered Accountants. She is a retired corporate executive and currently sits on the board
and/or committee of voluntary welfare organisations including Thye Hua Kwan Moral
Charities, Singhealth Foundation and the Tsao Foundation.
She was previously the General Manager of Corporate Development at Pan-United
Corporation Ltd, a SGX Listed company. Prior to that, she was an Assistant Director in a
Singapore based private equity firm specialising in mezzanine financing for growing
companies in the Asian region.

Mr Chew Heng ChingChairman
Mr Chew Heng Ching is a prominent member of the Corporate & Business Community
in Singapore developing and promoting best corporate governance practices. He is the
Founding President of the Singapore Institute of Directors (SID) and Chairman of its
Governing Council from 1998 – 2009. He was a former Chairman and Board member of
the Singapore International Chamber of Commerce from 1996 to 2014. Today, he sits on the
board of various publicly listed companies.
In public life, Mr Chew was a former Deputy Speaker of Parliament and Member of Parliament
from 1984 – 2006. He chaired various parliamentary committees and was Coordinating
Chairman for all PAP Town Councils in Singapore. He also serves as a volunteer on the board
of various charities. He is currently Vice Chairman of Thye Hua Kwan Moral Charities Ltd.
Mr Lee Kim Siang BBM(L), BBM, PBMMember
Mr Lee is the Chairman for Thye Hua Kwan Moral Society and Thye Hua Kwan Moral Charities
Limited, a voluntary welfare organisation that runs more than 70 centres and services
islandwide.
Mr Lee is currently a member of the Care and Share Steering Committee, a national fund-
raising and volunteerism movement led by the Community Chest since 2013 under the
Ministry of Social and Family Development. He is also a member of the Responsible Gambling
Forum, formed by the Ministry of Social and Family Development since 2013, with the aim of
promoting responsible gambling practices across all forms of gambling in Singapore.
Er Chang Meng Teng JP, BBM, BBM(L), PPA(P), PBMVice Chairman
Er Chang is a registered licensed Professional Engineer of PE Board. He left the former
Public Utilities Board in 1979 to form and head Squire Mech Pte Ltd, a Mechanical &
Electrical engineering practice. He is very active in both the engineering fraternity and civic
organisations. He was the President of Institution of Engineers, Singapore and the Society of
Project Managers.
He had served on many committees and statutory boards of Government Ministries. Er Chang
is currently a Board Member of the Energy Market Authority and a Fellow of the Singapore
Academy of Engineering.
Mr Robert P.C. TockVice Chairman/Hon. Secretary
Mr Robert Tock is a practising advocate and solicitor who is presently the sole proprietor of
the law firm, Rodrigo, Tock and Wilson.
Mr Tock, also sits on the board of Thye Hua Kwan (THK) Moral Charities as Hon. Secretary
and THK Nursing Home as Vice Chairman. He is also the Chairman of the Management
Committee of various homes, namely Moral Home for the Intellectually Disabled, Moral
Home for Disabled Adults, Moral Welfare Home, Angsana Home and Banyan Home.
Mr Zulkifli Baharudin BBM, PBMHon. Treasurer
Mr Zulkifli Baharudin is the Executive Chairman of Indo-Trans Corporation, a logistics and
supply chain company across Indo-China. Mr Zulkifli Baharudin is also Singapore’s Non-
Resident Ambassador to the Republic of Uzbekistan and the Republic of Kazakhstan.
Mr Zulkifli is an Independent Director of Singapore Post Limited, Ascott Residence Trust
Limited and Trustee of the Singapore Management University.

Dr L. JayaramMember
Dr Jayaram Lingamanaicker is one of the leading cardiologists recognised for his expertise
in performing Balloon Angioplasty and stenting. He has more than 20 years of experience in
Clinical Medicine and Cardiology.
He also served as a Consultant Interventional Cardiologist based at Changi General Hospital,
where he later became Senior Consultant and Head of the Division of Cardiology. Dr Jayaram
practices at Mount Elizabeth Hospital currently and is also a senior lecturer at National
University of Singapore.
Prof Philip Choo BBM, PPA(P), PBSMember
Professor Philip Choo is the Group Chief Executive Officer for National Healthcare Group.
Professor Choo is well-recognised as a Geriatrician with more than 20 years of experience.
He was awarded TTSH Doctor of the Year in 1996.
Professor Choo currently serves on several Boards which includes Johns Hopkins Singapore
International Medical Centre, Ren Ci Hospital, Integrated Health Information Systems Pte Ltd
and The Courage Fund Limited. He also sits on various Ministry of Health Committees and is
a member of the Lee Kong Chian School of Medicine Governing Board.
Mr Chew Hai ChweeMember
Mr Chew Hai Chwee spent more than 27 years working for Asian and US MNCs listed on
the Singapore Exchange (SGX), NYSE, and NASDAQ in USA where he was responsible for
various business functions and responsibilities.
He is on the council of Singapore Red Cross and is Chairman of the Committee for Humanitarian
Assistance and International Relief of Red Cross Singapore. Mr Chew also served as CEO of
Bright Vision Community Hospital and was on the Board of Advisors for Kemin Asia Pacific
for 14 years. Mr Chew owns a consultancy and investment firm currently.
Dr Tan Tiong HarMember
Dr Tan Tiong Har who was awarded the Colombo Plan Scholarship for Medicine is a
pulmonologist who recently retired from private practice at Gleneagles Medical Centre. He
has more than 40 years of experience in respiratory medicine. He was previously Consultant
Physician, (Deputy Head) Department Of Medicine IV and subsequently the Head of
Department of Tuberculosis Control at Tan Tock Seng Hospital.
He then ventured into Private Practice while serving as a Visiting Consultant at Changi
General Hospital and TTSH.
He is also the Medical Director at Melta Home Services.
Er Ong Ser HuanMember
Er Ong Ser Huan has over 45 years of experience as a Professional Engineer and also serves
in the disciplinary committees of Singapore Law Society and Singapore Medical Council. He
was President of the Institution of Engineers Singapore (1998-2000), and was elected as
Chairman of Asean Federation of Engineering Organisations.
Er Ong also serves as the Chairman of the Serangoon Moral Family Service Centre, Vice
Chairman of the Thye Hua Kwan Moral Society and Banyan Home.
Mr Goh Geok Cheong PBMMember
Mr Goh Geok Cheong is the Managing Director of ABV Engineering Pte Ltd. He has more than
35 years of experience in civil engineering and was involved in several patent inventions and
innovation development.
Mr Goh is also the Chairman for Kaki Bukit Community Centre and the ambassador for the
Singapore Corporation of Rehabilitative Enterprise. Mr Goh is also member of the Institution
of Engineers Singapore.

Corporate Governance Statement
Board of DirectorsThe Board’s role is to provide overall leadership to the strategic direction of the Hospital’s
operations.
In line with corporate governance, the Hospital has renewed the Board members in this
financial year. We have brought on board five new Board members from different professions
while we have seven Board members who has served the Hospital for more than 10 years
retiring.
All the Board members of the Hospital have accepted their roles without remunerations
and are committed to serve the Hospital to bring to our patients and the community better
healthcare services.
Under the guidance of our new Board members with Mr Chew Heng Ching as the Chairman,
the Hospital has undergone more stringent review of processes and is in compliance with
Corporate Governance. In performing its duties, the Board of Directors have maintained
effective working relationships with the Hospital’s Management.
Conflict of Interest PolicyAll Board members, employees and volunteers of the Hospital are required to read
and understand the conflict of interest policy and make full disclosure of any interests,
relationships and holdings that could potentially result in a conflict of interest. When a
conflict of interest situation arises, person involved shall be abstained from participating in
the discussion, decision making on the matter.
Whistleblowing PolicyThe Hospital is committed to uphold our staff’s integrity and ethical behaviour by fostering
and maintaining an environment whereby employees can act appropriately without fear of
retaliation.
To maintain these standards, the Hospital has in placed a whistleblowing policy that allows
employees who have concerns about suspected serious misconduct to come forward and
express these concerns without fear of punishment or unfair treatment.

Ang Mo Kio – Thye Hua Kwan Hospital is a community hospital providing rehabilitative and sub-acute
care. Using a patient-centered approach, our healthcare team provides a personalised and comprehensive
plan for each patient at AMK-THKH. We strive to help our patients achieve independence and assist
them in their integration back into society.
The Hospital works closely with Restructured Hospitals (RHs) to refer patients to us. About 72% of our
patients are referred by Tan Tock Seng (TTSH), Singapore General Hospital (SGH) and Khoo Teck
Puat Hospital (KTPH).
About 80% of the patients admitted to AMK-THKH are above 65 years of age. The Hospital cares for
patients with a range of medical conditions, majority of whom require rehabilitation after fracture, stroke
and spinal conditions.

The Hospital has continued to expand its
scope of services to address the needs of
the aged care sector.
Palliative Care Unit
The Hospital’s Palliative Care Unit seeks to
meet patients’ and their families’ physical,
emotional and psychosocial needs so as to
maximise their quality of life. The palliative
care unit cares for terminal patients with
cancer as well as non-cancer conditions.
The programme has helped 71 patients
since January 2015.
Activities-based Group Therapy
Programme for Dementia Patients
The Hospital obtained a two-year funding
from the Tote Board to develop and
implement an activities-based group
therapy programme for dementia patients.
This programme offers arts and craft,
music therapy and gardening therapy
for patients with dementia to engage
them in meaningful activities. More than
140 patients have benefitted from this
programme since implementation.
Renal Care
Renal care is a growing area of need for
elderly patients. The management of
these patients’ medical conditions is more
complex and challenging. The Hospital
collaborates with B. Braun and KTPH by
referring inpatients who require dialysis to
receive their treatment at B. Braun Centre
while under the rehabilitative care at AMK
- THKH. The Hospital has managed 442
such patients since 2013.
The Hospital also started the Continuous
Ambulatory Peritoneal Dialysis/
Automated Peritoneal Dialysis programme
to train our nurses in caring for patients
requiring Peritoneal Dialysis (PD). The
training programme has certified 150 nurses
as of FY 2016.
Respiratory & Ventilator Care
This specialised unit caters to patients (e.g.
those with amyotrophic lateral sclerosis,
spinal cord injuries or have prolonged
Intensive Care Unit stays) who require
respiratory support which includes the use
of mechanical ventilators or cough-assisted
machines. Fifty-seven patients have
benefitted since the implementation of this
programme in 2014.

Expansion Programme
The Hospital is currently in the second phase of its
Expansion Programme which seeks to alleviate the national
bed shortage, address the increasingly complex needs of
our patients and improve the Hospital’s training facilities.
The first phase, which was completed in November 2014,
saw the Hospital’s bed capacity increase by 110 to 360 beds.
This new South Wing Ward, was designed specifically for
geriatric patients.
The next phase of the expansion consists of the building
of a new six-storey administration block and a four-
storey Annex block. These are expected to be completed
by December 2017. These new blocks will also have
10 isolation beds. Our training capabilities will also be
enhanced with the addition of staff training rooms. The
Hospital will also expand the therapy gym to create more
space for our patients as they carry out their rehabilitative
exercises.

As part of the Central Regional Health System (RHS), AMK-THKH works
closely with various partners like TTSH, KTPH, National Healthcare Group
Polyclinics (NHGP) to deliver quality care for our patients.
Stroke Management Care Pathway
The programme, managed in partnership
with TTSH & National Neuroscience
Institute (NNI) seeks to facilitate the
seamless transfer of stroke patients to AMK-
THKH, as well as provide training for the
Hospital’s clinicians to better manage stroke
patients. The programme has facilitated the
care of 69 patients as of FY 2016.
Hip Fracture Care Pathway
AMK – THKH has also initiated the Hip
Fracture Care Pathway with TTSH and
KTPH. This care pathway, started in 2012,
facilitates the timely transfer of patients
with hip fractures from the acute hospital
to AMK - THKH. Qualifying patients
also benefit from more coordinated and
subsidised outpatient rehabilitation after
they are discharged from our Hospital.
Some 423 patients benefitted from this
programme as of FY 2016.
Caregiver Training Programme
The Caregiver Training Programme aims
to help appointed caregivers provide
patients with the appropriate care upon
their discharge home. These training
programmes are customised for each
patient based on their needs and are
available to caregivers of all our patients.
Foreign Domestic Worker
Training Programme
As an extension of its current Caregiver
Training Programme, the Hospital has been
conducting customised caregiver training
programmes for foreign domestic workers.
Learning materials which are translated
into Burmese and Bahasa Indonesia help
domestic helpers from Myanmar and
Indonesia better understand the intricacies
of caring for our elderly. The initiative
has helped 40 caregivers complete their
training as of FY 2016.
AMK – THKH Care Coordination
with National Healthcare Group
Polyclinics (NHGP)
Under this initiative, AMK – THKH’s
Healthcare team identifies suitable patients
for referral to Ang Mo Kio Polyclinic for
management of their chronic diseases after
discharge. This aims to create a smooth
transition back to the community. This
ensures that continuity of care with our
primary care partners is in place for our
patients. The programme has reached out
to 154 patients as of FY 2016.

Day Rehabilitation Centre
Our Day Rehabilitation centre (DRC)
provides outpatient rehabilitation to
patients who require active rehabilitation
after they are discharged from the Hospital.
A total of 2,407 patients were enrolled in
DRC in FY 2016.
To help patients integrate better into the
community, DRC conducts a Community
Rehabilitation Programme in partnership
with Yio Chu Kang Community Centre
(YCK CC) in 2013. This partnership enables
patients to utilise YCK CC’s Active Ageing
Corner” to maintain fitness and improve
their physical condition. About 225 sessions
have been conducted as of FY 2016.
With the support of AIC’S Outing for the
Elderly Fund, DRC also organises regular
social outings for the elderly with mobility
difficulties to remain connected to society
by visiting attractions such as Jurong Bird
Park, Gardens by the Bay, Singapore Zoo
and River Safari.
Home Healthcare Services
This programme, funded by Tote
Board Community Healthcare Fund,
is targeted at homebound patients
who require continued health care after
they are discharged back home. The
services offered include Home Medical,
Home Nursing and Home Therapy. The
programme has managed around 585
patients as of FY2016.
As an extension of the programme, AMK-
THKH also collaborates with TTSH Post-
Acute Care at Home (PACH) team, to
provide a seamless transition for patients,
from the hospital back to their own home.
Under this partnership, patients whose
condition have stabilised will be taken care
of by the AMK-THKH Home Healthcare
team while TTSH PACH continues to focus
on patients requiring more complex care
needs. A total of 13 patients have been
referred to this programme as at FY 2016.
To further support Hospitals (Restructured
& Community) with their discharges and
reduce the caseload of some Specialist
Clinics, our Home Healthcare programme
has trained our staff to be able to provide
the following clinical services:
i. Doctors – Change PEG, Tracheostomy,
Palliative Care
ii. Nurses – Male Catheterisation, Manage
Dementia and Palliative patients, VAC
dressing, Venepuncture, ACP
We administer schemes such as the
Seniors Mobility and Enabling Fund (SMF)
to help needy patients obtain subsides for
the purchase of medical consumables. As
of FY 2016, 107 patients have benefitted
under SMF scheme.

ACTION Team Care coordination
Initiated by the Agency of Integrated Care
(AIC) in November 2012, this programme
facilitates patients who are discharged
back to their homes and community. The
team assists by identifying patients’ care
needs, raising referral and coordinating care
services prior to discharge. Upon discharge,
our care coordinators also follow up with
patients to ensure that the community
support services are in place to care for the
patient.
This initative also helps to reduce the
number of re-admissions among ACTION
patients to the Emergency Department. Our
ACTION Team have reached out to 1,691
patients as of FY 2016.
Community Health Centre
To support our GPs in managing
patients with chronic diseases, the
Hospital started the Community Health
Centre (CHC) in May 2016. The centre
offers services such as Diabetic Retinal
Photography, Diabetic Foot Screening,
Outpatient Nurse Counselling and
Outpatient Dietetics Consultation.
Patients may be referred to the CHC
through GPs’ referrals, bringing more
convenience to outpatients in the
community.
Amputee Support Group
The Amputee Support Group was initiated
in December 2010, by the Hospital’s Medical
Social Workers. The aim was to provide a
platform for the amputees to share issues
and challenges faced in their daily life and
to establish and maintain a social support
network for the amputees.
The Amputee Support Group collaborates
with TTSH Rehab as well as Handicaps’
Welfare Association. To date, 83 patients
have participated at this support group.

Power-up Rehabilitation
Conventional gait training usually requires
at least two therapists to assist the patient
and has many limitations such as; patient
fatigue, occupational injuries for therapists
and as a result of the previous two
limitations, the possibility of poor quality of
training.
The newly introduced programme, Power-
Up integrates technology that uses air
pressure to reduce patients’ body weight,
allowing patients to exercise longer without
exerting stress on their injuries. Fifty-
one patients have been identified for this
programme between November 2015 and
March 2016.
Empowering our Patients & Caregivers
Advanced Care Planning (ACP)
The ACP programme seeks to prepare patients and their
families for the future by ensuring suitable family members
are identified to make informed decisions on behalf of the
patient during a crisis. Our Care Team members who are
trained in ACP, engage patients and their family members
about the patient's future healthcare plans and guide them
on the decision making process. A total of 61 patients’
families completed the ACP discussions with our staff since
the implementation of this programme in January 2015.
Palliative Care Training
To help our nurses better manage palliative patients and their
families, the Hospital conducts related training programmes
which include the End of Life Nursing Education Consortium
Course (ELNEC) and Share The Care (STC) Programme. The
ELNEC, which covers advanced principles of palliative care,
is offered to our senior staff, as well as nursing staff from
other hospitals. AMK-THKH has trained 165 staff for this
course by the end of 2015.
The STC Programme, which introduces the basis and
development of palliative care, covers communication skills,
loss, grief, cultural and ethical issues related to non-cancer
palliative care. This programme has seen 224 staff trained
as of FY 2016.
Homework Therapy for Patients
The ‘Homework Therapy Programme’ aims to maximise
the use of patients’ time during weekends. Introduced in
March 2016, patients are encouraged to carry out simple
exercises by following the steps provided in handouts and
via a live demonstration. The handouts are prepared in four
languages to facilitate patients’ learning.
Cross-Training of the Therapy Assistants
The Cross Training Project, initiated in November 2014,
aims to train suitable Therapy Assistants to become
Physio-Occupational Therapy Assistants (PO-TA). Under
this initiative, these PO-TAs are able to perform both
Occupational Therapy and Physiotherapy treatments on
patients. This has helped maximise manpower and increase
productivity. The Hospital has trained 29 PO-TAs as of FY
2016.
Patient Empowerment Programme
The primary aim of this programme is to equip patients
with basic knowledge on chronic disease management and
preventive care. It also seeks to ensure patients receive their
recommended influenza vaccinations and are not at risk of
osteoporosis.
With better knowledge on how to care for their chronic
conditions, and having had the proper immunisations
before discharge, only 1.54% of the 2,850 patients enrolled
in this program were readmitted to a Restructured Hospital
within 28 days of their discharge.

The Hospital has taken various initiatives
to reach out and educate the community
on health issues. These Reaching Out
Seminars, which include talks and
workshops cover topics such as diabetes,
stroke, and active ageing. The participants
are encouraged to take proactive steps in
managing their well-being. A total of eight
such sessions were conducted, reaching
out to about 250 seniors in the community.
The Hospital also works closely with our THK sister agencies and community partners
in the Ang Mo Kio region to help the elderly stay independent in the community:

Ang Mo Kio – Thye Hua Kwan Hospital has
always taken pride in being a part of Thye
Hua Kwan Moral Society. Our staff have
volunteered to support THK in various
activities and fund raising events.
The annual Inter – Racial Inter-Religious
Harmony Nite (IRIR) 2016 which took place
at the Marina Bay Sands Grand Ballroom
on July 3, 2016 celebrates the multi – racial
and multi – religious harmony of Singapore.
The event was graced by President Tony
Tan Keng Yam. The Hospital staff served as
ushers at this event.
THK Triple Celebrations this year took place
at the Marriott Hotel, marking the Holy
Birthdays of our two Patron Saints, Hua Toh
Zu Shi (Master of Medicine) and Lu Chun
The Singapore Health Quality Service
Award (SHQSA) 2016 saw 293 of
our Hospital staff honoured for their
dedication to quality care and excellent
service. Among them were 17 Star award
winners. The ceremony was held on
January 26, 2016, at University Cultural
Centre and was graced by Dr Amy Khor,
Senior Minister of State, Ministry of Health.

Ms Jocelyn Ng was featured in the annual
publication "Moving On" by the Institute of
Technical Education.
Titled "She's Lovin' It", the article charted
her rise from ITE as an enrolled nurse to a
Nurse Educator who is currently pursuing a
Masters in Education.

dated March 14, 2016 mentioned the
upcoming of the new Community Health
Centre (CHC) at Ang Mo Kio - Thye Hua
Kwan Hospital. The CHC offers services such
as diabetic foot screening, physiotherapy,
dietetic advice and nurse counselling.Volunteer Activities
Our dedicated team of volunteers run about
25 weekly programmes for our patients. These
activities aim to engage our patients in a social
setting. Activities include art and craft sessions,
games as well as art and pet therapy. For the
past year, the Hospital recruited 167 volunteers,
of which 60 of them are volunteering on a
regular basis.
A total of nine companies did their Corporate
Social Responsibility projects at the Hospital.
The companies included Society for the
Physically Disabled (SPD), VMWare Pte Ltd,
Yahoo, Celanese Pte Ltd, Singtel, IBM, Camfill
Sg Pte Ltd, Mindef and RAMO industries. These
programmes were well received by our patients.
Ang Mo Kio – Thye Hua Kwan Hospital was
mentioned as one of the three providers who
will be starting a programme focused on
senior care. Called the Eldercarer Training
Programme, caregivers will receive training
to perform daily tasks such as tube feeding
and diaper changing.
Health Minister’s Visit at AMK-THKHThe Hospital was delighted to
welcome Mr Gan Kim Yong,
Minister for Health and Mr Chan
Heng Kee, Permanent Secretary
on July 20, 2016. The visit, which
included other MOH senior officers
provided them with insights to the
services offered, as well as the
care philosophy of the Hospital.
The patients, who were delighted
to see Mr Gan, also received well-
wishes from him.
