Ar ran y Llywodraethwyr a’r staff, pleser o’r mwyaf yw cyflwyno Adroddiad Blynyddol Ysgol Gyfun Gwynllyw am y flwyddyn academaidd 2014-15. Mae hon eto yn flwyddyn lle gwelwyd gwelliant gyda dysgwyr yn ffynnu ac yn ennill canlyniadau clodwiw ar draws y cwricwlwm gan ddatblygu fel unigolion mewn awyrgylch ddiogel a chyfeillgar. Mae hefyd yn galonogol iawn i weld twf addysg Gymraeg yng Ngwent gyda sefydlu’r ysgol uwchradd newydd yng Nghasnewydd. Yr ydym wrth ein bodd mai Pennaeth yr ysgol newydd honno yw Rhian Wyn Dafydd sydd â chysylltiadau gyda Gwynllyw a Chwm Rhymni fydd yn fodd o barhau a chryfhau partneriaethau yn y dyfodol. Mae llwyddiant yr ysgol yn ddibynnol ar nifer o ffactorau; dysgwyr sydd â chymhelliant i ddysgu ac sy’n cymryd y cyfleoedd a gynnigir iddynt yn ddyddiol; athrawon a staff cynorthwyol sydd ag angerdd i weld dysgwyr yn gwireddu’u potensial ac arweinyddiaeth sy’n darparu awyrgylch lle gall dysgwyr a’r staff ffynnu. Hoffwn ddiolch i’r dysgwyr a’r staff am eu gwaith caled yn ystod y flwyddyn sydd wedi esgor ar y llwyddiant hwn. Cymerwch ychydig o amser i ddarllen yr adroddiad hwn gan ei fod yn adlewyrchu holl lwyddiannau’r rhai hynny sydd wedi gweithio mor galed a’r teuluoedd sydd wedi’u cefnogi yn ystod y flwyddyn.
On behalf of the Governors and Staff I have great pleasure in introducing the Annual Report for Ysgol Gyfun Gwynllyw for the academic year 2014-15. It is a year in which we have seen continued improvement, with pupils thriving, gaining success with excellent exam results across the curriculum and developing as individuals in a safe and friendly environment. It is also encouraging to see the growth in Welsh medium education in Gwent with the creation of the new secondary school in Newport. We are delighted that the new Head of that School is Rhian Wyn Dafydd, who has connections with both Ysgol Gyfun Gwynllyw and Cwm Rhymni, which can only serve to strengthen partnerships in the future. The success of a school is dependent on many factors. Pupils who want to learn and who take hold of the opportunities they are given to develop each day. Teachers and support staff who have a passion to see the pupils exceed expectations, and leadership that provides an environment where both pupils and staff thrive. I would like to thank the pupils and staff for their hard work during the year which has led to this success. Please take time to read this report as it reflects the success of all who have worked so hard and the families who have supported them during this last year.

Ers Mai 2013 mae trefniadau newydd mewn grym o ran cyfarfod blynyddol llawn y
llywodraethwyr / rhieni. Yn hytrach na bod yn rhaid i gorff llywodraethol gynnal cyfarfod
blynyddol gyda rhieni, mae gan rieni’r hawl i ofyn am gyfarfod gyda chorff llywodraethu ar
faterion sy’n destun pryder. Gall rhieni arfer yr hawl hon hyd at dair gwaith yn ystod
blwyddyn ysgol, cyn belled mai pwrpas y cyfarfodydd yw trafod materion sy’n ymwneud â’r
ysgol, nid cynnydd a chyflawniad disgyblion unigol na chwynion ynghylch aelod o staff neu
aelod o’r corff llywodraethu. Caiff cyfarfod ei drefnu gyda’r Llywodraethwyr drwy
ddefnyddio adran 94 o Ddeddf Safonau a Threfniadaeth Ysgolion (Cymru) 2013.
Since May 2013 new arrangements have been in force regarding the annual parent /
governors meeting. It is no longer obligatory for the governing body to hold an annual
meeting with parents but parents have the right to ask for a meeting with the Governing
body to discuss matters of concern to them, Parents may exercise this right up to three times
in any academic year, so long as the purpose of the meeting is to discuss matters of concern
to the school and not individual pupil achievement or grievances against staff members or
members of the Governing body. Such a meeting may be arranged with the Governing Body
by using section 94 of the School Standards and Arrangements Act (Wales) 2013.
Mae’r corff Llywodraethu yn adolygu a diweddaru polisïau fel rhan o gylch parhaol. Yn ystod
y flwyddyn 2014-15 adolygwyd a derbyniwyd y polisïau isod.

Caiff Prospectws yr Ysgol ei addasu yn flynyddol er mwyn cynnwys unrhyw newidiadau sy’n
ofynnol gan Lywodraeth Cymru a chyrff perthnasol eraill. Rhoddir copi i rieni’r plant sy’n
dechrau yn yr ysgol am y tro cyntaf neu sy’n trosglwyddo o ysgol arall. Darperir copi drwy
gais i swyddfa’r ysgol.
The school prospectus is revised and updated annually in order to include any changes made

Mae Ysgol Gyfun Gwynllyw wedi ymrwymo i Ddiogelu a Hybu Lles ei disgyblion. Mae lles disgyblion yn hollbwysig. Cydnabyddwn y gall fod rhai plant yn hynod agored i niwed. Cydnabyddwn fod plant sy’n cael eu cam-drin neu eu hesgeuluso yn gallu ei chael hi’n anodd datblygu ymdeimlad o hunan barch ac ystyried y byd mewn ffordd bositif. Gall eu hymddygiad fod yn heriol tra’u bod yn yr ysgol. Rydym bob amser yn meithrin ymagwedd ystyriol a sensitif er mwyn cefnogi ein disgyblion. Byddwn yn sicrhau bod ein polisïau yn cydgysylltu i hyrwyddo diogelu plant. Swyddogion dynodedig: Miss Helen Rogers a Miss Rhian James. Ysgol Gyfun Gwynllyw is committed to safeguard and promote the welfare of it’s pupils. Pupil welfare is paramount. We acknowledge that some young people may be very open to harm. We acknowledge that children who suffer abuse or neglect have difficulty developing self respect and to perceive the world in a positve way. Their behaviour can be challenging in school as a result. We are always considerate and sensitive to these issues and strive to support our pupils on all occasions. We ensure that our policies all promote child welfare. Mae'r Corff Llywodraethol, mewn cydweithrediad â'r Prifathro, wedi trefnu polisi a phatrwm o weithredu darpariaeth i blant a chanddynt Anghenion Addysgol Arbennig, ac yn ceisio'n frwd i sicrhau bod trefniadau staffio ac ariannu addas ar gael. Apwyntiodd yr ysgol aelod o'r staff dysgu â chyfrifoldeb penodol am gydlynu adnabyddiaeth a darpariaeth i ddisgyblion a chanddynt Anghenion Addysgol Arbennig. Mae'r Cydlynydd Anghenion Addysgol Arbennig (CAAA) yn cydgysylltu ag aelod o'r Corff Llywodraethol sydd â chyfrifoldeb am Anghenion Arbennig. Mae polisi'r ysgol am Anghenion Addysgol Arbennig yn canolbwyntio ar adnabyddiaeth gynnar a darpariaeth briodol ar gyfer disgyblion a chanddynt anghenion arbennig. Dilyna’r ysgol y dull tri cham o weithredu fel y dynodwyd yng Nghôd Ymarfer yr Adran Addysg 2004. Yn gryno, y camau yw :
1. Bod yr athro dosbarth yn adnabod Anghenion Addysgol Arbennig y plentyn, yn ymgynghori â'r Cydlynydd Anghenion Addysgol Arbennig ac yn cymryd y camau cychwynnol. Y CAAA yn casglu gwybodaeth ac yn creu proffil un tudalen.
2. Bod y dysgwr a'r CAAA yn cael eu cynnal gan arbenigwyr o'r tu allan i'r ysgol. 3. Bod yr AALL yn ystyried yr angen am asesiad statudol, ac yn ystyried yr angen am
ddatganiad am AAA ac, os yn briodol, i wneud datganiad a threfnu, monitro ac adolygu darpariaeth.
Nodwyd yn Adroddiad yr Arolygwyr, Mawrth 2014: “Mae’r ddarpariaeth ar gyfer disgyblion ag anghenion dysgu ychwanegol yn effeithiol iawn. Mae’r prosesau pontio cynhwysfawr yn galluogi’r ysgol i adnabod anghenion disgyblion penodol yn gynnar iawn a pharatoi rhaglenni ymyrraeth priodol ar eu cyfer. Caiff cynlluniau addysgol unigol eu llunio ar y cyd â rhieni a disgyblion ac maent yn cynnwys nodau a thargedau sy’n cynnig canllawiau defnyddiol i gefnogi’r dysgu. Mae adolygiadau cynnydd rheolaidd yn cynnig cyfleoedd gwerthfawr i’r rhieni chwarae rhan lawn yn addysg eu plant. O ganlyniad, mae’r disgyblion sydd ag anghenion dysgu ychwanegol yn cyflawni’n dda iawn.” The Governing Body has, in co-operation with the Head Teacher, determined a policy and approach for the provision for children with special educational needs, which does its best to ensure that appropriate staffing and funding arrangements are in place. The school has appointed a member of the teaching staff with a special responsibility for co-ordinating the identification of, and provision for pupils with special educational needs. The Special Educational Needs Co-ordinator (SENCO) liases with the member of the Governing Body with responsibility for Special Needs. The school’s policy for special needs focuses upon the need for early identification and suitable provision for pupils with special needs. The school follows the three staged approach as identified in the 2004 Department of Education Code of Practice. In short the stages are:
Anghenion Dysgu Ychwanegol / Additional Learning Needs
13
2014 - 15
1. Class teacher identifies a child’s SEN, consults with SENCO and takes initial action. SENCO gathers information and creates a one page profile. 2. Learners and SENCO are supported by specialists from outside the school. 3. The LEA considers the need for a statutory assessment. The LEA considers the need for a Statement of SEN and if appropriate makes a statement and arrange, monitor and review provision. The Registered Inspector noted in his report of March 2014: “Provision for pupils with additional learning needs is very effective. Comprehensive transition processes enable the school to identify specific pupils’ needs at a very early stage and prepare appropriate intervention programmes for them. Individual education plans are devised jointly with parents and pupils, and they include aims and targets that offer useful guidelines to support learning. Regular progress reviews offer valuable opportunities for parents to play a full part in their children’s education. As a result, pupils with additional learning needs achieve very well.”
Nodau / Aims:
Sicrhau bod cynifer ag sy’n bosib o’r disgyblion yn cymryd rhan, yn mwynhau a chael cyfle i ddatblygu eu sgiliau chwaraeon i’r lefel uchaf bosibl. Ensure that as many pupils as possible take part, enjoy and have an opportunity to develop their sport skills to the highest possible level.
