SCHOLARSHIP CONTRACT -FOREIGN SCHOLARSHIP-

The Government of the Philippines through the City Government of Santa Rosa represented by City Mayor ARLENE B. ARCILLAS with principal office at the New City' Hall Building of the City of Santa Rosa, Laguna, hereinafter referred to as the GRANTOR; NORY GAZEL C. MAGSOMBOL, · of legal age, Filipino, Single, presently residing at 1131 Capt. Perlas St., Brgy. Pooc, City of Santa Rosa, Laguna; hereinafter call the GRANTEE; LOLITA A. CUYOS, of legal age, Filipino, Single, presently residing at 1131 Capt. Perlas St., Brgy. Pooc, City of Santa Rosa, Laguna, hereinafter called the GUARANTOR.

WITNESSETH:

That pursuant to the prov1s1on of Executive Order No. J.29 as amended and in consideration of the grant and · acceptance by the GRANTEE of a scholarship/training award to undergo on official time a program entitled Netherlands Fellowship Program for the Course on Urban Manag~ment Tools for Climate Change (UMTCC) offered by the Institute for Housing · and Urban Development Studies (IHS) in Rotterdam, The Netherlands from June 4 to 23, 2012, GRANTEE hereby agrees to fulfill the following terms and conditions;

1. That the GRANTEE . shall keep up with the standards of ·scholarship or award;

2 . That the GRANTEE shall conduct herself in a manner as not to bring disgrace or dishonor to her and/or her country;

3. That the GRANTEE shall submit to the head of her Office and to Special Committee on Scholarships (SCS) at the end of each · term, her official transcript of grades, certificate of performance or its equivalent;

4. That the GRANTEE must comp[ete the course. She shall not be allowed to cancel or terminate her scholarship/training grant without justifiable reason and without giving prior notice to and getting the conc;l..1rrence of the donor institution, her agency and

the NSC;

5. That, if there is sufficient reason for the extension of the GRANTEE's original schol.arship/training/award, she shall submit a formal request to his agencyJ with justification and recommendation from her Program Adviser, six (6) months prior to the expiration of her travel authority/validity of the passport . It is understood that approval of the extension shall be made only upon the recommendation of the GRANTEE's agency to SCS and up·on the GRANTEE's execution of a supplementary training/scholarship contract covering the extension period;

6 . That the GRANTEE shall report to her office upon the completion · or te rmination of her scholarship, fel lowship or training grant;

7. That the GRANTEE upon completion of training, shall submit to the Central LGSC and her mother unit, a Re-entry Plan or proposal for the application of newly acquired skills expertise, copy furnished the Office of the DILG Secretary and the National Screening Committee;

8. That the GRANTEE may be tapped as resource person by the DILG field office con·cern on the subject matter studied by her;

9. That the GRANTEE shall serve her office or any other government office or instrumentality for the period of __ as

. exigencies of the service required per Section 2 of Executive Order 367;

10. That the GRANTEE accepts the value of the fel lowship as follows:

11.That should the GRANTEE fail to comply ·with . the foregoing conditions through her · fault or willful neglect, resignation, voluntary retirement or other causes within her control, she refund to her Office the amount defrayed by the Philippine Government in Section 8.

Proportionate refund shall be allowed, provided that the GRANTEE has serve·d her Office or agency at least 75% of her total service obligation·.

12. That the GRANTEE shall have a guarantor as her co-maker or co-signo r in 'this contract who shall assume full responsibility jointly and severa lly should she fai,I to co'mply with the conditions set forth.

That the GRANTOR shall:

1. Allow the GRANTEE to study on official time as specified under paragraphs a and b of SCS MC No . 14, series 1999;

2. Terminate that GRANTEE's grant if circumstances warrant;

3. Pay the GRANTEE sa lary and other benefits while on scholarship; ( other· benefits is subject to availability of funds)

4. Relieve the · GRANTEE of all the duties and responsibiliti es for the duration of the grant;

5. Monitor the GRANTEE's academic performance and completion of her studies.
