A Crossrail construction worker's limited English skills were a contributing factor in his death, an inquest jury has ruled.Rene Tkacik, a 44-year-old Slovakian national, was crushed to death by poured concrete in Holborn, central London, on 7 March 2014.The jury ruled his death had accidental contributory factors including him not being able to understand all briefings.

Findings The Long View• Tutorial or language competence test?• Equal opportunities challenge?• Does everyone have to take the test?• Relevance large range of competence levels and roles?• One set of generic questions understood by all, or customised
to suit different job roles?• Computer-based test works for ‘reception’ (i.e. comprehension)
but what about interaction and production?• What is the pass rate?• Straightforward pass-fail or a graded score with different
outcomes?• What about those who do not pass?• Avoiding repeat of the safety passport test (e.g. CSCS)?
