What is 100,000 Homes campaign? • National movement• House vulnerable and chronically homeless individuals and
families
• 100,000 by July of 2014.
What is Vulnerability Index (VI)?• Began as research into
causes of death among chronically homeless individuals
• Tool for indentifying and prioritizing people who are homeless according to the fragility of their health and living situation
• VI Tool can be used to develop individualized housing plans
Colorado Counts: Communities working to house Coloradans in need•Fifth state to join the 100K Homes Campaign•Expanded vulnerability:
•Health vulnerability, AND•Behaviors or circumstance that jeopardize their situation,•At risk for system involvement, and•Frequent User.
•Targeting all populations, not just the traditional chronic•Incredible data tool

• Done in places veterans will be likely to frequent
• Use VI tool to focus on targeting housing /services
• Conduct initiative with other vulnerable homeless populations in 2013-2014
