Housing Association

The Community Support Service is available to anyone over the age of 60 in need of housing related support

in the Hart District Council area. This is regardless of whether you own your own home, rent from a private

landlord or from a housing association. A trained support worker will visit you at home and design a support

plan tailored to suit your own specific needs. We put you in touch with the right people who can help with

anything from cleaning and personal care to aids and adaptations, benefit claims and grants. We can also

offer services to help you feel more secure in your home. You may need a helping hand with other everyday

tasks like taking curtains down, changing light bulbs or dealing with correspondence.

Advice

Junction 5 offers free confidential advice to anyone living around the Junction 5 area of the M3. Services are

offered by a team of trained volunteers in association with Community Money Advice. The advice offered

Trussell Trust Foodbanks provide emergency food for local people in a crisis. Those receiving food are

referred by frontline care professionals. Local people donate non-perishable food. Food parcels are

designed to provide nutritionally balanced food for three days.

Basingstoke

With its friendly welcome and relaxed atmosphere, the Age Concern Cafe is an ideal place to enjoy a cuppa,

or early lunch, away for the hustle and bustle of the shops. The Cafe offers simple food at reasonable prices

including hot and cold drinks, toasted teacakes, scones, hot soup and filled jacket potatoes. An information

table is available each week with information on local services and events for older people. New customers

are always very welcome at the Cafe. In fact we offer 'Tea & Company' to people visiting on their own. Why

Alzheimer Cafe UKWhat is an Alzheimer Cafe? A monthly gathering where persons with dementia and/or their family and

friends can be together in a safe, welcoming environment, in the company of their carers, volunteers and

health care professionals, for the purpose of emotional support, educational and social interaction. A typical

evening starts with an informal chat over light refreshments. A presentation by a professional and an

informal interview with someone with hands-on, real experience of the subject discussed. Followed by a

question and answer session and time for social interaction and more refreshments. Occasionally live music

or other entertainment is provided.

Alzheimer's Society -

Singing for the Brain.

Hartley Wintney.

Singing for the brain, which uses singing to bring people together in a friendly and stimulating social

environment, is a service for those with Dementia and their carers and is run by the local Alzheimer's Society

offices in Blackwater and Basingstoke. Singing is not only an enjoyable activity, it can also provide a way for

people with dementia, along with carers, to experess themselves and socialise with others in a fun and

supportive group. Hidden in th e fun are activities which build on the well-known preserved memory for

song and music in the brain. Even when many memories are hard to retrieve, music is especially easy to

recall.

the Third Age)

It's never too late to learn! Join the 320,000-plus members in 915 U3As throughout the UK today. U3A is run

by its members for its members. It draws on the range of skills and expertise held by retired people and

those over 55 to create a large number of activities and groups. Groups in Yateley, Hartley Wintney, Fleet,

Alton, Odiham, Basingstoke - Old Basing U3A group hold their main monthly meeting every 2nd Wed.of the

month, but not in Aug, 2 - 4 pm at Elizabeth Hall, Hook.

The Association's aim is to get as many people as possible, who are disabled or infirm, on the Basingstoke

Canal. Hire our holiday boat 'Madam Butterfly' or our day boat 'Dawn'. The boats are based at Colt Hill in

exercise which can be adapted to suit individual needs. It is also very beneficial in aiding relaxation and

wellbeing.

Hart Health Walks Hart Health Walks, managed by Hart Voluntary Action, offer short weekly walks around the areas of Hook,

Hartley Wintney, Odiham and Crookham Village. The walks are designed to provide gentle exercise and to

provide a relaxed friendly atmosphere. At the end of the walk tea and biscuits are available and donations

are welcome. Just come along a little early if it is your first time to register.

Hook Allotment Association CiC is a not for profit organisation committed to providing allotments for our

community. We have limited income from plot holders wich is used just to cover our site running costs like

rent, water, insurance etc. The 5.4 acre site is located along the Reading Road on the right just after Hook

Garden Centre. There is a small car parking area on site. Our waiting list is open to residents of Hook and

Rotherwick and residents of other nearby parishes are also welcome to apply in case an allotment

opportunity arises.

‘Minding the Garden’ uses the great outdoors to improve health and wellbeing. Working as part of a team

Gardening Support

Service.

Assistance for Hampshire residents over the age of 60 to maintain their gardens. If you are over 60 and

receive a means-tested benefit, the service is available at a reduced rate. It is also available to those who do

not receive a benefit, at the standard rate. It is a year - round gardening support serivce, with up to six visits

per year, each visit lasting up to four hours. One-off visits can also be arranged. Shaw Trust's fully qualified

and experienced team, who are supported by volunteers, provide a professional garden maintenance

serivce. Shaw Trust has over 30 years experience of delivering horticultural services and all staff and

volunteers are checked by the Criminal Records Bureau.

Aster Living -

Handyperson Service

In partnership with Hampshire County Council, Aster Living provides a subsidised handyperson service for

Hampshire residents over the age of 55 or disabled, including families with children who have a disablility.

The service covers small jobs that would take up to two hours to complete. Jobs such as fitting key safes,

putting up shelves and curtain poles, repairing leaking taps and toilet cisterns, moving furniture from one

room to another, changing broken plug sockets, putting up smoke alarms, fitting grab rails and hand rails

and many other small jobs. The whole team is DBS (Disclosure and Barring Service) checked and fully trained

so you can be confident to invite them into your home. The labour charge is £12 or £18 per hour (including

VAT) depending on your circumstances, all quotes are free and materials are charged at cost.

Independent Age Independent Age provides regular support to more than 6000 people over the age of 70 in reduced

St. Michael's Hospice

(North Hampshire)

Quality of life - Choice

in Care

St. Michael's Hospice (North Hampshire) helps people faced with a life-limiting illness, their families and

carers by providing a choice of specialist care and support to enable them to attain the highest possible

quality of life. Based in Basingstoke, we service the whole of North Hampshire; around 45% of patient

admissions come from outside of Basingstoke.

We care for patients both in the Hospice and out in the community. As the majority of patients want to stay

at home for as long as possible our Hospice at Home Serivce works closely with professionals in the

community to support patients and provide them with a real choice about where they wish to be cared for.

Family support is also a significant and expending part of our work.

Hospice at Home is a service which takes the skills, ethos and practical nursing care associated with the

Hospice into the home environment. St. Michael's Hospice works with patients to ensure they receive the

necessary emontional, psychological and proctical nursing care to die at home, if that is their wish. The

Hospice at Home service is provided free of charge to support patients and those people caring for them.

Hospice Home Support was established to provide help and support for patients and their families, coping at

Deaf - plusDeaf PLUS is the working name of Breakthrough Deaf and Hearing Integration a registered charity and

company, limited by guarantee, working to encourage integration and equality between Deaf and hearing

people in all areas of life. The organisation runs a mobile advisory service where equipment can be seen and

Richmond Fellowship -

Floating Support

Service.

The Rushmoor and Hart Floating Support Service provides assistance and practical support to people with

metal health problems enabling them to maintain their accommodation and live independently. We offer a

flexible Support Service centred on the needs of the individual and forcused on preventing homelessness by

enabling people to develop the independent living skills necessary to sustain their accommodation and live

in the community. The Service also aims to reduce social exclusion by empowering people to take

advantage of the range of community-based resources available locally. Types of

service offered include:developing daily living skills, budget planning, support to access benefit agencies and

benefit claims, individual support planning, support to access community resources e.g. education, leisure,

assistance with liaising with landlords/housing associations.

Meals on Wheels Anyone living in the Hampshire County Council area, who is 55 years of age or over, can choose to receive

Meals on Wheels if they meet the criteria.

The meals are provided by Hampshire County Council's partner company Apetito and the customer will be

able to choose a main course and pudding from a menu of four options. The afternoon teas will be delivered

at the same time as the lunch time meal and offer a choice of savoury pastry, sandwich and cake as well as a

portion of fruit and a fruit drink.

Rail Information: A Senior Railcard can be purchased for £30 a year by those over 60 years of age. This will entitle the holder

to a third off their railfare, when travelling at off peak times. Proof of identity will be needed to buy cards at

a rail station. Disabled Person

Railcards are also available costing £20 a year and the holder can travel at any time with a third off most

fares. Accompanying helpers can also travel at a reduced cost. Wheelchair users or those needing

assistance will need to book 24 hours in advance.

Trains Trains run half hourly to London Waterloo and to Basingstoke. Sunday services run hourly.

Bus Passes Free national bus pass is available to all when over 60 years of age who were born before 6th April 1950.

After this age you become eligible for a pass when reaching pensionable age. You can travel from 9.00 a.m

until 11 p.m. on weekdays within Hampshire but from 9.30 a.m. until 11.00 p.m. on weekdays outside

Hampshire. At weekends and public holidays you are able to travel at any time throughout England. Always

have your pass with you as without it you will have to pay full fare. With a disabled bus pass you can travel

at any time of day in Hampshire but only at off peak times on weekdays elsewhere in England. It is possible

to apply for a companion pass if you are unable to travel alone for medical reasons.

From 31st August 2015 the Stagecoach bus 13 route has been enhanced. The 13 route is now running from

to Alton from 7.20 a.m. and to Basingstoke from 6.56 a.m. approx half hourly until 9.44 and hourly from

Saturday buses run hourly from 7.40 to 18.40 to Alton/Whitehill and from 8.24 to 19.24 to Basingstoke.

Capital Cars run a Taxi Bus from the Raven Hotel and back via Griffin Way South, Quince Tree Way,

Community Hall, Four Acre Coppice, Holt Way, Pantile Drive, Holt Lane, Doctors Acre, London Road, Hook

House Farm, Geffrey's House, White Hart, Raven Hotel. Concessionary passes can be used after 9.30 a.m.

The bus currently runs 4 trips a day, on Tuesday, Thursday and Saturday and the service is timed to connect

as far as possible with the no. 13 bus and the train times. Timetables can be found in the Post Office, Trees,

Boots, Health Centre, Hook Station and bus stops. The timetable may be amended to suit the need. Please

encourage everyone to use it to keep costs as low as possible and encourage an expanding service. The taxi

bus is supported by Hook Parish Council.

Hook Care Group Do you have a medical appointment but can’t get there? Hook Care Group is a voluntary organisation

providing low cost car transport to medical appointments for those who have difficulty getting there. When

you need a car call our local Coordinator (see number below) who will arrange for a Driver to collect you.

Please give us at least 2 days notice as we are not an emergency service.

If you would like to Volunteer as a Driver or to organise lifts as a Coordinator then we would be delighted to
