WELCOME TO THE THORNTON PRACTICE

The Thornton Practice had its origins over 50 years ago when Dr Ben Andrews started his practice close to the current medical centre site. Society and medicine have seen many changes over the decades and so we have adapted to provide the best quality of medicine whilst keeping what is best about family medicine. The Thornton Practice has always been forward looking and innovative, and as a result we have a wide variety of professionals to cater for your needs. It’s important that we get you to the right person, so when you ring you may be asked a few questions about your problem. THE THORNTON PRACTICE PHILOSOPHY Our aim is to offer the highest standard of health care and advice to our patients with the resources available to us. We have a team approach to patient care, and endeavour to monitor the service provided to patients to ensure that it meets current standards of excellence. We are dedicated to ensuring that Practice staff and Doctors are trained to the highest level to provide a stimulating and rewarding environment in which to work. SURGERY OPENING TIMES & APPOINTMENTS MONDAY – FRIDAY Thornton Branch Reception Opening Hours 8:00am to 6:30pm Phones Operating from 8:00am to 6:00pm Carleton Branch Monday and Thursday 8:30am to 6:00pm Tuesday, Wednesday and Friday 8:30am to 1:00pm Appointments The Practice operates an advanced booking system. These appointments are available up to two weeks in advance. For an urgent appointment, or to be seen ‘on the day’, please ring at 8:00am for an appointment. When booking an appointment our Patient Advisors will ask a series of questions to ensure you are seen by the most appropriate Health Care Professional.

Enhanced Access Service We also provide out of hours appointments 7 days a week located at Freckleton Health Centre and Fleetwood Health Centre. These appointments are for routine general practice issues only, including but not limited to GP appointments, Ear Syringing, Smears, Dressings, Blood Pressure, ECGs and Blood Tests.

111 Service When the Practice is closed help and advice is available 24 hours a day from the 111 service, which is provided by the Primary Care Trust. They will direct you to the most appropriate care – simply dial 111.

Midwives If you think you may be pregnant or would like advice, please make an appointment with the midwife. Please call 01253 951499.

District Nurses District Nurses are available to visit patients who are housebound and require nursing care in their own homes. The telephone number is 01253 957597

Treatment Room The Treatment room is run by our District Nurses and is based within the surgery. They offer appointments for Blood Tests, Wound Dressing, Removal of Stitches and Ear Syringing. Please call 01253 951065 to book an appointment.

The Clinic This is based in our building for people who have been referred to any of the following services: Podiatry, Orthoptists, Counselling, CAMHS (Children), Anticoagulant. They also have self-referring clinics such

as: Smoking Cessation and Midwives. Please call 01253 951499.

ONLINE SERVICES Repeat prescriptions

Please give 48 hours’ notice for your repeat prescription. Prescriptions should be ordered by returning the tear-off slip on the right hand side of your prescription, either in

person or by post. It will normally be ready for collection at the surgery or your nominated pharmacy after 48 hours. If you need it urgently please ask at reception or via the prescription line. The prescription line is open from 9:30 to 12 noon Monday to Friday to order prescriptions or for any medication queries, please call 01253 956282 option 3. Please try not to telephone the main line for your prescription as this may block the line for somebody who needs the doctor. Patient Access You can also order your medication online via Patient Access. Simply sign up on our website www.thedoctors.co.uk or come into the surgery with some photo ID and we can sign you up. Please take responsibility for your own health and do not let your medication run out. You can also book appointments, change your contact information and browse your medical record on patient access.

Access for Disabled Patients All of our appointments are on the ground floor so easily accessible for disabled patients. We offer a foreign language translation service, provided by Language Line and also sign language translators. These should be booked with Reception 2 weeks in advance of any appointments.

We also run regular clinics including, but not limited to: COPD, Asthma, Coronary Heart Disease, Diabetes, NHS Health Checks, Smears, Cortisone Injections, Seasonal Influenza vaccinations. We also offer Travel Health Advice and Immunisations and run regular Baby Clinics for Vaccinations and Health Checks. If you require any information on any of the services we offer please speak to Reception or the Patient Advisors on the phone.

Home Visits If you are too ill to come to the surgery and need a home visit, please try to ring the surgery between 8:30 and 10:30. Urgent requests for visits may be made at any time as there will always be a doctor on duty to deal with emergencies. Please remember Home Visits are usually for the terminally ill and housebound patients, however we are aware some illnesses can temporarily make you housebound.

Test Results Please telephone for test results by ringing 01253 956282 (Option 4) after 11am when the telephone is not as busy and the patient advisors have more time to find the results for you.

USEFUL INFORMATION

Local minor injuries and ailments unit Your local Accident Department for minor injuries and ailments is at the Same Day Health Centre at Fleetwood. They will see anyone who has had a recent injury or a sudden onset of symptoms. They can bandage and stitch wounds and x-ray limbs. They can prescribe medication for infections, pain etc. They can also give advice about treatments and further care. They work on an appointment basis which can be booked by calling 0300 123 1144.

111 Service When the Practice is closed help and advice is available 24 hours a day from the 111 service. You will be directed to the most appropriate care – simply dial 111.

Staying for a short time in the area If a person is staying with you for a short time and becomes ill, they can obtain medical care from the practice. They will be asked to complete a temporary resident form in order for us to do this. If you go on holiday in the United Kingdom and become ill whilst away from home, you can also register as a temporary resident at a GP Practice near your holiday venue to be seen. Students living away from home whilst attending college/University should register with a doctor in the area where they spend the most time.

Change of Name/Address/Telephone numbers If you change your name, address, telephone number, (including mobiles), and email address, please let us have your correct details in writing as soon as possible so we can update

your record.

Medical Students The Thornton Practice is a teaching practice so is involved in the training and teaching of Medical Students including GPs, Nurses and Business Admin Apprentices. If a Medical Student is to be present during your consultation you will be informed when you arrive at Reception. If you would prefer the student to vacate the room during your consultation, please advise the Receptionist.

Practice Expectations from Patients The Practice expects patients to keep their appointment and always inform the surgery if they cannot attend. We would like all patients to respect the premises and to supervise children at all times. It is also the patient’s responsibility for their own health and safety while on Practice premises. Staff are here to help you so please respect them.

Patient Services Team The team are often your first point of contact and here to help. They may need to ask you for further details when you telephone for medical advice; this ensures that we can assist you as quickly as possible. All the team are bound by the same rules of confidentiality as the doctor.

The Health Visiting Team The Health Visitors can be contacted on 0300 247 0040. They can offer you confidential advice on all aspects of health for expectant mothers and pre-school children.

District Nurses District Nurses deliver a high standard of nursing care in the home to meet the needs of patients and their carers. Their aims are to:

Nurse patients in their own home following discharge from hospital

Offer specialist nursing care for acute and terminally ill people

Work with GPs to maintain the health of patients by promoting their independence

Community Midwife The Midwife attends all the ante-natal clinics which are held weekly. They can be contacted on 01253 951499.

Treatment Room The Treatment Room is open from 8am to 1pm. To book appointments for wound care, venepuncture, injections, ear care, blood tests, removal of sutures and minor injuries, please ring 01253 951065.

In order to ensure excellent customer services all our telephone calls are recorded for monitoring and training purposes.

OUR SERVICES Appointments

Computer appointments system is used

Appointment time is allocated on 12 to 15 minute consultations

We endeavour to run the Doctors’ clinics on time, although sometimes late running is beyond our control

Late arrivals for appointments may be seen at the Doctor’s discretion

Same day appointments are available although it may not be possible to see your preferred Doctor

Please be prepared to give the triage administration team details regarding the nature of your visit as this helps us to arrange for you to be seen by the most appropriate member of our team

Home Visits If you are housebound and unable to access the surgery, a home visit can be arranged. Please contact the surgery who can pass on your details of the neighbourhood Core Team who will contact you to arrange a visit.

Home visits are for patients who are too ill to leave home to visit the surgery/house bound/bed bound

For home visits please contact the surgery between 8:30 and 10.30am

Did Not Attend (DNA) These are appointments which are not attended without prior

notice. Please be aware that this will be recorded against your patient records.

Test Results Please telephone for test results by ringing 01253 956282 (Option 4) after 11am when the telephone is not as busy and the patient advisors have more time to find the results for you.

Repeat Prescriptions – 48 hours’ notice is required and can be requested:

Child Health Surveillance Dr Guest, the Practice Nurses and the Health Visitors run Child

Health Surveillance Clinics. We offer checks at 8 weeks.

Referral Information Our Secretary deals with all aspects of your referral and can be

contacted on 01253 957058 if you wish to discuss this.

Access to Medical Records/Freedom of Information Queries regarding access to medical records should be discussed with the Practice Team who will discuss the process and the fees involved.

The Practice supports the NHS Zero Tolerance Policy in Primary Care regarding aggressive/abusive behaviour.

Patient Records All manual and computerised patients’ health records are accessible under the Data Protection Act 1998. The Practice conforms to the Data Protection Act and all staff members who have access to patients’ medical and computerised records comply with confidentiality rules. Competent patients may apply for access to their own records, or may authorise a third party, e.g. their lawyer, to do so on their behalf. Parents may have access to their child’s records if this is in the child’s best interest and not contrary to a competent child’s wishes. Requests to access Health Records should be made in writing to the Practice Secretary (please note there will be a fee for this service).

Confidentiality All matters relating to individual patients are treated as strictly confidential at all times. We will only give details about appointments, test results etc. to you personally and not to a relative or representative, unless we have in writing to do so. If you wish to discuss something in private, but feel

you cannot do so at the desk, please make this clear to the reception staff and they will arrange for some privacy.

PRACTICE PRIVACY NOTICE

Your data, privacy and the Law. How we use your medical records This Practice handles medical records

according to the laws on data protection and confidentiality.

We share medical records with health professionals who are involved in providing you with care and treatment. This is on a need to know basis and event by event.

With your explicit consent some of your data is automatically copied to the Shared Care Summary Record by NHS Digital on behalf of NHS England.

If necessary for your care, we share your Summary Care Record (with your explicit consent) with local out of hours, urgent or A&E providers, including Same Day Health Centres and Walk-In Centres.

Data about you is used to manage national screening campaigns such as Flu, Cervical cytology and Diabetes prevention.

De-identified data about you is used to manage the NHS and make payments.

We share information when the law requires us to do, for instance when we are inspected or reporting certain illnesses or safeguarding vulnerable people.

Your data is used to check the quality of care provided by the NHS.

PATIENT PARTICIPATION GROUP (PPG) Thornton Practice PPG was announced as the “PPG of the Year 2016/2017” and again in 2017/2018 out of 20 Practices across Fylde and Wyre. In 2019 the PPG were named as National PPG of the Year by NAPP. WHAT DOES A PATIENT PARTICIPATION GROUP DO? 1. Looks at the needs of all patient groups. 2. Publicise events and new ideas to patients via notice board

(located on community corridor), social media and posters in the Patient Waiting Area.

3. Have regular minuted face-to-face meetings. These meetings last about one and a half hours and take place during the day.

4. Talking to patients and writing down patients' ideas, comments and experiences and then feedback this information to Patient Participation Group members, the Practice staff and GPs.

We are always looking to recruit new members onto the PPG. The PPG is hoping to recruit younger members drawn from the Practice to give a more vibrant dynamic to the Group. If you would be interested in joining our Patient Participation Group please contact the surgery.

We at the Thornton Practice are members of NHS Fylde and Wyre Clinical Commissioning Group (CCG) ‘As a CCG we are responsible for commissioning or “buying” health and healthcare services for the people of Fylde and Wyre.’ About the CCG They are led by family Doctors and currently serve a population of around 151,400 people across the towns of Fleetwood, Kirkham, Lytham St Annes, Poulton-le-Fylde, Thornton and a significant number of rural villages. They receive around £230million a year from the Government, and from that they plan and buy hospital and community health services. They must, by law, make sure these services are high quality, and spend your money wisely.
