How to use this guidePlease take a few minutes to review this guide, which is intended to help you make the best use of your benefits. It includes information that will help you make the most of your health care coverage through BCBSIL, including how to find a physician, facility or urgent care center, submit a claim and understand your explanation of benefits statements.

For most plans, Boeing contracts with different service representatives for pharmacy, behavioral health and vision care*. An overview of benefits is provided for those enrolled in those services.

About BCBSILWhat is BCBSIL?Blue Cross and Blue Shield of Illinois provides health care coverage to more than 7.3 million members. BCBSIL is a Division of Health Care Service Corporation (HCSC), the largest customer-owned health insurer in the United States and the fourth largest health insurer overall. HCSC is headquartered in Chicago, Ill., and has a full service unit in Jacksonville, Ill., that handles member service and claims processing for Boeing members.

Doctors and other providersHow do I find a physician or hospital?Keep in mind that Medicare is primary and your Boeing plan provides secondary coverage to Medicare. Your plan does not include a network requirement, but you may wish to confirm with your provider that he or she accepts Medicare reimbursement.

The BCBSIL website includes a Provider Finder® tool to help you locate physicians and facilities that meet criteria you set. You can

view the most current list of providers online by visiting www.bcbsil.com/boeing. Select Provider Finder under the Doctors & Hospitals tab or at the bottom right-hand side of the home page.

What type of information will Provider Finder give me?Available information includes:

• Physician or facility name, address and telephone number

• Provider specialty

• Accepting new patient status (yes or no)

• National Provider Identifier number

• Map and driving directions to a physician’s office or facility

Through Provider Finder, you can also locate an urgent care center.

On a BCBSIL plan, do I need to select a primary care physician or obtain a referral to see a specialist?BCBSIL plans do not require you to select a primary care physician or obtain a referral to see

a specialist. However, an ongoing relationship with a primary care physician can help you manage your health and address any concerns you have early on. You can use Provider Finder from your computer or mobile phone Web browser to search for primary care physicians for you and your covered family members in specialties such as internal medicine, family practice and pediatrics.

Will I receive a provider directory?Provider directories are not printed as they are updated frequently. You may view the most current list of providers online by visiting www.bcbsil.com/boeing and selecting Provider Finder under the Doctors & Hospitals tab or at the bottom right-hand side of the home page.

Health care benefitsHow can I get more information about my health care coverage through BCBSIL? Please visit BCBSIL’s website at www.bcbsil.com/boeing for additional information about your health care coverage, or call Member Services at 888-802-8776.

Where can I get a summary of my benefits?You may view your plan benefits online by visiting www.bcbsil.com/boeing and registering and logging in to Blue Access for Members after you receive your BCBSIL ID card. You can also call Member Services at 888-802-8776 or view your plan benefits via Boeing TotalAccess.

ID cards and claimsWhen will I receive ID cards?If you have not already received a new medical ID card from BCBSIL, you will receive it soon. If you don’t already have a separate Express Scripts (formerly Medco®) prescription drug card, you will also receive a card directly from Express Scripts in the mail. Please be sure to show your new ID card to each provider you see so they can update their

billing systems with your new information. ValueOptions will not issue ID cards. You should provide your BCBSIL member ID card when seeking services.

Why do the ID cards have only the subscriber’s name and not all family members’ names?The BCBSIL ID cards display only the subscriber (employee or retiree) name. The cards are to be used for all eligible family members even though their names are not printed on the card. If you need an additional card, you can request one by contacting Member Services at 888-802-8776.

How do I submit a claim?If you use a Medicare provider, the provider will submit claims directly to Medicare and then on to BCBSIL. Be sure to show your new ID card to each provider you see. If you need to file a claim yourself, you must fill out and submit a claim following the instructions on page 10.

How and when can I call BCBSIL Member Services?You can reach Member Services from 5 a.m. to 5 p.m. Pacific time (8 a.m. to 8 p.m. Eastern time and 7 a.m. to 7 p.m. Central time). Traveling Using BlueCard Worldwide®

Peace of mind wherever you go

Before you leave the United States, contact BCBSIL for coverage details. Your coverage outside the United States is provided through BCBSIL. Please remember to take your BCBSIL ID card with you.

If you need assistance outside the country, call 800-810-BLUE (800-810-2583) or call collect to 804-673-1177 for information. Through the BlueCard Worldwide program, you have access to physicians and hospitals in nearly 200 countries and territories around the world. An assistance coordinator, in conjunction with a medical professional, can arrange your physician’s appointment or hospitalization, if necessary. (In an emergency, go directly to the nearest hospital and then call BlueCard Worldwide at one of the phone numbers provided above.)

In most cases, providers that participate in the BlueCard Worldwide program will not require you to pay up front for inpatient care. You are responsible for the out-of-pocket expenses such as deductible, copayment and coinsurance and non-covered services. The physician or hospital should submit your claim.

You also have coverage at non-BlueCard Worldwide hospitals, but you will have to pay the physician or hospital for care at the time of service and then submit an international claim form with original bills to the address on the form. You can get a claim form by going online at www.bcbsil.com/boeing or calling Boeing Member Services at 888-802-8776.

Criteria include how many of a particular procedure the facility has performed and the results. Blue Distinction Centers have a proven history of delivering better results.

The goal is to help consumers find consistent specialty care, while encouraging health care professionals to improve the overall quality and delivery of care nationwide.

Blue Distinction Centers for Bariatric Surgery®: A full range of bariatric surgical care services is offered, including inpatient care, post-operative care, follow-up and patient education.

Blue Distinction Centers for Cardiac Care®: A full range of cardiac care services is available, including inpatient cardiac care, cardiac rehabilitation, cardiac catheterization and cardiac surgery.

Blue Distinction Centers for Complex and Rare Cancers®: Inpatient cancer care programs for adults are offered, including those treating complex and rare subtypes of cancer, delivered by multidisciplinary teams with subspecialty training and distinguished clinical expertise, with a focus on treatment planning and complex, major surgical treatments.

Blue Distinction Centers for Knee and Hip Replacement®: Inpatient knee and hip replacement services, including total knee and total hip replacement surgeries, are available at these centers.

Filing a ClaimFollowing these guidelines will help ensure that your claims are processed quickly

Provider-submitted claimsMost physicians and hospitals will file a claim directly with Medicare. Show your BCBSIL ID card to the physician or hospital when checking in to confirm that they will file claims directly.

Member-submitted claimsIf you need to file a claim yourself, you will need to fill out and submit a form like the one shown on the opposite page. You may call 888-802-8776 for assistance, or download a form online:

1. Go to www.bcbsil.com/boeing.

2. Under Boeing Member Resources, click Forms.

3. Click Medical Claim Form and print the form and instructions.

Then, fill out the form completely, attach a copy of the original itemized bill, and mail to the address shown on the claim form.

Tips for submitting claims

Help us avoid unnecessary delays by answering all questions completely and accurately, and including original itemized bills that contain the information we need to process claims.

• Your primary health insurance payer is Medicare, so your provider will send all claims to Medicare first; BCBSIL will generally coordinate with other payers to pay any eligible claims for services not covered by that payer (including international).

• Show the instructions accompanying the claim form to the persons providing your care and ask them to provide bills that follow the instructions.

• Itemized bills for medical treatment or surgery should include:

- Physician’s name, address and phone number

- Physician’s tax identification number

- Full name of patient (not just name of person to whom bill is addressed)

- Place where service was received (hospital, office or clinic)

- Diagnosis of illness or injury; if an injury, give the date it happened

Understanding Your Explanation of Benefits Statements

The EOB may include additional information:

• Information about amounts not covered shows which benefit limitations or exclusions apply.

• Information about out-of-pocket expenses shows an amount when a claim applies toward your deductible or counts toward your out-of-pocket expenses.

• Information about appeals explains your rights regarding review of claim denials.

• Fraud Hotline is a toll-free number you can call if you think you are being charged for services you did not receive or if you suspect any fraudulent activity.

Sign up for Blue Access for Members at www.bcbsil.com/boeing for quick, convenient and confidential access to your claim information and history.

To support our commitment to eco-friendly business practices, you can choose to opt out of receiving EOBs by mail. This saves resources and offers you additional confidentiality. Just go to www.bcbsil.com/boeing, click Settings at the top and select Preferences.

Review completed claims by mail or onlineAn explanation of benefits (EOB) statement is a notification form provided to members when a health care benefits claim is processed by BCBSIL. The EOB displays the expenses submitted by the provider and shows how the claim was processed.

The EOB has four main sections: Claim Information, Summary, Service Information and Coverage Information. A sample EOB is shown below, and descriptions of specific information are provided on the next page. If you have questions about an EOB, call Boeing Member Services at 888-802-8776.

Fraud increases costs and decreases benefitsFraud may cost the health care industry (both

public and private payers) more than

$200 billion each year. As a BCBSIL member,

you can help fight fraud by understanding

it and following the tips provided below.

Identifying fraud

Commonly identified schemes involving providers include:

• Misrepresenting services – This includes intentionally billing procedures under different names or codes to obtain coverage for services that aren’t included in a member’s plan.

• Upcoding – Deliberately charging for more complex or more expensive services than those actually provided is one example.

• Non-rendered and/or “free” services – Some providers intentionally bill for tests or services never provided. This can also mean that the provider offered “free” services in order to bill the insurance company for services not performed or needed.

• Kickbacks, bribes or rebates – An example is referring patients to a provider or facility in which the referring provider has a financial interest without disclosing it.

Commonly identified schemes involving members include:

• Identity swapping – Allowing an uninsured individual to use a member’s insurance card.

• Identity theft – Using false identification to gain employment and the health insurance benefits that come with it.

• Non-eligible members – Adding someone to a policy who is not eligible or failing to remove someone when that person becomes ineligible.

• Prescription medicine abuse and diversion – Obtaining controlled substances through deception or dishonesty for personal use or sale.

Tips for fighting fraud

The BCBSIL Special Investigations Department (SID) includes individuals with medical, insurance and law enforcement backgrounds as well as data analysts experienced in detecting fraudulent billing schemes. The SID aggressively investigates reports of fraud and refers cases for criminal prosecution. They also offer members these tips:

• Know your benefits and scope of coverage.

• Review all explanation of benefits (EOB) statements – Make sure the exams, procedures and tests billed were ones you actually had with the provider who treated you.

• Understand your responsibility to pay deductibles and copayments – Know what balances you can and cannot be billed for once your claim has been processed.

• Guard your health insurance card and personal insurance information – Notify BCBSIL immediately if your card or insurance information is lost or stolen.

• Sign and date only one claim form per office visit.

• Never lend your member ID card to another person.

• Don’t give out insurance or personal information if services are offered as “free” – Be sure you understand what is “free” and for what service you or your employer will be charged.

• Ask your providers exactly what tests or procedures they want you to have and why – Ask why the tests or procedures are necessary before you have them.

• Be sure any referrals you receive from your network provider are to other network doctors or facilities – If you’re not sure, please ask.

• Monitor your prescriptions at www.express-scripts.com – Make sure the medications billed to your insurance are listed accurately.

Reporting fraud

BCBSIL encourages you to report any suspected incidence of fraud by calling the Health Care Fraud Hotline, completing a form online, or sending a note in the mail. Suspicions of fraud can be reported to the SID anonymously.

There are three ways to report fraud to BCBSIL:

• Call the toll-free Fraud Hotline at 800-543-0867 24 hours a day, seven days a week – You can remain anonymous or provide information if you want to be contacted by a member of the Special Investigations Department.

VSP® Vision Care

VSP provides personalized vision care for you and your covered dependents. When you visit a doctor in VSP’s broad network, you receive discounts on services, such as exams and hardware, including glasses and contacts. You can still see other providers, but your costs will be lower when you visit a VSP provider. Please note that not all plans include vision coverage.

VSP providers

To find a VSP provider, log in to www.vsp.com or call 800-877-7195 and we’ll help you find a provider who is right for you. VSP does not send ID cards. When you make your appointment, tell them you have Boeing VSP services. Check your coverage on www.vsp.com before you go so you have a clear idea of what’s covered. Your VSP provider will submit claims for you. Benefits include:
