Key Activities – Jharkhand

1. Preparatory meeting on online NQAS assessment of District Hospitals

A follow up meeting to State Quality Assurance Committee meeting held on 11th September 2020 under the

chairmanship of the Secretory Health and presence of MD – NHM, to conduct online NQAS assessment for

District Hospitals as soon as possible was conducted on 21st September 2020 for orientation of assessors for the

upcoming online NQAS assessment. The meeting was conducted by State Consultant – Public Health & Quality

Assurance (PHQA) and attended by STOs (Dr. DP Taneja and Dr. Sourav) from USAID Vriddhi state team and

other identified state assessors. The following points were discussed in the meeting:

• The recent NQAS checklist was discussed by State Consultant – PHQA. It was reiterated that only this

checklist to be used.

• Wherever required records to be reviewed in online video call before marking a measurable element. • Any change in the schedule is to be informed to the facility and the State QA cell

• At least 12 departments are to be reviewed per facility. But if there is more assessable department that may

be included eg. Functional SNCUs in some facilities.

2. Virtual state assessment of DH Ranchi and DH Saraikela for NQAS certification by state Quality

Assurance Unit from 21st to 23rd Sep 2020 (3 days)

USAID Vriddhi being part of State Quality Assurance Unit (SQAU), conducted virtual state assessment of District

Hospital Ranchi (Dr D P Taneja, STO, Team D of National Quality Assurance Unit – NQAU) and District

Hospital Saraikela (Team A of NQAU) for NQAS certification along with other team members of the state

maternal and state quality assurance cell between 21st to 23rd Sep 2020.

3. Online mentoring of Giridih labour room by MNH resource center

An online mentoring of Labor Room at SNCU Giridih was conducted on 22th September by MNH Resource

Center mentor; Dr. Shilpa Tigga - Senior Resident ObGyn, RIMS. The DS Dr. Upendra Das, MO in charge of LR

- Giridih, Dr. Megha, sister in-charge, Nilu Kispotta and her team from the labor room and program in charge

Dr. Deepawali - Nodal Officer, MH cell NHM joined the meeting. Dr. Ritu Agrawal (Senior Technical Advisor)

from national team and STA from USAID VRIDDI state team also joined the mentoring.

The case management practices during childbirth with labour room staff of Giridih was discussed in detail as:

Evidence-based care during 2nd stage of labor by skilled birth attendant.

Evidence-based care during 3rd stage of labor by

skilled birth attendant.

Evidence-based routine postpartum care in

immediate postpartum period by skilled birth attendant.

Evidence-based management of primary post-

partum haemorrhage by skilled birth attendant.

• Evidence-based care during childbirth for

hypertensive disorders by skilled birth attendant.

All the aspects of mentoring checklist was covered and

indicated great scope of improvement and the labor

room staff assured to make all round changes in quality

of care practices for improvement in the labor room.

4. Online meeting of State Child health cell and USAID Vriddhi with SNCU, RIMS

State Child Health Cell in coordination with USAID Vriddhi state team conducted an online meeting with

Department of Neonatology, RIMS on 23 September to discuss issues faced by SNCU RIMS.

The key points of discussion were:

1. Equipment

Radiant warmer Outdated, Maintenance required occasionally / new one required.

LED phototherapy is not available (Old phototherapy machines not working properly).

Pulse oximeter and Infusion pump (out of 15 only 6 are working) replacement needed

5. District Dumka LaQshya support through Mock drill on OSCE (AMTSL, PPH Management and

Eclampsia management) DTC- Dumka, USAID Vriddhi team, conducted a mock drill on OSCE (AMTSL, PPH Management and

Eclampsia management) at DH Dumka on 23rd Sep 2020 in which LR and MOT staffs of Dumka Medical

College and Hospital participated.

6. 2nd online mentoring of Giridih SNCU

by MNH resource center

An online mentoring of SNCU Giridih was

conducted on the 24th September by MNH

Resource Center mentor’ Dr. Parmanand

Kanshi - Neonatology department, RIMS.

The DS Dr. Upendra Das, MO in charge of

Giridih SNCU Dr. Govind prasad and his

team from the SNCU along with program in

charge Mrs. Rajbir Kaur MCH consultant

from NHM joined the meeting. Dr. Ritu

Agrawal (Senior Technical Advisor) from

national team, STA and Associate Manager

(M&E) from USAID Vriddhi state team

also joined the mentoring session.

The case management practices of sick new-born of SNCU Giridih was discussed in detail as:

Appropriate diagnosis and investigation of neonatal

sepsis.

Appropriate treatment of neonatal sepsis

Specific feeding needs of sick young infants and those

with low-birth weight.

Recognition and management of jaundice.

Management of asphyxiated new-born.

All the aspects of mentoring checklist is covered and indicating

great scope of improvement, few improvements is already

initiated in the clinical practices of the SNCU and there is

noticeable understanding and knowledge adaptation has been

seen in the staff of SNCU Giridih district hospital.

7. Online LaQshya review of Hazaribagh medical College and Hospital on 24th Sep 2020: An online

LaQshya review of Hazaribagh medical College and Hospital on Medical Supritendent’s request was

conducted on 24th September 2020 using ICT platform under the chairmanship of State Nodal Officer -

Maternal Health. State MH consultant, USAID Vriddhi team (from State and District) and representative

from UNICEF along with nodal LaQshya faculty from OBG, DPM, Hospital Manager, and UNICEF district

representative attended the meeting. The following points were discussed in the meeting:

Dr. Sweta Laal, Asst Prof OBG will continue to act as LaQshya nodal for facility, coordinate & finalize the

SOP along with arrange for regular trainings

Procurement: DPMU to initiate the file and send it to Medical College administration for approval

Letter from state regarding deploying only SBA trained staff at the LR will be shared with the facility

Responsibilities to conduct various audits will be distributed by LaQshya nodal officer

Patient Satisfaction survey: Hospital Manager will coordinate with LR IC and SNCU data operator and

outcome indicators

Regular OSCE to be initiated. Daksh (National Skill Lab) trained nurses to coordinate

VRIDDHI project will support for documentation work

The DPM said that new staff nurses after recruitment by the district will be shared with the Medical College

Key Activities – Odisha

1. Advocacy for inclusion of VRIDDHI implementation model by State NHM for discussion in regional

MH review conducted by Ministry of Health and Family Welfare, Govt. of India:

Regional Maternal Health review for the Eastern States was scheduled for 23rd September but has been

postponed and new date will be announced. USAID Vriddhi team has carried out successful advocacy with

State NHM and provided brief notes on online LaQshya mentoring and pilot intervention for improved FHR

monitoring in Kandhmal district. Consequently, Vriddhi pilot intervention on improved FHR monitoring by

MOYO handheld Doppler has been included as State innovations in the presentation made by State NHM for

forthcoming MH review. State NHM has also introduced online LaQshya mentoring by VRIDDHI team under

MH best practices head.

2. Sharing Vriddhi support activities with new Additional Director (MH):

USAID VRIDDHI team scheduled a meeting with Dr. Mono Majhi, Additional Director – Maternal Health,

Directorate of Family Welfare Government of Odisha on 22nd September. Mr. Saroj Panigrahi, State

RMNCH+A Consultant also attended the meeting. New AD(MH) was briefed on Vriddhi’s technical support

in maternal health programmes including LaQshya, improved FHR monitoring through Moyo dopplers and

capacity building of providers on Safe Delivery App (SDA). AD (MH) has expressed his interest on visiting

health facilities to understand Vriddhi interventions including LaQshya quality improvement initiatives in

health facilities. He has suggested to plan the field visits once the COVID-19 situation becomes normal. He

shared that he has been temporarily deputed to Puri district for mentoring COVID control activities and he

would like to schedule another meeting with Vriddhi team to understand the progress in LaQshya.

Key Activities – Haryana

1. LaQshya review meeting at SHKM GMC, Nalhar

USAID Vriddhi state team conducted a LaQshya Review

Meeting with Director SHKM GMC, Nalhar Medical

College on 18th September, along with nodal persons

responsible for civil work and purchase. Associate

Professors, Assistant Professor from ObGyn Department

and District Technical Consultant USAID Vriddhi team

participated in the meeting. DTO briefed the Director

regarding the requirements for LaQshya and status of LR

Complex of SHKM GMC, Nalhar with regards to

LaQshya program. Discussion on Vriddhi technical

support to Medical College in the LaQshya Program was

also held. The major gaps as per the LaQshya Checklist

were highlighted and action plan was discussed. The

timeline for civil work completion and procurement was

decided to be with in next 3 weeks. It was decided that the internal reassessment using LaQshya Checklist is

planned for 10th October 2020.

2. District review meeting led by Director Health Services at Nuh, Mewat - 21/9/20

DTO, USAID Vriddhi team convened a

Review Meeting of the district on 21st

September under the chairmanship of

Director Health Services Nuh, Mewat.

Review of all health programs of district

was done. District Technical Consultant

presented updates of Vriddhi project in the

meeting. Discussion was done on:-

LaQshya Program, plan for starting new delivery points at Kaliyaka and Bai in Aspirational District,

Mewat. Status of newly started delivery Points at Bichor and Marora inaugurated on 18 August by the

Deputy Commissioner.

The FPC monthly progress report in SNCUs and NBSUs of Haryana was shared.

Discussion on SNCU State review online meeting that was held on 8 September was done. It was shared

that for NBSU Strengthening - Online NBSU Staff Training on Facility based management of small and

sick Newborn that will be held in the collaboration with NHM, Haryana.

Discussion on SNCU quality care index (SQCI) and DHQC Dashboard was conducted.

Key Activity Chhattisgarh

1. Preparedness for District Orientation on Generation and Interpretation of

SNCU Quality of Care Index (SQCI)

Using SNCU data for quality improvement by the facilities has been gradually improving

but an accelerated approach is required. In order to empower the facilities for using their

data for action, SNCU Quality of care index has been pioneer in the state. SQCI is being

generated by state with the support from Vriddhi project and since last Quarter the process

has been initiated to hand it over to districts.

With the objective to timely initiate action based on SQCI index a state level orientation

is scheduled for 25th Sept.20. The expected outcome from the orientation of SNCU Nodal

are

1. Gain understanding on need of actionable information system in SNCUs

2. Gain knowledge on SQCI and its benefits

3. Development of capacity of the districts on

• SQCI generation

• Interpretation of SQCI

• Action planning to resolve gaps

Interactive Virtual media will be used to orient the facilities and post orientation support

will be provided to analyze the index and develop an action plan. An improvised version

of tool has been shared with district to generate their SQCI. During orientation the nodal SNCU and program

officer will be inducted to develop action plan.

Way forward

Successive follow up will be made to review the SQCI generated and action plan prepared.

Record the progress made after using SQCI data for improving Newborn outcomes

Internalization of SQCI during district CH review meetings and discuss actionable points at local level

(SNCU & District

In combination with supportive supervision visit, discuss and understand underlying causes for low score

in the identified parameter.

Key Activities - Assam

1. Meeting with State Head, HLFPPT for Mobile Medical Unit on service data accumulation for planning

of training on IMNCI

A meeting was conducted with the State Head, HLFPPT Mobile Medical Unit Mr. Saurabhjit Uzir for

acquiring service related data, in order to plan for IMNCI training for service providers including Medical

officers and nurses of 59 Mobile Medical Units operationalized in 5 intervention districts of Upper Assam.

Based on the data gathered, tentative training plan for tea garden health care providers (59 MMUs) on IMNCI

package was prepared and shared with team for further discussion and finalization.

2. Orientation on Safe Delivery App for its implementation in the targeted Tea Garden Hospitals

Assam team participated in an orientation session on Safe Delivery App conducted by Maternity Foundation.

The orientation mainly focused on the introduction of the App followed by demonstration and hands on. Few

quick exercises were also conducted for easy understanding. Later on focus was bestowed towards its

practical application in installing followed by orientation of the tea garden health care providers for

improvement in knowledge and skills.

3. Meeting with ECHO India team to discuss on the continuation of the ECHO platform

A meeting was convened with team ECHO India to discuss the modalities about the continuation of the ECHO

platform for the Medical Officers and the nursing staff of tea garden health facilities. The meeting mainly

focused on signing a fresh MoU between ECHO India and IPE Global Limited for continuation of the

program. The discussion led towards proposing for orientation and immersion of staff early in the next month

and on provision of zoom license to be extensively used for ECHO curriculum.

Training cum review of Pharmacist, HWO & ASHA on FPLMIS using

online platform in Meghalaya

An online training on zoom platform was conducted on 19 Sep 20 of the Pharmacist, HWO and ASHA of

Phulbari CHC, West Garo Hills district, Meghalaya. The objective of the online training was to orient HWO,

Pharmacist & ASHA on FPLMIS; Review of the FPLMIS progress under West Garo hills District and to share

COVID19 related information and precautions. The meeting was conducted in two parts, training of ASHA &

HWO in the first half and review meeting on FPLMIS progress in 2nd half. During the training on FPLMIS app,

they were made to learn how to indent and update. During the review they were shared Covid 19 related

information & precautions with the participants, reviewed the performance of all FPLMIS indictor, stock out at

DWH as well BWHs and training status. In total 55 ASHA attended the training out of which 22 ASHA

indented during the training. The training was facilitated by state FPLMIS consultant, Ms. Shamina.

Key Activities – Himachal Pradesh

1. Meeting of State subcommittee of standing committee for MH

A virtual meeting of subcommittee constituted to

formulate comprehensive SOP’s for LaQshya

facilities was conducted on 18th September. The

members present in the meeting were:

2. State webinar on Poshan Maah 2020 for Management of Severe Acute malnourished children:

Participated and supported state NHM in state webinar on Poshan Maah 2020 for Management of Severe Acute

malnourished children conducted on 18th Sep 2020. USAID Vriddhi team supported by providing Zoom

platform of IPE Global for the meeting and broadcast of event on NHM HP youtube channel. Field health workers and WCD workers participated in the webinar.

3. Online State LaQshya review meeting with all District Quality consultants

USAID Vriddhi state team provided technical support to

NHM in conducting the state review meeting of

LaQshya on 23 September under the chairmanship of

SPO MH and Quality. USAID Vriddhi team along with

consultant – Quality – NHM and District Quality

consultants (Bilaspur, Chamba, Hamirpur, Kangra,

Kinnaur, Kullu, Mandi, Shimla, Sirmour, Solan & Una)

participated in the meeting. Review of LaQshya

activities from all districts was done and way forward

plan of state assessments was taken from districts.

Key Activities – Uttarakhand

1. Webinar on District Hospital Quality Care Index (DHQC) for handing over process

The tool District Hospital Quality of Care Index (DQCI) aims to provide regular performance tracking of the

hospital for evidenced decision making by facility in charge & program managers at district and state level. In the

state of Uttarakhand, the DHQC is being prepared for USAID-VRIDDHI project supported two aspirational

districts. During the discussion for handing over process with concern state officials it was decided that this tool

can be implemented in all 13 districts of Uttarakhand. Accordingly, the State NHM with technical support from

USAID-VRIDDHI project organized a webinar on Sept 24, 2020. Around 50 participants participated in the

webinar from all the Districts. The key highlights of the webinar are as follows:

1. Ranking of District based on Sores in DHQC

2. The DHQC can be integrated with Kayakalp and Laqshya

3. Timeline for generation of Dashboard and improvement plan.

The end line assessment along with telephonic interview was scheduled in the current week for ongoing SDA

study. The scheduled interviews and assessments were executed according to the plan.

Activity: Monitoring of ARI Cases

Vriddhi team from National level is regularly tracking ARI cases from 19 Health and Wellness centers. Total 63

cases of ARI screened in this week and 10 cases were diagnosed as pneumonia.

Impact on RMNCH services in project states

Services in all states have opened up in areas outside the containment zones and there is focus on resumption of

essential RMNCH services. State NHMs have been organizing online review meetings to discuss service

coverage indicators from HMIS.

However, spurt in COVID cases in states such as Jharkhand, Odisha, Uttarakhand, have resulted in services

getting disrupted in pockets.

The inputs obtained from project states reveal that the status of RMNCH services are largely the same as

previous week-

Outreach services

Antenatal Care

services

PMSMA was organized on 9th September 2020 as per GoI guidelines in Odisha,

Jharkhand, Haryana, Punjab, HP & Uttarakhand. States are actively following up

with the districts for ensuring social distancing and prior line listing of beneficiaries for PMSMA day. However, In Chhattisgarh, PMSMA has not been held since March

2020.

Use of e Sanjeevani for consults has been rolled out.

VHSND are being held as per GoI guidelines And Line listing of HRP is being done.

Home visits Integrated surveillance being undertaken by FLWs across states. ASHAs are doing

COVID related screening in the community and undertaking integrated surveillance for active case finding for TB, screening for NCD, surveillance and sensitization for

NDD and IDCM.

Routine

Immunization

RI in VHSNDs are functioning beyond containment and buffer zone as per GoI

guidelines maintaining social distancing

HBYC services ASHAs are making home visits along with COVID screening outside buffer zone.

HBYC is operational in only one district of HP, AD Chamba.

HBYC home visits not yet started in Chhattisgarh, as trainings have not been

completed.

Family Planning Home Delivery of Contraceptives by ASHAs is ongoing in all states and all

temporary methods (Condoms & Pills) are being provided to beneficiaries.

In MP, A new initiative under KILL Corona Abhiyaan , in which ANM & ASHA

will be involved in survey/screening. Instructions has been issued to distribute FP commodities during home visits.

Facility services

ANC services In all states facility level ANC services are being provided on walk in basis using

staggered approach* and following physical distancing norms. Non-COVID PW being transferred in non-COVID vehicles only is being ensured

Institutional

deliveries

All states have identified facilities both COVID and Non COVID for provision of

institutional deliveries and services remain uninterrupted. Blood Banks are

functioning.

Newborn care Birth doses continue uninterrupted, as does promotion of Breast-feeding practices

with early initiation of breast feeding and Kangaroo Mother Care.

Immunization

services

are being provided at facilities, for walk-in beneficiaries. Daily immunization for all

PW & Children at DH & SDH has been resumed in Haridwar district

Admission to SNCU SNCUs are largely functional and providing services.

In Jharkhand SNCU (Rajmahal) remains closed as is converted to COVID ICU.

In Himachal Pradesh one SNCU at Medical college Nerchowk Mandi, is not

functioning as facility is a dedicated COVID Hospital.

In Odisha and in Haridwar district, SNCU services are affected where the facility is

shut down due to positive cases found among providers

Family Planning

services

Now with increase in cases temporary closure of facilities is common and affecting

services.

In Jharkhand, Sterilization and IUCD/PPIUCD services ongoing but not in full pace.

For suspected cases, first COVID test need to be done. Few CHC has been converted

to COVID center due to which FP services especially sterilization have been

hampered.

IUCD/PPIUCD services are available in all designated health facilities in states.

Antara services are available at facility level

Sterilization services not still started in Arunachal Pradesh & Tripura

*Staggered approach- Where adequate seating for beneficiaries and caregivers with physical distancing is

ensured
