Part -I
VDPV2 Situation Update
Advisory Group discussion, conclusion and recommendations
• The Somalia mOPV2 advisory group meeting was held on November 20th
• The country program presented detailed risk assessment on the VDPV2 situation and requested for the approval of a mOPV2 immunization campaign response
• Risk Assessment• Virological risk: Medium to high• Contextual risk of further
transmission= Medium • Risk for international spread= Medium
• Assess need for mOPV2 and evaluate vaccine request [if applicable]• mOPV2 request received by advisory group and was approved as per
country program request • IPV [proposed for under 2 in both scenarios after the 2 mOPV2 rounds]

Risk Potential: “Low / Medium / High”
Background to VDPV2 from an ES Site in Mogadishu• A VDPV2 was isolated from an environmental sample from Mogadishu-Somalia
• The ES site is ‘21October’ Sewage site in Waberi District, Banadir Region in Central Somalia. Established on 19th September 2017.
The first positive sample was collected on 22nd October, 2017.
• KEMRI lab results was received on 1st Nov 2017 as PV2. Sequencing result from CDC was received on 9th Nov 2017 as VDPV2 with
38 nucleotide difference from the parent Sabin
• A second sample collected from the same site on 2nd November was also positive for PV2 and is awaiting sequencing at the CDC
Response activities so far;
• Joint visit conducted in selected public and private HFs to see the AFP surveillance activity and also sensitize the health workers about the event
• Heightened AFP surveillance with active case searches in the community and health facilities
• Additional ES samples collected from the existing 3 sites and from a new temporary site in one of the busy hospitals
Egyptian hospital- Mogadishu Yardimeli hospital- Mogadishu Banadir hospital- Mogadishu
Preparation for OPV2 response
• Two mOPV2 and 1 IPV campaigns are planned for Badadir, Middle Shabelle and Lower Shabelle regions of Central zone targeting 726,699 Under 5 years old children
• The proposed dates for the first round is 10th December• There will be 2 weeks interval between the two mOPV2
rounds • There will be also 2 weeks between the second mOPV2
round and the IPV campaign
Preparation so far• Micro-plans prepared• Training for RPOs, DPOs and DFs and data managers on how to
use satellite maps to enhance their micro-plans and identify DFA and team areas
• Vaccine will reach Mogadishu 30th November • Plans for C4D, Vaccine management and monitoring is being
developedDPO and DFAs identifying DFA areas
Catchment area mapping -21October sewage spot
• Using historical maps, conducting
GIS studies (elevations & contours)
of the area and discussion with
municipality sewage experts and the
local polio team, the catchment area
of the 21October ES spot was
identified
• Filed validation was also conducted
through household survey to
identify the actual boundaries of the
sewage system
Household survey in the catchment area of 21October sewage spot • A structured questioner was developed,
converted to ODK and 40 data collectors were trained (20 team of 2 individuals)
Objectives HH survey:• Collect geo information to know the
catchment area of 21October sewage site• Understand if households drain their west to
the main sewage system which is feeding the 21October sewage site
• Assess if there is/are any child in the household/ neighborhoods paralyzed in the past 12 months
• see vaccination status of all U-5 children in the HHs against polio checking their vaccination card
• See if there is/are any person who arrived in the past 3 months
• See if there is/are children who is sickly and visits the hospital frequently in the HHs
• Data collected from a total of 888 buildings. Of which, 476 are households, 234 are empty buildings, 81 are educational institutions and 60 are restaurants and hotels, 13 mosque and 24 other categories
• An average of 1.5 children found per each HH with a maximum of 5 children per house hold
• Only 11% (n=55) of HHs have their sewage draining to the main sewage system
• There are 58 children found sickly with frequent visit to hospitals; among them 8 had their household sewage draining to the main sewage system feeding the 21October site
• 59 households had a visitor in the past 3 months
Next step using the HH survey result
• Continue to analyze more variables from the survey that was just completed
• Plan to re-visits to some households including the 8 households that had children who were sickly with frequent visit to hospitals and their household sewage was linked to the main municipal sewage system draining in to 21October site
• Continue the active case search in the area
• Use this information to enrich the micro-planning process for the upcoming immunization campaigns

Surveillance Indicators (NP-AFP rate and Stool adequacy) by district, 2017
All the 115 districts in Somalia have reported
at least 1 AFP case since the beginning of
2017.
The NP-AFP rate for 113 districts is >2. The 2
districts that did not meet this indicator are;
Badan from Somaliland and Heliwa from
Central.
112 districts had stool adequacy above 80%
while three districts (Badade, Kismayo and
Bossaso) did not meet the required stool
adequacy rate. Badade and kismayo are
partially accessible from south while Bossaso
is fully accessible district in Puntland.

the country from all zones, all regions
and also from the accessible and the
inaccessible parts of the country.
All but one of the total 41 zero dose

reported from access compromised

94% (n=300/318) of AFP cases reported in 2017 are notified within 7 days of onset. 18 cases were notified more than seven days after the onset of paralysis from 7 regions {Lower Jubba (8), Gedo (3) , Middle Juba (1), Bari (1), Sanaq (1), Bay (2) and Galbeed (1)}.

Surveillance indicators at national and sub-national level perform above global standard EV isolation is low in Somaliland and the zonal team are investigating this, with focus on the reverse cold-chain practices Except 2 AFP cases; one from El-Waq district of Gedo region in South zone and one from Buloburti distrct of Hiran region in Central zone, all the other AFP cases were investigated within 48 hrs of
notification Three cases from Badade, Bosaso and Kismayo districts (one from each) had low stool adequacy levels.

VPVs continue to support AFP surveillance and have reported 81% (n=34/42) of cases from inaccessible and 47% (n=38/81) from partially accessible districts.
Most of the surveillance indicators in the accessible and access compromised areas are above global standards; however, SL isolation is very low in the access compromised districts, there are challenges in providing immunization services in those districts

Fever and rash Surveillance
- After showing a steady decline for about 14 consecutive weeks starting from week 24, the numbers have started to go up in recent weeks.- The cumulative reported cases also still remain higher than the number of cases reported at the same period in the past 4 years. - All regions have reported suspected measles cases in 2017, however, the top 5 reporting regions are Banadir, Togdhere, Hiran, Lower shabelle and Glbeed.

Since the first weeks of 2017, a total of 124,151 <10 years children were reached through the newly established permanent vaccination sites in fully inaccessible area in South and Central zones. 45% (n=56,448) of the children are vaccinated for the first time.

In 2017, 41 zero-dose NP-AFP cases were reported; 22 from South, 18 from Central and 1 from Puntland. 3 cases are off the age group 6-59 months. Review of five years data reveals, the proportion of zero dose AFP cases is declining progressively, however, there are significant proportion (32%) of children are under
immunized.

respective zones
