A. Brief Introductory Statement:
Dental Hygiene Program at Weber State University is three years in length. The first year consists of pre-requisite courses which may be completed at any accredited college or university that are acceptable transfer courses recognized by Weber State University. The next two years (4 semesters) must be taken on a full-time basis and includes professional dental hygiene courses offered on the WSU campus. A fourth year, leading to a Bachelor's Degree in Dental Hygiene, is optional. Enrollment in the WSU Dental Hygiene Program is limited to 30 students each fall semester. After acceptance into the program, students spend 35 to 40 hours each week in lecture and clinical dental hygiene courses. After completing the dental hygiene program and graduating from the university with an associate or baccalaureate degree in dental hygiene, a dental hygienist must become licensed to practice dental hygiene. Licensure is dependent upon successfully completing a written National Board Examination, a clinical dental hygiene examination, and specific state licensure requirements (i.e., written test on state laws, CPR certification) and evidence of graduation from an accredited dental hygiene program. A dental hygienist provides direct patient care to all individuals according to the principle that all people should have access to health care. Therefore, the dental hygienist will treat individuals with potential blood borne diseases in their education program and in their professional practice. The dental hygiene student will have the knowledge and skills to safely treat patients regardless of their health status and to respect the confidentiality of all patient information.
B. Mission Statement
Dental Hygiene Program Mission Statement The mission of the Dental Hygiene Program at Weber State University is to offer education and training with an emphasis on the integration of basic sciences education, clinical experiences and liberal arts which enable the dental hygiene graduate to make a maximum contribution to patient care as a member of the dental health care team. To achieve those ends, the department has created a learning environment which includes activities conducive to learning. Further, the Dental Hygiene Program supports as its clinical component the Dental Hygiene clinic. DEPARTMENT GOALS The Weber State University Department of Dental Hygiene is designed to produce individuals who are capable of meeting the needs of the dental hygiene profession and of society. The dental hygiene profession needs practitioners who have been instructed in the psychosocial, biophysical, and technical elements of dental hygiene practice. These practitioners should be able to expand current parameters of knowledge, as well as to apply existing behavioral and basic science concepts to specific problems. Furthermore, these individuals must be capable of making decisions, knowledgeable of public affairs and motivated to become life-long learners. They must be individuals whose behavior is guided by an internalized code of ethics. The specific goals of the dental hygiene program are to provide students with the:
1. Necessary biophysical, psychosocial and dental hygiene knowledge requisite for a comprehensive understanding of dental hygiene practice; (knowledge goal)
2. The technical skills, both fundamental and advanced, essential to the practice of dental hygiene; (knowledge and technical skills goal)
3. The reasoning and judgment skills necessary to engage in problem-solving behavior; 4. The theoretical bases, research and communication skills, necessary for the acquisition, advancement and
dissemination of dental hygiene knowledge; (knowledge goal) 5. The leadership and initiative capabilities essential for assuming roles of responsibility within the dental hygiene
profession and society; (knowledge goal) and

6. The moral habitude requisite for the effective performance of responsibilities within dental hygiene practice and within society through abiding by the ethical principles of the Professional Conduct Code which defines the professional responsibilities and conduct of a dental hygiene professional (values and ethics goal).

Assessment: Academic Standards Students will be evaluated and receive academic grades in all clinical and non-clinical courses. General education and prerequisite courses that are required within the dental hygiene curriculum must have an academic grade assigned by the university and all grades must be posted on the student’s transcript. Credit/non-credit course completion is accepted for DENT 2250 Professional Ethics and DENT 3301 Community Service Learning Lab only. All courses must be passed with a “C” grade or better, a “C-“is not acceptable. The student’s percent earned in each course is accompanied by the following course/examination grade. This is the recognized standard within the program for all courses.
A 95-100 B- 80-83 A- 90- 94 C+ 77-79 B+ 87-89 C 75-76 B 84-86 74 and below; non-pass
Students who receive below 75% in any course may not be allowed to continue in the program and will either be; 1) required to repeat the course or 2) be dismissed from the Dental Hygiene Program for unsatisfactory academic performance. If the student is dismissed, the student will need to re-apply to the Dental Hygiene program for acceptance to begin the program again, if they so desire. The student will not be given special consideration for re-acceptance or re-admittance.

High Impact Learning Aspects of Curriculum: Dental Hygiene is primarily a knowledge and technical skill discipline. Through the interdisciplinary nature of the Dumke College of Health Professions, the Dental Hygiene Program offers students opportunities to (a) a. Receive upper division elective credit for Study Abroad experiences that explore diversity, culture, health care and/or language. To date, Dental Hygiene student experiences have included: Guatemala S.A., Costa Rica, Ghana Africa, China (Study of Healthcare and Culture in China (2007-2014); participants totaled approximately 25 students which have been influential in generating SCH and increased number of Baccalaureate degrees with the Dental Hygiene Major. b. DENT 4780 Baccalaureate Thesis/Capstone. This course offers students the opportunities either to participate directly in applied clinical research, work as an interdisciplinary student group incorporating research to include other students at WSU (for example, laboratory science methodologies). c. The American Dental Hygienists’ Association national, regional and local meetings and on-campus Undergraduate Research Symposia provide students with opportunities to present their research through formal presentations and share findings with the department, campus and community. Community Involvement and Service Learning: DENT 3301 CEL Community Service Learning Lab (1 cr) and DENT 3336, DENT 3337 CEL Dental Hygiene III are courses that have obtained the CEL designation. Outreach activities include: Title 1 school dental health program; dental screenings and sealant clinics for 1 and 2 graders, Head Start Fluoride varnish program, Youth Impact dental education programs, Community health fairs in Weber and Davis counties, outreach to local High Schools through AHEC partnerships and others. The number of courses/sections and the number of students who have participated in each of the following –specific data on community engaged learning activity. DENT 3347 (Spring 2014) faculty: Prof Susan Alexander Students: all senior students participated in a wide-variety of projects. Project Title: Education and Encouragement, Community Partner: O & A (Observation & Assessment-Juvenile Facility, Weber County) PLAN: We want to give a presentation that focuses on nutrition/diet, etiology of caries and periodontal disease, effects of smoking and alcohol consumption and proper brushing and flossing OBJECTIVES/GOALS: We want to education the kids that are in the facility to help them improve their oral health and overall health. The ages range from 12-18 and there are both boys and girls in the facility. They are under strict supervision and their progress is monitored. They are in the facility for a maximum of 90 days and most of them do not go to the dentist on a regular basis if they have ever even been. We want to help them understand the important of caring for their mouths because we may be the only ones that ever teach them how to properly take care of their oral health. SUPPLIES: Toothbrushes, dental model, disclosing agent and flip charts. Leisa Jensen’s reflective paper, re: off-campus clinical rotation: Over the last 14 weeks, I had the opportunity of cleaning patients at the new Midtown Dental Clinic. I had many positive experiences in the office. Most rewarding was a patient who came in with so much calculus, I thought she would be the perfect board patient. After looking at her x-rays and checking probe depths, she had too much bone loss to be one. So I started cleaning her. She mainly spoke Spanish and I tried to explain to her that she had a disease. It was interesting to see how well she took the appointment. She wanted some local anesthesia to help her not feel the pain of her sensitive gums. She told me that she didn't feel the shot and that I did really good. This made me feel like I was getting better at giving my shots. I classified her as a class III due to her bone loss and calculus buildup. After I started cleaning, I was able to get two quads done in 2 hours. She was so patient with me and said she was numb the whole time. When Instructor Bunker came in for a scale check, I missed only two spots on the lower quad and about a million on the upper. I did spend more time on the lower arch than the upper, so it makes sense that I missed more on the top. It was so rewarding being able to clean a class III and only miss two spots in one quad. I have improved so much since last year. Just like in this example, I really enjoyed learning and being able to see myself get faster and become a better clinician. Another positive experience that sticks out in my mind is a six year old girl. This was her first time at the dental hygienist. Her mother

said she let her six year old daughter brush and floss her own teeth. It seemed like the mother didn't teach her daughter at all how to brush. This young six year old had more calculus on her lower anteriors than I see on most !B patients. Even the front of her teeth had some build up on them. The little girl was so good in the chair and she would smile and giggle when I would talk to her. I educated her about how to brush and floss and the reasons why we do that. I also educated her mother on this as well. I told her mother that she needs to help her daughter with her home care for a little while longer and monitor how she is brushing and flossing. The mother seemed to even learn a few things and seemed like she was willing to take on this new challenge. After I cleaned this girls teeth, I gave her a mirror to let her see the difference and she was smiling the entire way out of the office. She loved her new smile. This is another reason why I want to be a great clinician to make a huge difference in people's life, no matter what their age is. This girl seemed like she had a new reason to smile. The setting that the clinic puts off is very calming. It felt almost like I was in a real office. It was nice to be far enough away from the dentists that my patient didn't have to hear drills and other noises that they would in a smaller office. I had many patients comment on how great the clinic looked. The feel of the office is great with the patients. All the kids I have had in the chair have been great. I think it is the attitude that we as dental hygienists have combined with the setting has helped this. I enjoy many things about being a dental hygienist, but at Midtown Dental Clinic I really liked how we don't have to move our patient from room to room to take x-rays. It saves so much time to just take it right there rather than move the patient from room to room. Another aspect of being a dental hygienist at Midtown that I enjoy is the fact that we get to see so many different people and classifications. I have seen a few of every class there and I have to say it was nice to see some true class III's. At Weber State, I have tried to get in many patients that are harder classifications who all have called and cancelled on me. On a different note, it is super nice to not have to find our patients for the day. I was most surprised to learn how a real office can be. If I get into an office that doesn't have all the scaler that I like or other hygienists that don't sharpen, I will be able to use what they have with confidence. I was also surprised to see how fast I needed to be in order to get everything done that I wanted. There were many times when I set a time limit to be finished scaling by and reached it. I usually missed a few spots but nothing huge. It feels good to now be able to say that I can finish a 1B in an hour and a half. This has given me confidence to schedule more than one patient in the clinic in the same clinic session. I am excited to see how fast I will be by the end of the semester. The whole first half of the semester I was frustrated because I was under the impression that every patient needed to be finished before we grabbed the next. I barely got a lunch on some days, if I even had time for one at all. I didn't think that instructor Bunker knew that we were supposed to be on a schedule like the school or she would have told us to be done at a certain time with the last patient. It was difficult when these things would happen and the person in charge didn't say anything to change the flow of things and just let them keep going the way they were. I didn't have too many difficulties in the clinic. The only major difficulty that I can think of is more with the equipment than anything else. I was in the operatory in the very end on the left. Every time the light is put high enough for the light to turn off it swings as far as it can to the back wall. It is a pain to get up and walk around the patient every time I need the light. With my patient that had to be sitting up in the chair, The light wouldn't even stay in place long enough for me to be able to stick my mirror in her mouth. I ended up having Sierra or Laken holding the mirror for me. I normally use loupes with a light on them, but they were getting fixed for two weeks. Sierra ended up letting me use her loupes with the light for the rest of the appointment. When instructor Bunker would come in for my scale check, I would get ready to record on the computer and I would have to walk all the way around, out of the operatory and back in to get the light for her. It is just a pain.
The computer system was another one that I had troubles with. When I would try and take x-rays at the beginning of the semester I would set everything up, hit the button to take the x-ray and it wouldn't show up on the screen. It was a mystery to me how it didn't work. Everything was set up properly it just didn't work. It has been better the second half of the semester. I would change a few things about the experience I had. I first of all would have had everyone share patients. In our class, on the second to last clinic day I had only seen one quad of a class III. Sierra on the other hand had seen a lot more. If I could go back, whether we were counting quads at Weber or not, I would ask that we would be fair about the patients that we were seeing. I would have liked more of an experience with some harder classifications. Looking back now, I would suggest changing how we educate the patients. There could also be a video that the patient and their parents should watch teaching them how to brush and floss their teeth, and why they have cavities. Just a thought to make it so we can see harder class patients and less 1A's in a normal clinic day and still get the experience of seeing children. The thing I enjoyed was probably seeing so many children and not as many harder class patients. I love helping out the children for Dental Days but I would have enjoyed my time if it was more of a rewarding patient selection. Seeing a million children who have no calculus on them was a struggle for me. I felt a little like I was wasting a lot of my time and patients who didn't have anything on their teeth.

My overall impression of the Midtown Dental experience is a positive one. It was nice to get the extra experience in a different type of setting. I feel that it was definitely worth while except the seeing a ton of 1A patients. I am very grateful for the harder patients I was able to see and the little lessons I learned every day to improve my scaling.
Amanda Chugg: reflective journal entry DENT 4890: Setting up the clinic was a new experience for me. Being at Midtown certainly took me out of my comfort zone. It was a new environment, using a computer system I had no experience with, and we had to work around the glitches that accompany a new office. One of my weaknesses is being easily frazzled when in a new environment or when things don't go the way they should. I feel like I've been able to go back and think through what I could have/should have done differently, which has helped me set a lot of new goals for myself. The office was beautiful, and I love having the extra computer screen to help with OHA! Today I saw a pt who was medically compromised. He needed premed, had a pacemaker, and was a difficult case. I felt like I learned so much, I loved it! The 6 teeth he had were treatment planned to be extracted, and there was a lot of very soft, exposed dentin and severe root caries. Afterwards he told me no one had ever been so thorough or spent so much time and attention on his teeth. He was very grateful. My pt. from last Tuesday came back so that I could finish up her treatment. She had a lot of calculus in the lower lingual anteriors (Which is my weakness for sure.) Professor Bossenberger was good about showing me how to get it off with the ultrasonic, but when I got back to it, I still couldn't remove it! It was really frustrating to me. It took me a while, but I felt good when I finally was able to get it all. Instructor Wold gave me some good pointers on how to better apply lateral pressure to get off the tough calculus. My fulcrum finger sure got a workout today! I feel like I could be better with the ultrasonic, so I'm going to find some tutorials to watch. All in all, a lot learned. Although i didn't meet any of the goals I specified for this course, I do feel like I learned a lot. Today was really great for me, as I feel that I got a lot accomplished. I finished all three patients in one day, which felt like an accomplishment to me!

Since the site visit (February 2010) specific patient age ranges have been defined and tallied for each student. Their patient experiences are evaluated through the Clinical Evaluation System, outlined in the WSU Dental Hygiene Clinic Manual. Supportive documentation provides evidence of patient treatment experiences (Exhibit 1). The definition of Special Needs has been determined and student experiences with treatment planning, implementing care and evaluation of its outcomes is also included (Exhibit 2). Previous to the site visit, the patient experiences for each student was

tracked through a definition of periodontal conditions, dental hygiene services provided and a broader definition of age range that lent itself to a less definitive tallying system. Further, it was known that a wide variety of patients with special needs were being treated in the dental hygiene clinic. The definition of special needs was further defined and tallied for each student’s experiences. These procedures were in place from February 10 through April 23, 2010, as the academic year ended. It is planned that continued measures of student experiences through specific logging of patient age and special needs features continue (fall semester). With this monitoring, every student will be assured of having an equal opportunity for experience and dental hygiene skill building with a variety of patients and a variety of patients with special needs.
Recommendation 2 It is recommended that the laboratory sessions in the dental science courses not exceed one to ten to ensure the development of clinical competence and maximum protection of the patient, faculty and students. (DH Standard 3-6).
Describe the progress made in implementing this recommendation since the site visit. Compare the current situation with that existing at the time of the Site visit. Previous to the site evaluation, DENT 2219 Dental Materials laboratory portion of the course had supervision of one faculty member: Dr Carol Naylor for the laboratory exercises (semester 3 of the program). This presented a 1:15 ratio of faculty to students. An additional faculty member, Professor Kami Hanson has been assigned to this course for its laboratory sessions. This additional faculty member’s supervision in lab will result in a ratio of 1:7.5 (Exhibit 4).
Recommendation 3 It is recommended that the program have a formal written patient care quality assurance plan that includes mechanisms to determine the cause of treatment deficiencies and patient review policies, procedures, outcomes and corrective measures. (DH Standard 6-2, c and d).

The written patient care quality assurance plan that includes an audit of patient records and treatment deficiencies was in place for the site visit. However, it lacked the formality of a rigid, systematic written plan that assures consistent mechanisms to determine cause of treatment deficiencies and patient review policies, procedures, outcomes and corrective measures. The plan has been revised to include the necessary components to assess the quality of patient care and assure ongoing documentation of outcomes and corrective measures (Exhibit 5).
At the August 2010 meeting of the Joint Commission on Dental Accreditation, the commission stated that “the recommendations cited in the report have been met” and the WSU Dental Hygiene Program has been granted “accreditation, without reporting requirements”. The next site visit for the program is scheduled for 2017.

A. Reflecting on this year's assessment(s), how does the evidence of student learning impact your faculty's confidence in the program being reviewed; how does that analysis change when compared with previous assessment evidence? (2013-2014) The program continues to have a strong presence in the community being part of the larger healthcare arena providing public health services. The students are eager to use new technologies (diode laser) for clinical patient treatment. There are plans in place to partner with Radiologic Sciences Department and create a 3D Cone Beam Computerized Tomography lab that will be housed in Dental Hygiene and shared by both department. Courses will be developed and laboratory practice will be incorporated. It is the belief of the faculty that our advances in teaching and learning, with both community learning and technology enhancement, will strengthen our confidence toward a positive review. (2012-2013) With the current analysis, it is evident that the program has its strengths in basic dental hygiene knowledge dissemination, dental hygiene skills student attainment and the community interactions. Entry-level dental hygiene education is in place with plans for additional technologies to be introduced and used in the clinical dental hygiene courses. Comparing to previous assessments, it is apparent that the structure of the program is appropriate and faculty are confident that our graduates are prepared to enter the workforce as dental hygienists and participate as engaged citizens. (2011-2012)The program continues to evolve and is challenged by technologies that are becoming available at the forefront of dental hygiene education and practice. After the review of courses and outcomes, it is evident that faculty have increased confidence in the program’s ability to move forward with advancements and have assurance that the knowledge and skills of the currently educated, graduated dental hygienist is in high demand. The WSU Dental Hygiene Program continues to focus efforts on improving the health of others by providing dental hygiene health care workers that will be skilled, highly qualified and committed professionals. Comparing this year to previous years analysis, the program is working diligently at keeping pace with current and advancing knowledge in health care. B. With whom did you share the results of the year's assessment efforts? (2013-2014) Assessment outcomes are consistently shared with our Program Advisory group that meets two times annually. They are instrumental in providing feedback and direction as the department considers additional projects and technologies in our program. The University Development office (our representative) stays current with our program needs and the outcomes that support our efforts. (2012-2013) Assessment outcomes are shared with aforementioned groups as well as the University Development representative for the Dumke College of Health Professions. It is important for the program to disseminate information to all stakeholders, on and off-campus. (2011-2012) Assessment efforts are shared with all communities of interest, including the American Dental Association Accreditation Annual Report, the Dental Hygiene Program Advisory Committee, faculty (full time and adjunct) and staff members, in printed information for students pursuing dental hygiene as their educational goal. (2013-2014) As the technologies available increase and the many strategies for teaching delivery systems, the program has committed to a routine course review. The goal of this review process is the collaboration of faculty in planning for change and reflecting on past course teaching methodologies that could be enhanced. On-third of the courses will be reviewed each year through planned faculty meetings that focus on curriculum review only. Seven courses will have an in-depth review, while all courses will receive an overview of their course topics, methodologies in teaching and receive feedback from program faculty.
(2012-2013) Continue to review curriculum content and sequencing for goal attainment. Evaluation for technologies to be incorporated into the curriculum at all levels in a timely way. Service Learning continues to evolve in depth and breadth. The community activities will be enhanced in their direct and indirect outcome measures. Maintain and expand artifact collections to demonstrate student learning. (2011-2012) Review the new Standards for Accreditation of a Dental Hygiene Program in an effort to ensure that all standards are being met or exceeded. Continue to review curriculum sequencing and content to assure program goal attainment. Plan for several measureable outcomes for Service Learning to include both direct and indirect measures. Carry on with collecting artifacts to demonstrate students’ learning and activity in the program.
