We Are MovingWe are moving to a new purpose built healthcare facility in Brackley in Spring 2020.

The construction of Brackley Medical Centre and Community Hospital is well underway with an estimated completion date of May 2020. This is an important project for the town and the surrounding area and will play a vital role in the provision of high quality healthcare for decades to come. Our surgery at Washington House and the existing Brackley Health Centre, accommodating community staff and services, is inadequate for the needs of the population. The cottage hospital closed and there are no longer community beds in Brackley. 

We had a vision to develop this new facility working together with Northamptonshire Healthcare NHS Foundation Trust (NHFT) to provide modern healthcare facilities on one site. This is in line with NHS plans for Primary Care which aims to integrate community services with General Practice. 

This exciting project comprises a three-storey medical centre and community hospital with GP Services, a pharmacy and a cafe at ground floor level, community services on the first floor and 16 beds on the second floor, with ample free car parking spaces.Whilst the GP services are for patients registered at Brackley Medical Centre the other services in the building and the community areas, such as the café, will be for all local residents and the wider community. NHFT have confirmed they will provide the rehabilitation and short–term care beds, for those who require that extra level of support. 

There will be accommodation for district nursing, health visitors, physiotherapy, podiatry and other community services to allow better team working and integration of primary care and community care. 

The vision is to have X-ray and ultrasound facilities, consultant out-reach clinics, and an urgent care unit.

Brackley Hospital Trust has set up a web site to help raise funds for Brackley Hospital Trust, in order to provide facilities that aren’t funded by the NHS. In July 2019 we formed a Primary Care Network with 3 other local GP practices:

Springfield Surgery in Brackley, Brook Health Centre and Towcester Medical Centre in Towcester.

The formation of PCN’s is to enable general practice to work closely with community services to provide integrated care relevant to their local population.

We are all still members of the PML GP Federation called DocMed.

Our Patient Navigators

GPs can find themselves overwhelmed with tasks and requests that may or may not need their individual response or input.

To assist our GPs in prioritising their workloads we have very experienced members of staff who work closely with and under the supervision of the GPs, they have been trained by our GP’s and gained a certificate in Care Navigation. They are called Patient Navigators. They are there to help you with your queries and address your queries efficiently.

You may be contacted by them to gather information for the GP and when you have requested a home visit.

They may often be able to help you without the need to go through the GP therefore providing a quicker response.

Please note the Patient Navigators will not make clinical decisions and will follow surgery protocols supported by the GPs.

Our Reception Team

Our Reception Team have undertaken specific training called Care Navigation to enable them to signpost you towards the most suitable clinician from our Multidisciplinary Team.

In order for them to do this they will need to ask you some questions.

Our team of Medical Secretaries are responsible for ensuring all onward referrals to secondary care or community services are processed as quickly as possible. Any queries regarding your referral should be directed to them in the first instance.

AppointmentsUrgent Care Clinic

This clinic is for registered patients only. How it works:

1. Book in at reception (Brackley Health Centre site). Complete the attendance reason slip and hand it to the receptionist so you can be booked in.

2. A Health Care Assistant may call you in to take your blood pressure or other measurements and take further details about your condition.

3. Based on these findings one of our clinical team will call you through as soon as they are available. This clinic is led by our Advanced Primary Care Practitioners, but overseen by a GP where necessary.

Are these appointments just for medically urgent problems?YES. These are shorter appointments designed to deal with an urgent problem, so you may be signposted to an alternative, more appropriate service. If you have longstanding medical complaints or a more complex problem it would be better to book a routine appointment with your usual doctor. Can I request to see a particular doctor in this clinic?NO. The team will assess who is most appropriate to see you. What happens if I cannot come in the clinic times?We offer pre-bookable evening and weekend appointments in our Extended Access service (please see page 4).

Long Term Conditions

Our Nursing team carry out health checks for certain chronic diseases. If you suffer with a condition such as Diabetes, COPD, Hypertension, Heart Failure, Asthma or have had a Stroke or TIA in the past you will be invited to attend an annual review. These are essential to ensure we are looking after your ongoing needs and to be able to prescribe you the relevant repeat medication.

Home Visits

Home visits are for those patients who are permanently housebound and for those who a GP confirms are too unwell to travel. It is always better to be seen in the surgery whenever possible. All requests for home visits are triaged and we may call you at home for more information.

If you are requesting a home visit please call early, preferably before 10am, with full details of the illness. This will give us time to visit and process any further steps that result from the visit, for instance a hospital admission or prescription.

Home visits use a lot of the doctor’s time so please travel to the surgery whenever possible. If you have requested a home visit and your condition changes, please call us again or dial 999 in an emergency.

Other Clinics & ServicesAntenatalA community midwife is assigned to Brackley Medical Centre and will give both antenatal and postnatal care to expectant mothers. Midwife appointments are held at the Washington House Site on a Tuesday and the Brackley Health Centre site on a Friday. Doctors look after their own maternity patients with the midwife and/or consultant. Women can choose where they give birth. Patients may deliver at home or at any of the various local hospitals. Blood SamplesBlood test appointments are offered every day before our collection time of 2.45pm. We also offer some appointments in our Extended Access service.

Child Immunisations Age 0-4Thursday 9.00am - 12 noon

Family PlanningOur doctors will be happy to discuss individual needs, including the fitting of an IUCD (coil).

Flu ClinicsOur Practice Nurses run flu clinics starting in October each year. In accordance with Department of Health guidelines, we recommend an influenza injection and a pneumococcal injection for all those aged 65 and over. Both injections are also recommended for those with the following medical conditions: asthma, diabetes, heart disease, and lung or kidney disease. The influenza injection needs to be repeated each year, but only one pneumococcal injection is needed for extended cover in most cases.

Health PromotionThe aim of promoting health is to try and identify risk factors at an early stage and prevent them from causing ill health. For this reason we will ask about drinking and smoking habits and measure your blood pressure and weight from time to time. This is especially wise in people with a family history of heart disease, diabetes or stroke. We offer NHS Health Checks for patients aged 40-74 who are not on a chronic disease register. We also offer smoking cessation appointments.

Minor SurgeryWe offer minor surgery appointments with Dr Parsons assisted by a nurse or HCA.

Travel ImmunisationWe are a Yellow Fever Vaccination Centre and are available to offer comprehensive health-related travel and immunisation advice. Useful InformationOrdering Medication

You can order repeat prescriptions securely on-line through SystmOnline or the NHS app. This is the quickest and safest method of ordering. For more details please ask Reception.

You can also return your repeat prescription slip to the surgery.

Text Messaging

You will automatically be opted in to our text messaging service. This will include appointment confirmation and reminders and will allow our team to contact you by text if we have been unable to contact you by telephone. You can also use text messaging to cancel your appointment by texting CANCEL to the number given in your text appointment reminder.

Change of Details

If you change your address or telephone number please notify us as soon as possible. Please note if you move outside of our practice boundary we may write to you requesting you register elsewhere. The locations within our main practice boundary are:

Training Practice

We are proud to be a long-established training practice. As part of our teaching commitment, doctors and Practice Managers from other practices occasionally visit the medical centre to assess our standards and suitability to train GP’s.

Additionally, doctors may occasionally video their consultations to assess and improve their skills. We will always seek your consent before this takes place and you can of course decline. As you would expect, the normal strict rules of confidentiality apply to these processes.

GP Registrar Training

GP Registrars are attached to the practice as part of the North Oxfordshire GP Training Scheme. They are a FULLY QUALIFIED DOCTOR having already completed several years of hospital training. They usually join us for a six month attachment or for one year to complete their specialist GP Training. Dr Stevens, Dr Harrison, Dr Parsons and Dr Beardsworth are GP trainers and are currently responsible for GP registrar training.

Medical Student Teaching

Occasionally we are joined by medical students. They will sit in on surgeries, accompany doctors and nurses on visits and may see some patients on their own briefly before being joined by the supervising professional. You will be informed if a medical student is present and be given the opportunity to say if you do not wish them to be there when you see the doctor or nurse.

Dr Parsons is currently responsible for medical student training in the practice.

Continued Professional Development and Protected Learning Time

We are all committed not only to help train new health care professions, but to continue the development of our own knowledge and skills.

To that end, we regularly have clinical and team meetings for education as well as to cover business and administration matters.

Information Governance

Confidentiality

You have the right to privacy and confidentiality. Our staff do need to access patient information at various levels in order to carry out their duties. They all receive training aimed at ensuring confidentiality is maintained throughout the practice at all times.

In line with this requirement we are unable to disclose any information regarding a patient, unless they are deemed a minor or lacking capacity, to anyone other than the patient themselves, unless written consent is given that this disclosure can be made.

GP Online ServicesSign up to GP online services and you'll be able to use a website or app to:

view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results

view clinical correspondence such as hospital discharge summaries, outpatient appointment letters and referral letters

The service is free and available to everyone who is registered with a GP.

How can I start using GP online services?

Follow these steps to access GP online services:

1. Bring photo ID (passport or driving licence) and proof of address (such as a utility bill) to the GP surgery and tell us you want to start using online services. If you do not have any ID then either a member of staff will have to confirm your identity or you may have to answer questions about personal information in your GP record.

2. Fill in the short registration form that you are given.3. Once you have signed up, you will receive a letter, email or text with your

unique username, password and instructions on how to log in.

Patient Participation GroupWe have a Patient Participation Group (PPG) which represents the interests of registered patients and provides a means of communicating with their health care provider.

All patients over the age of 16 are eligible to join the PPG.

PPG Constitution (Agreed May 2019)

The principal objectives of the group shall be:

1. to improve communication between patients and doctors2. to feed back to the practice any voiced concerns and expectations from any

patient registered with the practice3. to attempt to disseminate among the patient base information communicated

to the practice from the NHS4. to act as a sounding board, representative of the patient base, regarding

suggestions and proposals that will impact on the general public5. to maintain a liaison between the practice, the patients and the local

community as a whole, as required and as appropriate

The group shall meet every three months. Additional meetings shall be called on an ad hoc basis and when required.

One of the GP’s and one of the senior administration staff shall be requested to attend each meeting of the group. They will have no voting rights.

The members of the group shall annually elect a chairman and a secretary. These positions may be held for up to three years.

Any decisions will be made by a simple majority of those present and voting. No binding decisions can be taken unless at least 5 members are present.

Why is it changing?

These common conditions can be treated at home with items that can be bought from pharmacies, supermarkets and other stores.

As you will not need an appointment at your GP surgery to obtain a prescription this will save you time. It will also free up appointments for patients with more serious conditions.

If your condition needs further attention your pharmacist will recognise this and recommend you make an appointment.
