The following Application Form (AF) template is based on the structure of the AF as available in

the e-Monitoring System (eMS). It may allow preparing several sections of the AF off-line. This

document however does not contain the financial part but there is a possibility to extract the related

tables from the e-MS (Excel sheets). For complementary information, the AF guidance is available

in the eMS as well as on our website.
