• Full and specialist registration (and with a license to practice) with the General Medical Council (GMC) in Medical Microbiology and Virology or Microbiology and Infectious Diseases, or be eligible for registration (within six months of interview).

SECTION 3 DUTIES OF THE POST 3.1 MAIN DUTIES OF THE POST To share with colleagues the responsibility for the day-to-day provision of a comprehensive consultation and advisory clinical microbiology service to:

• West Suffolk NHS Foundation Trust • GPs and Clinical Commissioning Groups in the Bury St Edmunds area • Such other purchasers of microbiological services as may contract with the Trust or

with North East Essex and Suffolk Pathology Services (NEESPS) This will include infection prevention and control advice as required and when the designated Infection Prevention Doctor is absent. All consultants will be expected to contribute fully to the various activities of the department including, but not limited to:

• Day-to-day supervision of diagnostic methods and examination of specimens • Authorisation & interpretation of results • Ward rounds and MDTs. The Consultant Microbiologists undertake a daily Critical

Care ward round, and are members of the orthopaedic MDT which meets twice monthly.

• Working with colleagues in the Trust, NEESPS and Public Health England to maintain and develop a quality laboratory and clinical microbiology service for all users, including contributing to the departmental activities necessary to achieve and maintain UKAS accreditation.

• On-call duties are currently 1:3 with aspirations to move to 1:4. On-call duties, including weekends, usually involve telephone cover only. The post-holder will be provided with a Trust laptop to enable remote access into the hospital and pathology computer systems. Out of hours infection prevention advice is provided by the on-call Consultant Microbiologist. The on-call availability supplement is Category A (8%). 1 of the 11PAs is in respect of the unpredictable out of hours commitment.

• To assist with teaching, training and leadership development of all grades of staff in the Laboratory and the Trust.

• Provision of prospective cover for annual and study leave. Leave is arranged by mutual agreement with consultant colleagues, six weeks’ notice of leave usually being required.

• To fulfil one of the lead roles within the department on a rotational basis with consultant colleagues (by mutual agreement): Clinical Lead for Microbiology, Trust Infection Prevention Doctor or Antimicrobial Lead.

Clinical Lead responsibilities include:

• To provide clinical leadership that champions clinical excellence, patient-centred care and a culture of the maintenance and continuous improvement of quality.

• Taking responsibility for the representation of clinical matters within microbiology and providing professional advice to management.

• Close liaison with the Microbiology Laboratory Manager regarding laboratory service provision and development.

• Attendance at relevant Trust and laboratory meetings. • Participation in the development of effective clinical governance and ensuring that

processes are in place which address quality assurance and accreditation, development of services, clinical risk, incidents, patient and user experience and complaints.

• Providing a report for the microbiology Annual Management Review. Infection Prevention Doctor Responsibilities include:

• Promoting an organisational culture of Infection Prevention and Control • Advising on and supporting all aspects of infection prevention in West Suffolk NHS

Foundation Trust • Providing advice and support to the Infection Prevention Team • Developing and reviewing Trust IP policies and advising on their implementation • Involvement in setting quality standards in relation to hospital infections and related

audits. • Dealing effectively with IP issues, including outbreaks, in conjunction with the IP team

and Trust staff • Attendance at relevant Trust meetings • Producing regular reports for the Trust Clinical Safety and Effectiveness Committee,

including the Director of Infection Prevention and Control’s Annual Report. Antimicrobial Lead responsibilities include:

• Chairing the Antimicrobial Management Group and leading on antibiotic stewardship issues within the Trust, supported by the Antimicrobial Pharmacist and Antibiotic Audit Nurses

• Membership of the Trust’s Drugs & Therapeutics Committee • The review and development of antimicrobial guidelines • Microbiology Lead for the Outpatient Parenteral Antibiotic Treatment (OPAT) team.

This includes providing clinical advice and support for the team, attendance at the weekly MDT discussions about patients who are receiving OPAT, and active involvement in developing the service.

In addition, the appointee will be a member of the West Suffolk Hospital Medical Staff Committee. 3.2 Job plan & Timetable A formal job plan will be agreed between the appointee, the Clinical Lead and their Clinical Director (CD) and their Associate Director of Operations (ADO), on behalf of the Executive Medical Director, three months after commencement. This will be signed by the Clinical Lead and the Associate Director of Operations and will be effective either from the commencement date of the appointment or at an alternative agreed date.

The job plan for the first three months will be based on the provisional timetable shown below. The job plan will be reviewed annually with the Clinical Lead and the Clinical Director of Support Services and ADO, and will be linked to Appraisal. The job plan will be a prospective agreement that sets out a consultant's duties, responsibilities and objectives for the coming year. It should cover all aspects of a consultant's professional practice including clinical work, teaching, research, private practice, education and managerial responsibilities. It should provide a clear schedule of commitments, both internal and external. In addition, it should include personal objectives, including details of their link to wider service objectives, and details of the support required by the consultant to fulfil the job plan and the objectives. In the event of disagreement on the allocation of duties, leave, short-term cover etc., the ultimate decision will lie with the Clinical Director and the ADO. For a full-time Consultant a standard week would consist of 8.5 sessions of DCC and 1.5 sessions of SPA. This DCC/SPA split has been agreed by the West Suffolk NHS Foundation Trust LNC. The Trust does pay additional PAs for those consultants performing agreed, specified additional roles. This post also has an additional 1PA given for on-call duties. The following will be incorporated within the job plan:

• Clinical duty sessions (to include attendance at ward rounds). These are rostered in advance, in proportion to the contractual PAs of each consultant.

• On-call and weekend working 1:3 with prospective cover (category A 8% plus an additional 1PA)

• One of the Lead roles as described above • 1.5 PAs for supporting professional activities including CPD

The pattern of working varies from week to week depending on the clinical duty sessions and other commitments. The following timetable is therefore notional.

3. 4 Mentoring The new appointee will be supported and mentored for the first six months of the post – the mentor will be agreed with the Clinical Director. SECTION 4 The Trust delivers services to patients seven days a week and aims to provide equality of treatment and outcome regardless of the day of the week. To meet these aims and changing service requirements, the Trust may need you either to work temporarily outside of

your core hours or to permanently change your core working days and hours. The Trust will give you reasonable notice, where possible, of temporary changes to your days or hours of work and will consult with you and/or your staff representatives about substantial permanent changes, but ultimately, these are changes which may be required of you. THE PATHOLOGY DEPARTMENT AT WEST SUFFOLK NHS FOUNDATION TRUST (WSFT) 4.1 INTRODUCTION The Department of Pathology at West Suffolk NHS Foundation Trust forms part of the Clinical Support Services & Women’s and Children’s Division. Clinical Support Services also includes Radiology, Endoscopy, Pharmacy, Out Patients, Health Records, Clinical Coding and Medical Photography. It is one of four Clinical Divisions within WSFT alongside the Medical Division, Surgical & Anaesthetic Division and Community and Integrated Services Division. Each Division has a Clinical Director, Associate Director of Operations (ADO) and Head of Nursing (and Midwifery in the case of CSS and W&C). The Clinical Director, ADO and Head of Midwifery and Nursing are members of the Trust Executive Group. The Clinical Director for Clinical Support Services is Dr Rachel Darrah (Consultant Radiologist); the ADO is Mrs Rose Smith; and the Head of Midwifery and Nursing is Mrs Karen Newbury. The appointment of the Lead Clinician for Pathology is made by the Clinical Director for Clinical Support Services in conjunction with the ADO for the Division. Pathology and Modernisation The Trust is part of a collaboration with East Suffolk & North Essex NHS Foundation Trust (ESNEFT) called the North East Essex and Suffolk Pathology Services (NEESPS). The partner trusts are both shareholders in, and users of, the service. NEESPS is a clinically led service and input is required from consultant pathologists into the governance and development of the service.

The Microbiology laboratory service is currently subcontracted to Public Health England, which therefore reports into the NEESPS governance structures. This arrangement is coming to an end and delivery of the service will pass to NEESPS, with the laboratory staff undergoing TUPE transfer. Consultant Microbiologists, who are employed by their Trusts, are members of the Consultant and Senior Management Group which meets monthly, usually by teleconference.

Within NEESPS there is a Clinical Speciality Lead for each discipline.

The role of laboratory directors in this arrangement is still under discussion. 4.2 STAFFING

4.3 THE MEDICAL MICROBIOLOGY DEPARTMENT The laboratory is located in the main West Suffolk Hospital building. It is actively working towards UKAS accreditation. Administrative support for the Microbiology Department, Consultants and Infection Prevention Team is provided by a named A&C Band 4 PA, managed by the Pathology Business Administrator. Team-based administrative support across Pathology helps ensure demand is met regardless of holidays, sickness and any other absence. This team approach also helps with appropriate support when most needed, for example during outbreaks. The Pathology Department has a Pathology Service Manager The Microbiology Laboratory is computerised (CliniSys WinPath Enterprise) and provides a comprehensive service covering all aspects of general microbiology to the West Suffolk Hospital NHS Foundation Trust (circa 450 acute beds), Newmarket Hospital and GPs in Bury St Edmunds and the surrounding catchment area. Laboratory workload 2018-19, including referred work

The Laboratory undertakes routine bacteriology, parasitology, serology and antibiotic sensitivity testing.

Mycology and Mycobacterial culture are performed in the Ipswich laboratory (AAFB staining is done on-site). Antimicrobial susceptibility testing uses both the VITEK 2 automated system and EUCAST standardised disc methods.

Gentamicin and Vancomycin assays are performed in the Biochemistry department, other assays are referred to the Bristol antimicrobial reference laboratory. Virology is mainly antigen or antibody detection using either ELISA or ELFA (Vidas and DS2 analysers). Rapid antigen testing is available for Norovirus and RSV. The laboratory offers same day HIV testing. Antenatal serology is performed in-house. Influenza, RSV and C. difficile toxin gene testing is performed on a Cepheid GeneXpert.

Additionally, point of care testing for influenza is being implemented at present. Viral PCR testing is performed at PHE Cambridge. Specialist virology advice is obtained from regional specialists. Chlamydia NAATs testing (BD Probetec) is performed for diagnostic specimens at present but this will be changing in 2020.

In addition, discussions are underway about the introduction of newer technologies, including molecular methods. Urgent overnight microbiology samples are currently transported to PHE Cambridge for testing. Abnormal results are communicated to the West Suffolk Hospital Consultant Microbiologist on call. The laboratory participates in the UKNEQAS schemes for general bacteriology, antimicrobial susceptibility, faecal parasitology, Chlamydia detection and appropriate viral serology. There is also an internal quality assurance scheme.

The Governance arrangements include a microbiology Quality Technical Committee. There is a well-established WSFT Pathology Operations, Quality, Health & Safety group which covers all pathology disciplines.

Quality and performance issues are monitored within the NEESPS governance structures. Office space containing a personal computer (with access to email and the internet) will be provided within the West Suffolk Hospital Microbiology Department. Clerical support will also be provided as outlined above. 4.4 INFECTION PREVENTION TEAM WEST SUFFOLK HOSPITAL The Infection Prevention Team consists of the Infection Prevention Doctor, a lead Infection Prevention Nurse (Band 8A), two Band 6 nursing posts, two Infection Prevention Audit Nurses and clerical support. The Infection Prevention Nurses sit within the Nursing Directorate. The Director of Infection Prevention and Control (DIPC) is Ms Rowan Procter, Executive Chief Nurse. The Infection Prevention Team has formal meetings with the DIPC each month and liaises with the Consultant Microbiologists and the laboratory on a daily basis. It advises the Trust on all aspects of Infection Prevention. The Infection Prevention & Control Committee reports to the Trust’s Safety and Effectiveness Committee and is chaired by the Chief Executive. The Committee includes the Infection Prevention Team, Medical Director, Consultant in Public Health, and representatives of the Facilities Directorate, Occupational Health Department, Pharmacy, Nursing Directorate, Clinical Governance, Clinical Directorates and CCG. It meets quarterly. The Team have good working relationships with the CCG Infection Control personnel and the regional Public Health England Unit. The lead IPN attends the Suffolk Infection Control Network meetings.

The CGCM provides a fast track four-year course for students who hold an existing degree. The course started in 2001 with 21 students being admitted. In September 2018 the course increased to 41 students. The hosting of the CGCM has seen an investment in education on the hospital site, with the Education Centre being built to provide improved educational facilities for the hospital, and attached community services and general practices. The Education Centre includes the provision of library and lecture theatre facilities, the Undergraduate and Postgraduate Medical Centre, and the Practical Clinical Skills Centre.

BURY ST EDMUNDS Bury St Edmunds is an attractive and thriving market town at the heart of East Anglia, a region which is rapidly expanding due to the influx of new technology around Cambridge and good connections with the Continent through the ferry ports to the east. The town is bypassed by the A14 dual carriageway, so it remains relatively quiet in comparison to many other towns. The town has an historic abbey, which was once one of the wealthiest in Europe and at which the Magna Carta was drawn up for presentation to King John in 1215. The abbey ruins are found in the beautiful, much photographed, Abbey Gardens. A very popular and traditional market is held twice a week. The town centre has a wide range of shops, including high street names and speciality shops. There are also numerous restaurants and a wide range of leisure and sporting facilities. Bury St Edmunds now has an exciting extension to the town centre. The £100 million investment has brought more shops, more homes, more jobs and much more to the heart of the town. A cultural festival is held every year in May lasting two weeks, which attracts nationally and internationally known musicians and artists in several fields. The beautifully restored Georgian Theatre Royal is the oldest purpose built theatre in England and features opera, dance and music as well as drama from the best touring companies. The Suffolk coast can be reached in just over an hour at Felixstowe, Southwold, or Aldeburgh. The historic cities of Norwich to the north, and Cambridge to the west are easily accessible. London can be reached within an hour and a half by either train or car. International travel is possible by ferry from Harwich, or by air from Stansted Airport, 45 minutes away, Norwich or Cambridge. Suffolk is a beautiful county which remains one of the safest in which to live and work. Catering “Time Out” Staff Restaurant is open from 8.00 a.m. until 8.00 p.m. for the serving of hot menus and drinks. Outside of these hours there are vending machines available selling meals suitable for microwaving, snacks and drinks. Workplace Nursery There is a 96-place workplace nursery on the West Suffolk Hospital site. The nursery has dedicated rooms for the care of babies, along with adjoining sleeping areas. Separate rooms are available for toddlers and pre-school children. INFORMATION TECHNOLOGY Staff are expected to develop the IT skills necessary to support the tasks included in their post. They will, therefore be required to undertake any necessary training to support this. E- CARE The trust is recognised as being one of the most advanced digital organisations in the country and holds national Global Digital Exemplar status. We have a fully integrated electronic patient record which is built around the Cerner Millennium system. We are ambitious about how and where we can use technology to improve patient care and clinician experience. MAJOR INCIDENTS The post holder is required to conform to the Trust's Policy, Strategy, Plans and Procedures for Business Continuity Incidents, Critical Incidents and Major Incidents (including for Security and Fire incidents), and is to contribute to the planning for such events. This is to apply to all Trust areas of responsibility, including the Community. Furthermore, the post holder is to attend mandatory training sessions and validation exercises as required. FREEDOM TO SPEAK UP / TO IMPROVE It is the pledge of the West Suffolk NHS Foundation Trust to never walk by an opportunity to make things better for staff, patients and relatives. It is the responsibility of all staff to highlight any areas of concern. CONFIDENTIALITY In the course of their duties employees will have access to confidential material about patients and members of staff. On no account must information relating to patients/staff/individuals be accessed by anyone unless there is a legitimate reason, for example, medical staff in relation to direct patient care, investigation of a complaint. STANDARDS OF BUSINESS CONDUCT & CONFLICT OF INTEREST The NHS Code of Conduct and Standards of Business conduct for NHS Staff require all employees to declare all situations where you or a close relative or associate has a controlling interests in a business [such as a private company, public organisation or other NHS or voluntary organisation] or in any activity which may compete for any NHS contracts to supply goods or services to the Trust. FRAUD, BRIBERY AND CORRUPTION The Trust has a zero tolerance stance towards any acts of Fraud, Bribery and all staff should make themselves fully aware and understand the contents of the Trust’s Fraud and Anti Bribery Policies. Any such activities will be subject to disciplinary and/or criminal action by the Trust. STANDING FINANCIAL INSTRUCTIONS All staff must comply with the Trust Standing Financial Instructions when committing the Trust to expenditure, including staff related costs. SUSTAINABILITY In supporting the Trust’s policy on Carbon Reduction it is the responsibility of all staff to minimise the Trust’s environmental impact by recycling wherever possible, switching off lights, computer monitors and equipment when not in use, minimising water usage and reporting faults promptly. NHS FOUNDATION TRUST Employees of West Suffolk NHS Foundation Trust automatically become staff members of the Foundation Trust, unless they choose to opt out.

On leaving the Trust, individuals automatically transfer to public membership, subject to their remaining in the catchment area, unless they request not to do so. POLICIES AND PROCEDURES The duties and responsibilities of the post will be undertaken in accordance with the policies, procedures and practices of the Trust, which the Trust may amend from time to time. COMMUNITY STAFF This also includes the policies and procedures that were in place for the community staff before they transferred into West Suffolk NHS Foundation Trust. REVIEW & MONITORING This policy and procedure will be reviewed and monitored annually by the Executive Director of Workforce and Communication in consultation with Trust Council SECTION 7 - CONSULTANT MAIN CONDITIONS OF SERVICE 7.1 The appointment will be subject to the Terms and Conditions – Consultants (England)

2003 as approved by the Trust Board and as amended by the Board from time to time.

7.2 The appointment is on a whole-time basis. A candidate who is unable for personal

reasons to work full-time will be eligible to be considered for this post. The ability to work part-time only should be indicated when applying and, if such a person is appointed, modification of the job plan will be discussed and agreed between yourself and the clinical director/general manager.

7.3 If the Consultant appointed chooses to contract on a whole-time basis and wishes to

undertake private practice, this private work must be carried out in accordance with the Code of Conduct for Private Practice.

7.4 Consultants are expected to provide cover for colleagues for leave and other authorised

absences from duty upon a mutually agreed basis. 7.5 Arrangements for annual leave: This is arranged by mutual agreement of consultant

colleagues and approval of the Clinical Director and General Manager, in accordance with standard Trust/NHS regulations. It is essential that six weeks’ notice is given to allow for proper planning and prevent cancellations of patients’ appointments/surgery.

7.6 All Consultants have a continuing responsibility for the care of patients in their charge

and for the proper functioning of their department, and are required to undertake the administrative duties associated with that responsibility.

7.7 The successful candidate will be required to reside within ten miles or 30 minutes

travelling time of Bury St Edmunds. 7.8 The Trust requires the appointee to have and maintain full registration with the General

Medical Council and hold a Licence to Practise. Medical and Dental Staff are advised to continue to be members of one of the medical defence organisations.

7.9 The present salary scale is £79,860 - £107,668 per annum, plus an on call availability

supplement if appropriate. The successful candidate will commence on the minimum of the scale unless advised differently during the appointment process.

7.10 This post is exempt from the provisions of Section 4 (2) of the Rehabilitation of

Offenders Act 1974 by virtue of the Rehabilitation of Offenders Act 1974 (Exemptions) Order 1975. Applications are, therefore, not entitled to withhold information about convictions which, for other purposes, are “spent” under the provisions of the Act and, in the event of employment, any failure to disclose such convictions could result in dismissal or disciplinary action by the West Suffolk Hospitals NHS Foundation Trust. Any information given will be completely confidential and will be considered only in relation to an application for positions to which the Order applies.

7.11 Health Questionnaire – Prior to taking up appointment, the successful candidate will be

required to pass a Health Assessment from the Occupational Health Department and provide evidence of Hepatitis B status from a United Kingdom Trust or Public Health Service Laboratory.
