GENERAL INFORMATION

HISTORYNew Dimensions Beauty Academy was founded in 2006 in Parker, Colorado and moved to its present location in 2009. The Academy began as an advanced esthetics and permanent makeup school. In 2006, Cosmetology, Hairstyling, Barbering, Nail Technician, Esthetics and advanced classes were added to our list of programs offered. New Dimensions Beauty Academy is recognized as NDBA throughout this catalog.

OWNERS: Wendy Peters, President and Education Director: B.S. Behavioral Science, Colorado State Credentialed Teaching License, former

primary education teacher, Credentialed Instructor, licensed Esthetician, licensed Cosmetologist, licensed Barber, certified Permanent Makeup Technician, certified Microblading Technician and holds several advanced certifications in hair, skin and nails.

FACILITIES AND EQUIPMENTNDBA is a private cosmetology institution. There is approximately 8200 square feet in a three-level building in a small shopping center. The school layout consists of a reception area, large clinic floor, administrative offices, three classrooms, a practical area for cosmetology, manicure and pedicure area, five restrooms, four facial rooms with beds, a microdermabrasion room, waxing/eyelash extensions/permanent makeup rooms and a dispensary, shampoo areas and laundry facilities. Reference libraries are located in each classroom and contain books, videos and DVDs. The space also includes 2 study areas and lockers. The facilities are designed to meet the criteria for a modern, progressive and advanced cosmetology school.

ESSENTIAL CAREER CONSIDERATIONS / REQUIRMENTSApplicants interested in pursuing a career in cosmetology or a related field should consider all aspects of such a decision. Person who want to become a professional in the beauty industry must:

1) Have finger dexterity and a sense of form and artistry. 2) Enjoy dealing with the public and be able to follow client directions.3) Keep abreast of the latest fashions and beauty techniques. 4) Work long hours while building a personal clientele in order to make the desired income.5) Make a strong commitment to the educational process and finish school.6) Learn business skills applicable to the desired position.

In addition, prospective students should be aware that: 1) The work of a licensed professional in the cosmetology/barber/beauty industry can be arduous and physically demanding because of long hours

standing with hands at shoulder level, bending to work on all areas of a client’s head, bending to complete shampooing or other wet services, etc. Estheticians will spend a significant amount of time standing to complete facial services and body treatments, waxing, or makeup services. Nail technicians will spend long hours sitting at a nail technician’s station. Instructors will spend long hours standing while teaching and walking to monitor their classrooms / clinic areas.

2) A personal investment may be required for advertising and promotions such as printing business cards or cooperative advertising. 3) There will be exposure to various chemicals and fumes which may cause allergic reactions. 4) The practice of safety and sanitation is essential for effective and successful performance within the industry. 5) Methods of compensation vary and may include straight salary, salary plus commission, sliding scale commission, retail commission or

independent contracting (renting space and equipment from an existing salon).

ADMISSION POLICY AND PROCEDURES

PROGRAMS OFFEREDThe academy offers the following programs:

Cosmetology – 50 credits/1500 hours Barbering – 50 credits/1500 hours Hairstyling – 50 credits/1200 hours Esthetics – 21.3 credits/638 hours Nail Technician – 20 credits/600 hours

Cosmetology school credit hours are computed as follows: 30 theory/lecture contact hours = 1 semester credit

ADMISSIONS REQUIREMENTS1) Interview with an Admissions Representative.2) Complete an Application for Acceptance/Non-Refundable Fee. 3) Complete and Enrollment Packet/Non-Refundable Registration Fee.4) Affidavit of Eligibility (not required for advanced classes). 5) Proof of Age (i.e. Driver’s License, State I.D., Birth Certificate or Passport). 6) Copy of High School Diploma, GED Certificate, copy of transcript showing high school completion, or documentation for secondary completion if

home-schooled. Self-pay students may provide a post-secondary degree from an accredited school in place of a High School Diploma. Advanced class students will be required to provide a copy of their license instead. Foreign-trained applicants must include a translated/evaluated copy of a foreign diploma and education. Please see Policy of Evaluating the Validity of High School Diplomas.

7) Social Security card (Not required for advanced classes). 8) Must be at least 16 years of age.9) If enrolled under a Training agreement with the School District, Government Agency or other entity, this will meet the admissions requirement

of all parties involved. 10) Secondary students not enrolled under a Training agreement may be admitted, if the applicant meets the state requirements for admission,

obtain permission in writing from their secondary school, is in good standing with their secondary school and completes a pre-evaluation. Enrollments under this requirement will not exceed 10% of the annual student body.

11) Parental/guardianship signature for any required parental consent. 12) A high-school aged student is eligible to sign up for school prior to graduation, however, they must provide documentation of high school

diploma or transcripts prior to starting classes. 13) Strong command of the English language, including reading, writing and speaking. All programs are taught in English. 14) Students who do not hold a high school diploma, GED, or post-secondary degree must follow the Ability To Benefit (ATB) Policy. After June 30,

2012, ATB tests will not qualify a student for financial aid. Start dates are provided in the Addenda section of this catalog. Enrollments are accepted year round. Financial Assistance is available for those who qualify. The school does not guarantee the transferability of its clock hours to any other institution unless there is a written agreement with another institution.

TRANSFER POLICYStudents transferring to NDBA with existing hours must fulfill the following admissions requirements:

1) Fulfill NDBA Admissions Requirements.2) Upon receipt and evaluation of previous training, up to 100% of a program may transfer to NDBA. The percentage of transfer hours/credits

accepted is at the discretion of NDBA. For a student transferring hours/credits, administration will determine the level of competency by evaluating a written and practical transfer evaluation test. A Transfer Evaluation Test fee will be assessed. For a student transferring a license or a student transferring within NDBA, a transfer test may or may not be required. Acceptance of sanitation hours/credits may not require an evaluation test.

3) All the equipment, books and supplies necessary to participate in the program required. 4) Tuition will be adjusted by a credit hour/clock hour rate equivalent to the program. 5) This policy does not financially or in any way affect the refund policy.

NDBA does not recruit students attending or admitted to another school offering a similar program of study.

Students transferring from NDBA with existing hour must fulfill the following requirements: 1) All transfers must be requested in writing to the Education Director. 2) The school does not guarantee the transferability of its credits/clock to any other institution unless there is a written agreement with another

institution. 3) Unpaid tuition, kit or an open balance must be paid in full.

DEPARTMENT OF VETERAN AFFAIRSNDBA maintains a written record of the previous education and training of the veteran or eligible person and clearly indicates that appropriate credit has been given. The training period will be shortened proportionately, and the veteran or eligible person and the Department of Veterans Affairs will be notified.

ABILITY-TO-BENEFIT POLICY1) Definition of an Ability-to-Benefit Student: A student who is beyond the age of compulsory education, lacks a high school diploma or its

equivalent, and has the ability to benefit from the education or training offered at an institution. 2) Admissions of Ability-to-Benefit Students: In order to be admitted on the basis of his or her ability to benefit, a student shall, prior to admission,

complete a nationally recognized standardized, or industry developed test which meets the NACCAS guidelines. The passing score will be kept on record in the student’s file.

This test is administered by appointment at the following locations; students will need to present their photo ID prior to the test and there is a $50 charge. NDBA will recognize all students whose ATB scores / Accuplacer test results pass all 3 categories with minimum scores in Reading Comprehension (55), Sentence Skills (60) and Arithmetic (34).

POLICY ON EVALUATING THE VALIDITY OF HIGH SCHOOL DIPLOMASRegulation 34 CFR 668.16 (p) requires Title IV schools to establish policies and procedures to confirm the authenticity of high school diplomas in the event that the school or the Secretary of the U.S. Department of Education has reason to question the validity of a student’s high school diploma. NDBA may require further documentation in the form of a certified copy of final high school transcripts for the high school in question or information from a company that evaluates foreign diplomas (in the case of a foreign diploma). Foreign diplomas must be translated and evaluated by a recognized agency such as NACES (National Association of Credential Evaluations Services) or WES (World Education Services). Student self-certification is not considered sufficient proof of validity. In addition to checking online for further information about the school issuing the diploma and its accreditation, NDBA may also contact the Department of Education in the state where the diploma was issued to determine if the school in recognized on the state list. The school maintains a list of known diploma mills as a reference when admissions receives a diploma from and unknown and questionable source. It is understood that list of schools in the diploma mills may not be all-inclusive, and that the list of schools in the FAFSA drop-down menu online may also not be all-inclusive. NDBA makes every reasonable effort to verify the validity of questionable high school diplomas. Giving false information for the purpose of admission or access to financial assistance is a violation of law and could result in criminal prosecution, as well as denial of admission or termination from the academy.

RE-ENTRY / RE-ENROLLMENT POLICYStudents who have been terminated or withdrew from school may re-enroll (if determined eligible) within 180 days of their last day of attendance and will not incur additional charges. However, these students will be responsible to pay any remaining balance from the previous enrollment that cannot be covered with reinstated federal funds.

A determination of Satisfactory Progress will be made and documented at the time of withdrawal. That determination of status will apply to students at the time they return to school. The student may appeal a negative Satisfactory Progress determination according to the appeal policy. Elapsed time will be treated like a Leave of Absence and does not affect Satisfactory Progress and will extend the contract period by the same number of days. Students re-entering after exiting the school will not be evaluated as new students and consideration will be given to the student’s progress status at the time of previous withdrawal.

Students who have been terminated or withdrew from school and re-enroll (if determined eligible) after more than 180 days from their last day of attendance will pay a $100 registration fee and will be charged for contracted hours at the current tuition rate. All re-enrolling students will be provided the school’s re-enrollment policy and will be evaluated by the school Director for placement in the curriculum and kit needs. The student may be required to repeat credit/hours that were previously earned. Re-enrolling students may be required to purchase the current school kit. Students applying for re-entry or transfer-in from other schools may be required, as a condition of enrollment, to ring delinquent prior student loans to a current status.

All requests for re-enrollments will be evaluated by the school Director. Students are asked to submit a letter explaining why they withdrew or were terminated from school and how the issues have been resolved. The student’s record/file from their previous enrollment will be reviewed to determine whether re-enrollment will be granted. Re-enrollment is at the direction of the school administration.

Students who re-enroll after more than 180 days into their original program will have their Colorado refund calculations revised to be charged specifically for the credits/hours completed. If any payments were made that are in excess to the revised calculation, that amount will be applied to the new contract. If a student switches from one program to another program, the student will be required to pay the original refund calculations in full. Payment plans may be available. Students who switch from one program to another or choose to enroll in a second program will be treated as transfer students. Transfer Hours accepted by the school are counted as both attempted and completed hours for max time and SAP (policy can be found under SAP Policy).

GRADUATION REQUIREMENTSFor Advanced Stand Alone classes, please see individual pages.The graduation requirements for any of NDBA’s program courses area as follows:

1) Complete the required credits and hours determined by the State of Colorado and NDBA, per student contract. 2) Maintain an academic 75% cumulative grade point average and a cumulative attendance of 80%. 100% attendance is still required for

graduations, therefore make-up hours are available. Students may not graduate before their calculated graduation date, unless the graduation date falls on a non-scheduled day.

3) Take a final examination on all practical and theory with a score of no less than 80%.4) All workbooks and tests must be completed with a passing grade.5) Students will not be eligible to register for the state exam until all requirements have been met.

A CERTIFICATION OF COMPLETION WILL BE AWAREDED UPON GRADUATION FOR ALL PROGRAM COURSE AND STAND ALONE CLASSES. Advanced Stand Alone Certificates will be held until complete licensure is achieved.

STUDENT SCHEDULE CHANGESStudent’s permanent schedules will be determined and approved on the enrollment contract. Schedule changes are not permitted during basic core training. A permanent schedule change that requires your weekly hours to change on your enrollment contract requires an addendum to the enrollment contract and a schedule change fee.

CANCELLATION / COURSE CHANGESWith prior approval from the Directors, a student my change from one course to another course per the following criteria:

1) The student must pay a cancellation/course change fee/refund calculation.2) The student will only receive applicable credits/hours to another course with the required academic and attendance records in accordance with

the State of Colorado.

NON-DISCRIMINTATION STATEMENTThe school in its admission, instruction and graduation policies does not discriminate on basis of age, sex, race, ethnic origin, color, gender, religion, citizenship, financial status, or on the basis of handicap as required by Section 504, 34 Code of Federal Regulations. If accepted, NDBA will do its best to accommodate all students based on their needs, however NDBA reserves the right to discontinue their program/education if the school is not able to do so. The academy facility is handicapped accessible.

STUDENT SUPPORT SERVICES

ORIENTATIONNDBA requires all students to attend an orientation which provides information about the instructional programs, goals of each course, policies and services available to students.

STUDENT ADVISING SERVICESStudents have access to advisement from members of the school Staff regarding their program of study, progress, financial affairs, placement or other areas in which the student may need assistance. This includes referring students to professional assistance when necessary. Students whose progress in the course is unsatisfactory are counseled and provided additional assistance, if available.

OTHER AVAILABLE SERVICESNDBA is centrally located in Parker, Colorado.

1) NDBA may participate in scholarship through discounts, donations, or coupons to local area School District fundraisers, or other promotional events such as Parker Days, Elizabeth Stampede, Career Days or Expos at city, county and state levels.

2) The Academy will post the scholarships, discounts, donations and location on the website prior to any events.

ADDITIONAL PROGRAMSStudents enrolling in multiple programs may be eligible to transfer credits/clock hours from one program to the other.

STUDENT COMPLAINT POLICY AND PROCEDUREIn accordance with the institution’s objectives, NDBA will make every effort to resolve any student complaint that is not frivolous or without merit. Complaint procedures will be included in new student orientation, thereby assuring that all students know the steps to follow should they desire to register a complaint at any time. If a third party is paying for a student’s education, it the student’s responsibility to provide this information about complaints to the third party.

1) The student should first try to resolve the grievance with the Staff member nearest the source of the issue. 2) If the grievance cannot be resolved, it may be taken to a Director. The complaint must be in writing on a Student Complaint Report Form

available on the wall right outside of the Admissions Office. 3) The complaint will be reviewed by school administrations within 15 business days, and will then be discussed with the student to work towards a

resolution. Depending on the extent and nature of the complaint, interviews with appropriate staff and other students may be necessary to reach a final resolution. This will also be documented in writing and kept in the complaint file.

4) If the case merits further investigation, the student will receive a response after the investigation has been completed. 5) Should the student find the response to be unsatisfactory, the student may appeal the decision to the owner/administration. Appeals must be in

writing and include copies of the original grievance, the original response, and any additional pertinent documentation. 6) The owner/administration will review and further investigate the matter, and will subsequently issue a final written response. Every attempt will

be made to issue the final response within 60 days of receipt. 7) The Colorado Division of Private Occupational Schools has a two-year statute of limitation (from the student’s last day of attendance) on

students filing a complaint. Complaints must be filed in writing and online at http://www.highered.colorado.gov/dpos. Phone number: 303-862-3001. They can also be made at the National Accrediting Commission of Career Arts and Sciences at http://www.naccas.org.

LICENSING REQUIREMENTSThe Office of Barber and Cosmetology monitors and dictates admission to the licensing examinations through a designated testing agency: PSI Services, LLC. An applicant must be a graduate of a state approved school in Barbering (50 credits/1500 clock hours), Cosmetology (50 credits/1500 clock hours), Hairstyling (40 credits/1200 clock hours), Esthetics (21.3 credits/638 clock hours) or Nail Technician (20 credits/600 clock hours). The applicant must have completed the required credits/clock hours for the prescribed course, submitted certified proof of graduations, made the application and paid the required examination fee for the prescribed examination. The Office of Barber and Cosmetologist Licensing/designated testing agency conducts all examinations in English and offers testing in some other languages with supporting documents (check with PSI online for additional information). All education and learning environments in our school are conducted in English, including classroom activity and clinic floor education. The applicant must have a valid social security number prior to licensure examination, and have completed an affidavit of eligibility form. Students are responsible for paying for licensure exam fees. CONTINUING EDUCATION CLASSESNDBA offers advanced classes throughout the year. All advanced and Stand Alone Courses are listed under Programs of Study. Class Fees are listed in the Addenda.

OFF-CAMPUS CLASSES / BEAUTY INDUSTRY SHOWSInstruction hours may be modified by the school to accommodate special events or training (i.e. field trips, outside classes, career fairs, fashion shows, and community involvement). Students will be notified in advance of upcoming events. All outside classes must be pre-approved.

CAREER SERVICESWhile we will assist in your job search, we make no guarantee, expressed or implied, of future employment. Preparation for an effective interview, client building skills, retail sales, etiquette and professionalism are taught throughout the program.

1) Employment opportunities are posted and on the Career Board for all students and post-graduates. 2) NDBA participates in events, allowing students to network with licensed professionals. 3) Graduates are contacted after graduation to follow-up on the job placement. Post-graduates are encouraged to keep NDBA posted of any

changes in their employment, allowing further assistance if they so desire.

STUDENT SERVICES, HOUSING AND COUNSELINGThe school conducts an orientation program prior the first day of class which provides information about the instructional programs, goals of each course, policies affecting students and services available to students. Students have access to advisement from members of the school staff regarding their program of study, progress, financial affairs, housing, placement or other areas in which the student may need guidance. Students who have unsatisfactory in the course are advised and provided any additional assistance available. When requested, the school may provide assistance and advisement in acquiring adequate housing facilities. A student requiring housing assistance should contact the school prior to enrollment. Housing is not provided by the school.

MEDICAL CONDITIONSAny students enrolling or attending school with pre-existing medical conditions or the need for prescribed medication must disclose this in the NDBA student orientation packet. Staff will be notified as needed, and all information will be kept strictly confidential.

PERSONAL CALLS AND CELLULAR PHONESStudents may not receive incoming calls on the School’s business line other than an emergency. Every effort will be made to locate the student if an emergency call is received. Cellular phones may be used in the break room and outside the building only. With prior approval, cell phones can be used for tutorial educational purposes, or music during approved times. This rule applies to both verbal and text communication. Students must be on break or clocked out while taking personal calls. All other areas are off limits as they are distracting to classmates and clients receiving services.

VOTER REGISTRATIONNDBA encourages its students to be registered voters and to exercise their right to vote. Students can register to vote in Colorado at www.registertovote.org and at www.longdistancevoter.org.

VACCINATIONSWhile NDBA encourages its students to lead happy, healthy lifestyles and stay disease-free, it does not have a specific vaccination policy.

SMOKINGWe are a non-smoking facility. Smoking is only allowed in designated areas 100 feet from any building entrance. Cigarette butts will be properly disposed of or disciplinary action will be taken. This applies to all the businesses adjacent or near the Academy. Vaping is not allowed in the building.

DRUG AND ALCOHOL POLICYNDBA actively supports the prevention of drug and alcohol abuse. Upon enrollment, students and employees are provided the school’s policy on drug and alcohol abuse. A list of agencies and counselors are available to anyone requesting assistance as well as drug and alcohol prevention programs. Use of, possession of, or distributing/selling drugs is not allowed on the school premises and is illegal under both state and federal laws. It is unlawful to sell or provide alcohol to a person under the age of 21; it is illegal for a person under 21 to possess alcohol. Such laws are strictly enforced by NDBA. Violators are subject to the institution’s disciplinary action and may be subject to criminal prosecution, fine and imprisonment. Students or employees whose behavior is negatively influenced by the use on nonprescription and prescription drugs may be asked to leave the premises. Our expanded drug and alcohol policy and Annual Security Report is kept in our Safety and Security Pre-Enrollment Binder. Our annual security report is updated and emailed annually to all students and staff.

SAFETY:NDBA asks its students to maintain a safe environment by:

MEDICAL EMERGENCIES: First Aid Kit is located in the DispensaryNotify an instructor or Staff member immediately if any medical emergency arises. Instructors or Staff Member will:

OSHA REQUIREMENTSIn compliance with the United States Department of Labor Occupational Safety and Health Administration requirements, NDBA advises its students of the chemicals used in cosmetology and related training. During the course work, the student learns about the importance of safety in the work place and how to use and follow the Safety Data Sheets (SDS) for chemicals used in cosmetology or related training. During each unit of study, students are apprised of the various chemicals used and safe practices that apply. A complete file containing Safety Data Sheets for the chemicals used is available in the administrative office. The school endeavors to facilitate a safe environment for staff and students by teaching the proper and safe use of equipment, tools and products. The school does not assume responsibility for injuries resulting from improper or unsafe use of equipment, tools or products.

PRIVACY AND RECORD ACCESS POLICYThe Family Educational Rights Privacy Act of 1974 prohibits the release of privileged and confidential information concerning individual students unless authorization is received by the student and parents/guardian of dependent children. It is the school’s policy that no information is released without written authorization by the student. Forms are available at the Director’s office. To review a student file, a written request must be made with a Director. A student may review or give permission to an outside individual to review their student records. These records may include academic, financial and counseling records. A student may not review income and asset information on parents/guardians in the file without written consent of the parents. The school will maintain complete student records for a minimum of six years from the date the student graduated or discontinued training, in accordance with Colorado Private Educational Act of 1981 Rules and Regulations. Rules and Regulations were revised June 14, 2018. The school will release information to any federal, state or local official, auditor or accrediting body having the authority to request such information.

SAFEGUARDING STUDENT INFORMATIONNDBA is committed to implementing and maintaining a comprehensive information security program, to maintain and safeguard your non-public personal information against damage or loss. The policy covers all student records in whatever form (hard copy or electronic). The school Director/Administrator shall be responsible to coordinate the school’s information security program. The Director shall, at least once every 3 years, assess foreseeable internal and external risks to the security, confidentiality and integrity of student information that could result in the unauthorized disclosure, misuse, alteration, destruction or other compromise of the information. The risk assessment shall cover every relevant area of school operations, including employee training and management, network and software design, information processing, storage, transmission and disposal, and ways to detect, prevent and respond to attacks intrusions or other system failures. The Director shall design and implement safeguards to control identified risks and shall monitor the effectiveness of them, recommending changes whenever warranted. Records for prospective students who are not accepted or who do not enroll in the school will be held at the latest 36 months and then destroyed in a secured manner. Records of enrolled students shall be maintained in accordance with federal and state law and accreditation requirements. Students shall receive notice of this policy at the time they enroll. The school shall only enter into servicing agreements with service providers who also maintain appropriate safeguards for customers’ non-public personal information.

ESCAPE ROUTESEach floor of the building has floor plans posted showing a direct escape route. Please familiarize yourself with the escape routes in all areas.

TORNADO WARNINGSWhenever there is danger of a tornado touchdown, the tornado warning sirens should sound. Based on weather service information, the Staff will make the decision when to evacuate everyone to the basement.

FIRENDBA is equipped with a monitored alarm and sprinkler system. Whenever a fire is detected, a continuous siren will sound and water will spray. In cases of a fire alarm, students and Staff must:

1) Instruct all guests to evacuate the building2) Assist all guests3) Evacuate the building immediately

Fire Department or Security will notify when it is safe to return to the building. Fire drills will be conducted to prepare for an emergency. Everyone must follow normal procedures in evacuating the building. Only Staff personnel will give clearance to re-enter the building upon completion.

INCIDENT / ACCIDENT REPORTSFor all minor or serious accidents, gather the following information on an incident report form and give to the school’s administration:

1) Name, address and phone number of the injured person2) Name of student/instructor working on the client (if applicable)3) Date and Time of Incident/Accident4) Description of how the Incident/Accident happened5) Document all witnesses to the Incident/Accident with pertinent information

PRODUCTSProducts used to perform services on our clients and for practical experience are selected by the school. Products are available for students in the dispensary for clinic floor use only. Products not approved by the school are not permitted to be used on campus without administration approval.

VISITORSPersons visiting a student are required to enter the school through the front door and check in at the front desk. Students must not receive visitors unless they are clients. Clients and friends are not allowed in the break room, classroom or any of the offices. These rooms are for the students and Staff ONLY. The clinic floor areas are for servicing the clients. If friends or family come to meet you at school for lunch or at the end of a shift please let them know they will need to wait in the reception area. Administration reserves the right to authorize exceptions to this policy.

PARKINGStudents will park in the areas designated for student/public parking, which is the lot northeast to the back door. If the back lot is full you may have to park on the street north of the school. The school does not provide security for cars and is not responsible for student vehicles or its contents.

CAMPUS CRIME STATISTICS AND SECURITYNDBA makes every effort to provide a safe and secure learning environment for students. A handout detailing crime statistics on our campus is provided at the time of enrollment and to every enrolled student by October 1 st of each year. The report is also make available upon request from the school Director at any time and is kept in our Safety and Security Pre-Enrollment Binder.

LOST AND FOUNDStudents should report lost items and turn in any found items to their instructor. The school is not responsible for lost, damaged or stolen items. Any unclaimed items turned into the school will be kept for 10 days. After that time the items will be considered abandoned. Students are advised to take all belongings with them at the end of each day or put them into a locker or their roll—about.

EQUIPMENT AND SUPPLIES1) Students will need to send in their warranty cards for all equipment disbursed in their kit. Once a student signs for their Kit equipment and

supplies, it becomes solely their responsibility (i.e. warranties included). There will be no redistribution of kit items. Any items not disbursed due to backorder will be disbursed as soon as delivered. Most items can be replaced through the school if needed, at replacement cost. All implements, equipment, towels and stations should be appropriately sanitized before use on any client as required by the state regulatory agency.

2) Personal items must be kept in your assigned locker or not brought into the building. 3) Kit items left out will be placed in Lost and Found.

4) Students are required to have all of their equipment and supplies available and in working order each day. Supplies needed for State Board Testing and Baby Boards are the responsibility of each student. Certain items carried by NDBA can be purchased and some of the Baby Board kit items are available for students to rent.

5) If any personal/kit items are left at the school and no contact has been made to retrieve their items or clear their school balance, they will be removed after 30 days and we will not be held responsible.

6) If you lose, break, ruin or use up your kit supplies YOU must replace them within 10 days. If you do not, NDBA may replace them for you and you will be charged the replacement cost of those items.

STUDENT SANITATION DUTIES1) Each student is assigned a daily sanitation duty to be performed. Sanitation hours are required and set by the Colorado State Board of Barber

and Cosmetology. 2) Failure to perform the assigned duty may result in disciplinary action.3) In addition to daily sanitation duties, it is the student’s responsibility to clean and sanitize their own work station daily.4) Students are to keep all equipment and tools in proper safety and sanitary conditions.5) Students are expected to store all their belongings each day upon departure.6) Proper sanitation procedures are to be used in all areas at all times in accordance with each specific area. 7) Pedicure stations are to be cleaned, disinfected and documented in the sanitation log.

POLICY ON FEES CHARGEDFee charges are listed in the Addenda.

1) Application fee is charged time of the Application; Course Registration is charged at the time of Enrollment; Online fees for barbers and nail technicians is charged at the time of enrollment.

2) Cancellation/Withdraw fee is charged at the time of withdrawal. 3) Re-enrollment or Re-Entry fee is charged at the time of re-entry.4) There is a schedule change fee for permanent schedule changes.5) There is a fee for credit checks for TFC loan applications.6) There is a Friday/Saturday fee if you are not present on a scheduled attendance date.7) Transfer Evaluation Test fee for transfer students.8) Stand Alone Class Registration fee is charged at time of enrollment.9) Fee for replacement name tag/time card.10) Additional copies of Certificates of Completion or request for Transcript for former students will incur a fee. 11) There is a fee for document or paperwork copies. A reasonable amount of time for the request is required.12) Staff or students are not permitted to write checks or use the credit card machine for cash. Any returned checks written to the school for tuition

or services will be charged a fee if check is returned to the school for insufficient funds or closed account.13) Any collection charges or legal fees incurred as a result on returned checks or monies will be the responsibility of the student. 14) Missed days on advanced courses such as permanent makeup, piercing, tattooing, eyelash extensions, etc. or one-on-one training for any class

will incur fees based on an hourly rate.15) In the event the student fails to complete their course by the On-Time Completion date, the student may be permitted to continue their

program if they adhere to the rules and regulations of the Academy. Extra Instructional Charges incur for any student failing to complete their course by the On-Time Completion date on their enrollment contract.

16) Students having to repeat a segment of Basics due to not passing either the week or Basics test-out will be required to retake and will be charged the current credit rate.

17) Kit Rentals (optional): Nail kits ($15), Esthetics kits ($20), Cosmetology/Hairstyling/Barbering ($30). Outside students will require a $50 rental fee and a deposit for the kit in the amount of the kit cost. If students fails to return kit, or if the kit has missing items, the student will be charged the full price.

18) If a student’s Milady Coursemate online account expires, they will incur an additional fee through Milady Cengage.

FINANCIAL PRACTICES AND MANAGEMENT

FINANCIAL ASSISTANCEFinancial assistance may be available for those who qualify from Federal Direct Student Loans or Grants, private banking institutions, credit unions, or student loan programs available through the school. More information is available from the Financial Aid Director. The school complies with all State and/or Federal regulations in any advisement to students. For more information concerning Title IV financial aid programs, you may contact the Financial Director at 303-840-6424.

Please Note: Paying the minimum monthly payment may not pay your entire tuition balance before completion of the program. Your tuition balance must be paid in full within 5 days of program completion, unless arrangements have been made with the financial director. Your Record of Completion/Certificates/Transcripts may be held until these obligations are met.

All students are eligible to make monthly interest-free cash payments. If a student chooses this method, payments are determined by: Total Amount Due / Total Number of Months the student will be in school. All monthly payments are due on their due date. They are subject to a $10 late fee on the 5 th day after the due date. All payments over 30 days late may be sent to collections, at which time the student or guardian will be responsible for legal fees, collections fees and all monies owed to the school. All payments at that time may be subject to 18% interest on all monies due. A student may pay in full only under special circumstances and approval.

Other State and Federal ProgramsFinancial assistance may be available through such government programs as the Bureau of Indian Affairs (for documented Native American Indians), Colorado Vocational Rehabilitation (for the physically or mentally impaired), Work Investment Act (operated through the Douglas County Job Service), the Air Force Aid Society or Army Educational Assistance Program (for children of active duty military personnel), and Colorado Workforce. Grants and scholarships may also be available through the State and other private institutions. More information can be found at http://www.collegegrant.net/colorado/.

FEDERAL ASSISTANCE PROGRAMSNDBA is approved as an eligible institution by the U.S. Department of Education to participate in Title IV grant and loan programs. The packaging of financial assistance is determined according to guidelines set by the U.S. Department of Education. A variety of programs are available for students qualifying for assistance:

Federal Grants: Federal Pell Grant: Intended to be the basis of the financial aid package and may be combined with other aid to meet the full cost of attendance. The Federal Pell Grant is a need-based aid program in which an eligible recipient does not have to repay the funds received.

Federal Direct Loan Program: These are low-interest loans for undergraduate and graduate students that are made available through the Federal Government. This program includes the Federal Direct Subsidized, Federal Direct Unsubsidized and Federal Direct Plus loans. There are grade level progressions and loan limits used for administration of these loans.

The following Direct Loans are made through the William D. Ford Direct Loan (Direct Loan) Program, which is administered by the U.S. Department of Education:

Federal Direct Subsidized Loan: This is a need-based loan for which the Federal government subsidizes the interest until repayment begins and during any period of deferment. This is a loan and recipients must begin making payments at the end of their six-month grace period.

Federal Direct Unsubsidized Loan: This is a non-need-based loan for which the Federal Government does not pay the interest subsidy. Interest accrues during all periods. The recipient has the option to pay the interest or to defer payment of the interest for the grace period, known as capitalization. This is a loan and recipients must begin making payments at the end of their six-month grace period.

Federal Direct Plus Loan: This loan is available to parents of dependent undergraduate students to help pay for the cost of the dependent’s education. Borrowers of PLUS Loans are required to undergo a credit check by the lending institution. The definition of a “parent” for PLUS Loan eligibility is a student’s biological, adoptive or step-parent in the event that person’s income would have been taken into consideration when calculating the student’s expected family contribution (EFC). Interest is charged during all periods.

Entrance and Exit Counseling: Students receiving Federal Direct Loans must complete Student Loan Entrance Counseling prior to disbursement of funds and Student Loan Exit Counseling prior to receiving graduate exit paperwork. Students withdrawing from school will be notified to complete Student Loan Exit Counseling within 30 days.

Title IV Credit Balances:Tuition and fees are charged by payment period. Federal student aid is received by payment period. Generally there are two payment periods in a full academic year. When Title IV funds received for the payment period are greater than the charges for the payment period, a credit balance is created on the student’s account. This credit balance will be refunded to the student (or parent in the case of a Parent PLUS loan) in the form of a check within 14 calendar days of the credit balance being created, unless an Authorization to Hold Credit Balance Form has been completed by the student.

VERIFICATION:Students selected by the U.S. Department of Education for the process of verification are frequently required to submit additional information and/or parents’ financial and household information to the Finance office. The verification procedures will be conducted as follows:

1) When selected by the U.S. Department of Education for the process of verification, the student must submit all required documentation to the finance office within 14 days from the date the student is notified that the additional documentation is needed for this process.

2) If the student does not provide all of the required documentation is received and the student’s eligibility for federal student aid has been established.

3) The Finance office reserves the right to make exceptions to the policy started above on a case by case basis for extenuating circumstances.4) U.S. Department of Education regulations [34 CRF 668.16 (g)] require schools to defer the Department’s office of the Inspector General any

credible information indicating that an applicant for Title IV aid may have engaged in fraud or other criminal misconduct in connection with his or her application. The school will report any suspected fraud to OIG at 1-800-MISUSED.

The Finance office will notify the student of changes to their financial aid award resulting from corrections made due to the verification process. An adjustment will be made to the student’s financial aid award as required by federal regulations and an addendum to the existing award letter or a new award letter will be issued.

COST OF ATTENDANCE BUDGETS (COA)In order to determine a student’s level of loan funding, the Department of Education requires us to develop annual cost of attendance budgets. These budgets include an average allowance for room & board, transportation, miscellaneous, loan fees and, if applicable, child care and expenses related to disability. CONFLICTING INFORMATIONNDBA understands that all conflicting information must be resolved before any Title IV funds can be disbursed. Students with conflicting information in their files or paperwork may be selected for verification by the school or its servicer in order to resolve the information in question.

RETURN OF TITLE IV FUNDSThe law specifies how a school must determine the amount of Title IV program assistance that a student earns if they withdraw from school. The Title IV programs that are covered by this law are: Federal Pell Grants, Direct Student Loans and Direct PLUS Loans.Calculation of the Amount of Title IV Funds That a Student Has Earned Upon Withdrawal: When a student withdraws during the payment period, the amount of Title IV program assistance that they have earned up to that point is determined by a specific formula. If the student received (or the school or parent received on the student’s behalf) less assistance than the amount that the student earned, the student may be able to receive those additional funds. If the student received more assistance than they earned, the excess funds must be returned to the school and/or the student. The amount of assistance that the student has earned is determined on a pro-rata basis. A calculation is done to determine the percentage of the payment period completed. Only scheduled hours are used to determine the percentage of the period completed by a student withdrawing from a clock-hour program. From there, the amount of aid received for the payment period is multiplied by the percentage of period completed to determine the amount of aid earned. For example, if you completed 30% of the scheduled hours in your payment period, you earn 30%of the assistance you were originally scheduled to receive. When the result of this calculation is great than 60%, the student earns all the assistance that they were scheduled to receive for that period. At the start of the program, the kit and books are issued to those students purchasing them from the school. Once the kit and books are received, the student is responsible for the total amount due. If for any reason the student is terminated or withdraws from the program, they can return only their textbook or workbooks that are in brand new condition as long as it is within 20 days of their withdrawal date. There cannot be any visible damage such as torn pages, bent corners, scratches on the cover, or writing anywhere in the textbooks. The kit is a sanitary item and once it is opened it cannot be returned for any reason.

Per the terms listed above, the student will be charged full price for their kit and any unreturnable books upon termination or withdrawal from their program. Therefore, the unreturnable items will also be excluded from the R2T4 step 5 prorated cost of attendance.

The Condition under Which a Student Would Be Owed a Disbursement of Title IV Program Funds upon Withdrawal from the Institution: If the student did not receive all of the funds that they earned, the student may be due a post-withdrawal disbursement. If the post-withdrawal disbursement includes loan funds, the school must receive the student’s permission before it can disburse the funds. The student may choose to decline some or all of the loan funds so they do not incur additional loan debt. The school may automatically use all or a portion of the post-withdrawal disbursement of grant funds (including loan funds, if they are accepted) for tuition, fees, and books or supplies (as contracted with the school). The school does need the student’s permission to use the post-withdrawal grant disbursement for all other school charges. If the student does not give their permission (some schools ask for this upon enrollment), the student will be offered the funds. However, it may be in the student’s best interest to allow the school to keep the funds to reduce their debt at the school. There are some Title IV funds that the student was scheduled to receive that cannot be disbursed to them upon withdrawal because of other eligibility requirements. For example, if the student is a first-time, first-year undergraduate and had not completed the first 30 days of the program before withdrawal, the student will not receive any Direct Loan funds that they would have received had they remained enrolled past the 30th day.

How the Withdrawal Date for a Student Who Withdraws Officially or Unofficially is Determined: For clock hour schools, the actual last date of attendance is the last day the student was physically in attendance; this is the withdrawal date. The determination date of the withdrawal date on a student who had been previously attending could be up to, but will not exceed 14 calendar days from that student’s actual last date of attendance. An active student is determined to be officially withdrawn on the day they notify the school’s administrative office of their intention to withdraw from school or to not return from an approved leave of absence. An active student is determined to be unofficially withdrawn when they have been absent for 14 calendar days from their last date of physical attendance without notifying the school’s administrative office. A student who does not return from an approved leave of absence is determined to be withdrawn on the day they were scheduled to return but did not.

Time Frame for the Return of IV Funds: The school has 30 days from the last date of attendance to complete an R2T4 calculation. The school then has 45 days from the date of determination of the withdrawal to return any funds due to the Title IV programs and no more than 60 days from the date of withdrawal or last date of attendance. The Order in Which Title IV Program Funds Must Be Returned: The school and/or the student, if applicable, must return Title IV funds to the programs from which the student received aid during the payment period or period of enrollment as applicable, in the following order, up to the net amount disbursed from each source:

1) Unsubsidized Direct Stafford Loans (other than PLUS loans).2) Subsidized Direct Stafford Loans.3) Direct PLUS Loans.4) Federal Pell Grants for which a return of funds is required.

The requirements for Title IV program funds when a student withdraws are separate from any Institutional Refund Policy that the school may have. Therefore, the student may still owe funds to the school to cover unpaid institutional charges. The school may also charge for any Title IV program funds that the school was required to return. A copy of the school’s refund policy is contained in the school catalog. If a student has questions about their Title IV program funds, they may call the Federal Student Aid Information Center at 1-800-4-FEDAID (1-800-433-3243). TTY users may call 1-800-730-8913. Information is also available on Student Aid at www.studentaid.ed.gov.

CODE OF CONDUCTNDBA is committed to ensuring the integrity of its employees and students with respect to all aspects of its schools and operations. Employees assisting students in the financial aid process are required to affirm that they will be 1) ethical and conduct themselves with integrity, 2) avoid any conflicts of interest and comply with all Codes of Conduct required by the school, 3) provide prospective and enrolled students with accurate and complete financial aid and policy information, 4) keep student information confidential and comply with the Family Educational Rights and Privacy Act (FERPA), and 5) comply with federal and state laws and regulations, accreditor rules, and school policies and procedures.

SCHOOL REFUND POLICY

CANCELLATION / POSTPONEMENT AND REFUNDS1) An applicant not accepted is entitled to a refund of all monies paid within 30 days. 2) If a student or legal guardian cancels their contract in writing any time prior to midnight of the third business day of the signing of the

enrollment agreement, all monies collected by NDBA shall be refunded. 3) Students withdrawing in writing after three (3) business days, but before commencement of classes, are entitle to a full refund of all tuition and

fees paid except the maximum cancellation charge or $150.00 or 25% of the contract price; whichever is less. 4) Postponement of Start Date, whether at the request of NDBA or the student, requires a written agreement signed by the student and the

school. The agreement must set forth: a. Whether the postponement was for convenience of NDBA or the student, andb. A deadline for a new start date, beyond which the start date will not be postponed: If the course is not commenced, or the student

fails to attend by the new start date set forth in the agreement, the student will be entitled to an appropriate refund of prepaid tuition and fees within 30 days of the deadline in accordance with the school’s refund policy and all applicable laws and rules concerning the Private Occupational Education Act of 1981.

5) The student will receive a full refund of tuition and fees paid if the school cancels or discontinues a Program/Stand Alone course within a period of time a student could have reasonably completed it, except that this provision shall not apply in the event the school ceases operation.

6) Additional charges for kit equipment, books or supplies that have been dispersed to a student are non-refundable and will not be in the calculations of the Refund Policy. Kit supplies and books that are unopened/unused, that are returned within 20 days, may be refundable.

7) The policy for granting credit for previous training shall not impact the refund policy.

REFUNDS FOR WITHDRAWLS/LEAVE OF ABSENCE/TERMINATIONS OR DISMISSALS/SCHOOL CLOSURENDBA will retain a cancelation fee, application and enrollment fee plus a percentage of tuition according to the Colorado State Refund Policy chart shown below:

1) All withdrawals must be written, state the reason for withdrawal and submitted to one of the Directors.2) All refunds will be made within 30 days from the date of withdrawal or termination which shall be determined by:

a. Student failure to return from an unexcused Leave of Absence (LOA) - the official date of withdrawal is the earlier date NDBA determines that the student is not returning or the day following the expected return date (per DPOS).

b. Postmark or the date written notice is delivered to a Director in person.c. Terminations/Dismissal will be the date on which the student violates published school policy which provides for dismissal, which shall

occur no more than 14 calendar days from the last day of physical attendance (excluding LOA).3) The refund policy will apply to tuition and fees charged in the enrollment agreement.

a. Students owing monies to the school will be asked to pay off all debts in full or make payment arrangements with the Financial Director.

b. Students defaulting on any payment arrangement may incur an interest rate of 18%, which will be assessed on the first day after default.

c. Any payment arrangement not met will require NDBA to follow legal action and will go to collections at the student’s expense; this includes any legal fees, interest charges, and any other fees that may have occurred due to a default of payment.

d. Any return of Title IV funds will be paid as applicable: 1) Unsubsidized Federal Direct Loan, 2) Subsidized Federal Direct Loan, 3) Federal PLUS/Direct PLUS Loan, 4) Federal Pell Grant, 5) Other Federal, State, private or institutional aid, and 6) the student. If the student has received personal payments of Title IV aid, they may be required to refund the aid to the applicable program. See Return of Title IV Funds located in the catalog.

4) Reasons for dismissal: a. Not fulfilling financial obligations to the school.b. Failure to comply with rules and regulations of the school. c. Failure to comply with the tardiness/attendance/academic policies of the school.d. Insubordination or failure to follow NDBA STANDARDS OF CONDUCT.e. After a student has been absent for 14 calendar days from their last date of physical attendance. f. In the case of a LOA, the documented date of return, or the student does not return.

5) School Closure Policy:a. NDBA will continue to teach out all students, if accepted by the student.b. Or arrange to have students transferred and accepted to another school in our geographic area and shall not charge the students a

greater amount than the original school would be entitle to under its contract and for which the student has not yet paid. c. Or the student shall be entitled to 100% of the prepaid, unearned tuition at the time of closure.

STUDENT INFORMATION

SATISFACTORY ACADEMIC PROGRESS POLICYThe satisfactory progress policy applies to all students enrolled in a NACCAS approved program while receiving Federal Title IV funds, partial funding assistance, or self-pay. Satisfactory Progress in attendance and academic work is a requirement to continue eligibility for funding. To ensure all students receive information on our Satisfactory Progress Policy, they receive the student handbook and catalog prior to enrollment.

ATTENDANCE POLICYAll students must attend school; in accordance with our Satisfactory Progress schedule. It is a requirement of the school to attend classes promptly and follow the schedule you have been given. Attendance is evaluated on a cumulative basis. Students may make up hours if approved by a Director and only at specific times. If a student leaves early without approval, they may lose their hours for the day unless a Director determines extenuating circumstances apply and grants an approval. Actual attendance is based on the number of successful completed clock hours divided by the number of scheduled clock hours. Students are required to maintain 80% of clock hours achieved versus attempted. During Basics, students are encouraged not to be absent as information may not be repeated until the next Basics class. Continued absences may result in a warning and/or termination. Poor attendance will affect GPA and SAP. Any student will be dismissed if it is determined they cannot complete their program in the allotted 125% maximum time period. When a student takes a LOA, the student’s graduation dates and maximum time will be adjusted to reflect the length of the LOA. Transfer Hours accepted by the school are counted as both attempted and completed hours for max time and SAP. Periods of time when a student is not receiving Title IV financial aid are still counted towards max time.

ACADEMIC PROGRESS Qualitative and quantitative factors will be evaluated by tests, projects and practical worksheets using the shown grading system. If a student grade on their weekly practical sheet is below 75%, then they must make up the grade sheet and will not be granted the credit. Practical grade sheets are turned in weekly. Incomplete grade sheets will have to be made up at the end of your program which may result in additional monies being owed. If a grade sheet is lost or misplaced, a new one will be issued and the student will be required to start it over. Students are not allowed to have more than two grade sheets at any given time. A cumulative GPA of 75% must be maintained in order to meet SAP requirements. Progress reports are given to students monthly, although SAP is monitored based on the following determination of progress. SAP evaluations will be kept in the student’s file and can be requested by a student at any time.

DETERMINATION OF PROGRESSA student’s SAP is evaluated at the end of each of their payment periods. The Department of Education defines the minimum academic year for clock hour programs as 900 hours and 30 weeks. Payment periods are calculated as the midpoint of the student’s academic year. Programs with over 900 hours will have two academic years. If a program or remaining portion of the program is less than 900 hours, the academic year is the entire program or the remaining amount of the program and the midpoint is the payment period. Students meeting requirements at the end of each payment period are considered to be making Satisfactory Progress until the next evaluation period. In order to be considered making Satisfactory Progress, the student must meet both attendance and academic minimum requirements in both quantitative (academic terms) and qualitative (academic performance). Students will be evaluated within three days that they are scheduled to complete the end of each evaluation period (points), and notified of SAP results:

Please Note: NDBA is a continual school and not term-based, therefore there is not a summer term.

WARNINGStudents who fail to meet requirements for attendance or academic progress will be placed on Warning Status. Students on Warning Status may continue to receive Title IV funding for one evaluation period. Students are not required to appeal a Warning Status.

APPEAL AND PROBATIONStudents on Warning Status who do not meet requirements at the next evaluation point will lose eligibility for Title IV funding. Students who are not meeting satisfactory academic progress who wish to appeal the school’s determination of loss of eligibility (for reasons such as death of a relative, injury/illness of the student, or other special circumstances) must appeal within the required time frame and on the form required by NDBA (see Appeals). If the school grants the student’s appeal, the student will be placed on Probation Status until the next evaluation point and the student’s eligibility for Title IV funding will be reinstated. It must be determined that the student will be able to meet the SAP standards by the next evaluation period for probation to be granted. A student may do this by: 1) Making up missed tests and assignments and increasing grade average to 75% or better, and/or 2) Increasing cumulative attendance to 80% (i.e. make up hours, attend scheduled hours). A student may be placed on Probation Status for one evaluation period. The academy determines Satisfactory Academic Progress at the Evaluation points stated in the chart under Determination of Progress, mathematically, should they not meet SAP by the following evaluation period they will lose eligibility for Title IV funds.

APPEAL PROCEDUREIf a student is determined as NOT making Satisfactory Progress, the student may appeal the negative determination. The student must submit a written appeal on the school’s form to the school administration within 5 days of the determination, with supporting documentation including reasons why they failed to meet SAP requirements and why determination should be reversed. This will need to include what has changed or will be changing and a request for re-evaluation. An appeal hearing will take place within five (5) business days of receipt of the written appeal. This hearing will be attended by the student, parent/guardian if applicable, and appropriate school Staff. A decision on the appeal will be made within ten (10) business days after this hearing, and will be communicated in writing and documented in the student’s file. This decision will be final. Students who prevail upon appeal will have eligibility for reinstatement of Title IV funds. Students who do not achieve minimum satisfactory progress standards and do not prevail upon appeal are no longer eligible for Title IV funds. If a student is denied an appeal, they may choose to continue on with school as a self-pay student until they can reestablish their status (see Reestablishment of Status below).

DETERMINATION DATE / WITHDRAWAL DATE (Official VS Unofficial)The actual last date of attendance would be the last day the student was physically in attendance. A withdrawal date on a student who had been previously attending could be up to, but not to exceed 14 calendar days from that student’s actual last date of attendance. An active student is considered officially withdrawn when they have been absent for 14 calendar days from their last date of physical attendance.

REESTABLISHMENT OF STATUSA student determined NOT to be making Satisfactory Progress will become ineligible and may reestablish Satisfactory by: 1) Making up missed tests and assignments and increasing grade average to 75% or better, and/or 2) Increasing cumulative attendance to 80% by the end of the evaluation period. A student cannot reestablish eligibility if they do not bring their grades and/or attendance back into SAP, even if they self-pay for the period. An interruption in enrollment will also not reestablish eligibility without the student demonstrating SAP. The minimum requirement for reinstatement of aid is a cumulative GPA of 75% and 80% of clock hours achieved versus attempted.

REINSTATEMENT OF FINANCIAL AIDTitle IV aid will be reinstated to students who have prevailed upon appeal regarding the status of Satisfactory Progress and have been on Probation or who have reestablished Satisfactory Progress by their next evaluation point.

COURSE INCOMPLETES, REPETITIONS AND NON-CREDIT REMEDIAL COURSESCourse incompletes, repetitions and non-credit remedial courses have no effect on the institution’s Satisfactory Progress Policy. A student cannot withdraw from individual courses/subject matters.

Students who have been terminated or withdrew from school may re-enroll (if determined eligible) within 180 days of their last day of attendance and will not incur additional charges, however these students will be responsible to pay any remaining balance from the previous enrollment that cannot be covered with reinstated federal funds.

A determination of Satisfactory Progress will be made and documented at the time of withdrawal. That determination of status will apply to students at the time they return to school. The student may appeal a negative Satisfactory Progress determination according to the appeal policy. Elapsed time will be treated like a Leave of Absence and does not affect Satisfactory Progress, and will extend the contract period by the same number of days. Students re-entering after exiting the school will not be evaluated as new students and consideration will be given to the student’s progress status at the time of previous withdrawal.

Students who have been terminated or withdrew from school and re-enroll (if determined eligible) after more than 180 days from their last day of attendance and will pay a $100 registration fee and will be charged for contracted hours at the current tuition rate. All re-enrolling students will be provided the school’s re-enrollment policy and will be evaluated by the school Director for placement in the curriculum and kit needs. The student may be required to repeat credits/hours that were previously earned. Re-enrolling students may be required to purchase the current school kit. Students applying for re-entry or transfer-in from other schools may be required, as a condition of enrollment, to bring delinquent prior student loans to a current status.

Students who re-enroll after more than 180 days, into their original program will have their Colorado refund calculation revised to be charged specifically for the credits/hours completed – if any payments were made that are in excess to the revised calculation, that amount will be applied to the new contract. If a student switches from one program to another program, the student will be required to pay the original refund calculation in full. Payment plans may be available. For students who switch from one program to another or choose to enroll in a second program, they will be treated as transfer students. Transfer Hours accepted by the school are counted as both attempted and completed hours for max time and SAP (Policy can also be found under our Admissions Policy).

LEAVE OF ABSENCE (LOA)All students must follow the Institution’s policy on requesting an LOA. A student must submit, in advance, a leave of absence form in writing which is signed and dated to a Director, unless an unforeseen circumstance prevents a student from providing a written request. NDBA will document a decision and collect the written request at a later date. An LOA will only be approved is there is a reasonable expectation that the student will return. When a student takes a LOA, the student’s graduation dates and maximum time will be adjusted to reflect the length of the LOA. The Financial Services Department will be notified of the student leave. An LOA addenda will be documented and signed by all appropriate parties.

1) LOAs are on an approval basis with the Director’s discretion. A LOA may not be granted if a situation arises that may affect students who are currently on probation for attendance or academic issues, scheduling conflicts, or a late request. LOAs may be granted for medical, vacation/personal, and emergencies, etc.

2) A leave of absence will extend the student’s contract date and maximum time frame by the same number of days of the leave of absence. The start date of the LOA will be recorded as the first date the student was unable to attend school.

3) A student’s new graduation date will be documented in their LOA addenda. 4) Students who are on a leave of absence must continue to meet their obligations of payment unless other arrangements have been agreed upon

on the LOA addenda. Further questions on payment obligations can be answered by the Finance Director. 5) No Title IV loan funds (i.e. Stafford Loans or PLUS Loans) will be disbursed while a student is on a leave. 6) No State financial aid will be disbursed while a student is on leave.

NDBA policy allows multiple leaves up to (4) for Cosmetologists and Barbers, (3) for Hairstylists, and (2) for Estheticians and Nail Technicians at the discretion of the Directors. Students needing extra LOAs will be approved at the discretion of the Director. Leaves of Absence cannot exceed 180 calendar days in a given year, based on the first day the LOA was taken. LOA requests will not be approved for less than seven (7) consecutive calendar days, and may not be approved during the Basics training phase of a program, unless special permission is obtained from the Director. During Basics, is a LOA is granted, the student may have to continue their LOA until the missed module repeats, depending on the subject of the module. A student granted an LOA that meets these criteria is not considered to have withdrawn, and no refund calculation is required at that time. NOTE: Please see Dismissal #5 below. This would constitute an unapproved LOA. If the student is unable to return to school within the maximum timeframe for a LOA, their enrollment will be terminated. A returning student on LOA will not incur additional charges by NDBA. If a student fails to return from LOA or an extended LOA, the official date of withdrawal is the earlier date the Academy determines the student is not returning or the day following the expected return date. For the purpose of calculating a refund, the withdrawal date is always the student’s last day of attendance. All refund calculations will also be based on that date.

DISMISSALNDBA’s intent is to prepare every student for a career in their field of choice, and the Staff and Directors will advise all students should any difficult situation arise. The final decision for any dismissal will be made by the Directors. If a student has been dismissed from school, they can be considered for re-enrollment and re-entry fees will apply per the Re-entry/Re-enrollment Policy. Any student will be dismissed if it

is determined that the student cannot complete their program in the allotted 125% maximum time period. The school may dismiss a student prior to the completion of their program for one or more of the following reasons:

1) Failure to fulfill financial obligations to the school.2) Failure to comply with the rules and regulations of the school. 3) Failure to comply with the Tardiness/Attendance/Academic Policies of the school.4) Insubordination or failure to follow NDBA STANDARDS OF CONDUCT.5) Absent 14 calendar days from the student’s last date of physical attendance. 6) In the case of a LOA, the student does not return on the documented date of return.

FRIDAY / SATURDAY ATTENDANCE POLICYScheduled Fridays/Saturdays are mandatory unless prior arrangements were made in writing and approved with an Instructor and Director. Calling in on any given Friday/Saturday will not be approved as an excused absence without valid documentation for missing said day. The $100 fee may be waived based on documentation and at the Instructor’s or Director’s discretion. Students have one week to submit doctor’s note from their return of absence. Due to students not always being able to have documentation for their absences or needing to request a specific Friday or Saturday off, each student is allotted a number of fees that can be waived based on their program and floor schedule listed below:

EXCUSED ABSENCES / UNEXCUSED ABSENCES

Excused Absence: A student may request a day off ahead of time by filling out their student schedule change form. Student needs to follow requirement directions of getting proper signatures and submitting to the Front Desk Coordinator. Unexpected excused absences, such as illness or emergency requires students to call in within a half hour of their scheduled start time.

Unexcused Absences: Calling in after more than 30 minutes of scheduled start time, or no call/no show to scheduled school day is unacceptable and my lead to a being placed on probation, suspension, or dismissal. Unexcused absences may be excused at the discretion of a Director

Both requested and unexpected excused absences would need to be made in order to graduate on time.TARDIESExcessive tardiness may affect your satisfactory progress resulting in disciplinary action. Students more than 10 minutes late for the theory class will not be allowed to interrupt without instructor approval.

TIME CLOCK POLICYStudents are responsible to clock in and clock out for days/hours attended in order to receive their credit.

1) Anyone who has not clocked in/out appropriately will be subject to loss of hours.2) At any time a student leaves the building (smoke breaks, walks, use of cell phone, etc.), they are required to clock out. If a student is at lunch or

on a break outside of the building, they must notify the student/client coordinator.3) A student may not clock in or out for another student. 4) Violations concerning the Time Clock Policy are grounds for disciplinary action and/or termination.

DRESS CODEStudent’s dress attire is a reflection of NDBA. Students not complying with the dress code can be sent home and put on probation. NDBA reserves the right to make a judgment on whether an individual’s attire meets dress code requirements. Students are required to wear ONLY the following attire. Items of clothing not listed are not permitted. Exceptions to the dress code may be made at the discretion of the Directors or documented medical circumstances only. It is important students remember they are in a professional service and fashion business. Our appearance to the public states our desire to be professional in all areas of our chosen profession.

1) All students will wear a name tag if you are clocked in. If you have lost it, a new one will be issued at a fee of $4, which will need to be paid for at that time, or can be invoiced to the student’s account at their request/approval.

2) Personal items should be kept in student’s lockers when they are on the clinic floor. 3) NDBA requires that students dress in a professional manner. A NDBA smock, pressed and hemmed. Medical scrubs, slacks, capris, skirts (no

shorter than 2” above the knee) and Bermuda shorts (no shorter than 2” above the knee) are acceptable; all colors are currently allowed. Leggings are permitted (conservative detail or sheer siding accepted), only if covered by a smock or tunic 5” above the knee. Any color of shirt with no logos is accepted. Tank tops under smocks are permitted. Joggers may be worn, but must not be sweatpants material. Denim of any color is allowed, as long as it is conservative. While manufactured distressed slits are acceptable, denim cannon be torn or ripped. Clothing is to

be clean and professional, and in a good state of repair at all times. Items of clothing that are faded, torn (in an inappropriate fashion or area), or ragged are not permitted. If at any time clothing appears tasteless, offensive or too revealing, Staff may ask you either to change or go home.

4) We strongly recommend shoes are of good quality and adequate support. No high heels or open-toed shoes are allowed on the hair floor mats. Socks and hosiery are permitted. Flip-flops and slipper shoes are not permitted. No dirty or multi-colored tennis shoes. Shoes much be clean, professional and in a good state of repair at all times.

5) Hair and makeup is to be professionally styled and completed before clocking into school. Do not arrive at school to do your hair and makeup before or after your class has begun.

6) Students must be hygienically clean, use deodorant, have fresh breath and be well-groomed at all times. 7) No sweatshirts, hoodies, sleeveless tops, midriff tops, shorts, no visible advertisements no visible spaghetti straps, spandex or see-through

shirts. During cold weather, students are permitted to layer clothing. 8) Trendy, stylish hats are permitted by pre-approval only. 9) Body Piercing will be discreet and not outlandish. NDBA reserves the right to determine appropriateness of piercings. 10) Tattoos will be discreet and not outlandish. School Directors reserves the right to determine appropriateness of tattoos.

STANDARDS OF CONDUCTThe student STANDARDS OF CONDUCT have been prepared to provide the necessary guidelines for effective student relationships and apply to all students enrolled at NDBA. If these standards are not followed, the school may place a student on probation, suspension or dismissal.

1) All students must attend classes as scheduled an on time daily. Some exceptions include if the student has a documented doctor/dentist visit or a documented court appointment. Students are required to maintain attendance of 80%. 100% attendance is required for graduation, therefore make-up hours are available. Students receive monthly Progress Reports. No call in or no show to school is unacceptable behavior.

2) Students are responsible to get their worksheets/sanitation sheets signed daily after performing their tasks. If a sanitation duty is not completed, you must inform your instructor or the client/student coordinator why, and you could be assigned another task.

3) Cigarette/Vape smoking is not permitted except in designated areas and NEVER inside the school. Eating and drinking are only permitted in the lunchroom. The only exception is a closed container at your station. Breath mints are acceptable. Gum-chewing should be a consideration during client services.

4) Staff and clients must be treated courteously and professionally at all times. Clients should never be left waiting; students will adjust lunch periods and breaks accordingly. If necessary, a client may be moved to another student to accommodate lunch or break.

5) Only topics of ethical and professional subject matter may be discussed among students while serving clients, in classrooms, or on break time on the school premises. Unprofessional language, profanity, inappropriate slang, spreading rumors or gossiping is not permitted. Tips should not be discussed.

6) Students may NOT refuse to perform a client service even if all areas on your grade sheet are completed. Students may not refuse to do a sanitation duty. If this occurs, the student may be sent home, put on probation and/or suspended. Exceptions not to perform a service could be if a client has a communicable disease which will be determined by the floor instructor.

7) Conflicts of any nature with another student or Staff member will not be permitted on the school premises.8) Insubordination (defiance of authority) is not permitted.9) Use, possession, or sale of nonprescription drugs or alcohol is not allowed on the school premises. Students whose behavior is influenced by the

use of nonprescription or prescription drugs, alcohol or any unknown substance will be asked to leave.10) Cheating, dishonesty, falsification of records or theft is not permitted. NDBA reserves the right to search personal property if just cause is

present. NDBA is not responsible for loss of personal property. 11) A student must have Staff approval for breaks and lunch. NDBA follows the Department of Labor & Employment guidelines.12) Willful destruction of school property is not permitted. Any student vandalizing school property or equipment will be financially responsible for

replacement of the article(s) and will be subject to disciplinary action.13) NDBA prohibits the presence of firearms, weapons, or other hazardous devices or substances on school grounds. 14) NDBA prohibits the use of electronic devices such as cameras, camera phones, recording devices, etc. on school premises without advance

permission by the Directors and the stated individual. Uses of these devices are a violation of individual’s personal privacy.

15) Any pictures of clients require permission for a student portfolio. 16) You will be given a “traveler” (ticket) for each client, friend or family member or personal service. Make sure every service performed is re-

elected on the traveler. If you have any questions about additional charges, ask your instructor or front desk coordinator. If you do not bill your client, friend or family member correctly, you will be held responsible to collect the money.

17) Suggest retail products to educate your client on proper hair, nail or skin home care. Walk your client to the front and ask them if they would like to reschedule.

OCCUPATIONAL OBJECTIVE: To educate the student in the art of Permanent Makeup from a cosmetic viewpoint. Students will be educated in the aspects of sanitation, disinfection and safety for Permanent Makeup. Students are taught machine and manual techniques, with a main focus on machine application. Our program follows and fulfills all Colorado requirements. Students will fulfill class requirements with theory, lab projects, practical and hands-on techniques. Students work on patrons throughout the practical segment class. We focus on full preparation with our students including pre and post procedures, intensive client consultation, risks and complications, and business and marketing perspectives. GRADING PROCEDURES:

Students are assigned academic learning and a minimum number of practical experiences. Academic learning is evaluated monthly. Practical assignments are evaluated as completed and counted toward course completion only when rated as satisfactory. If a student grade on their weekly practical grade sheet is below 75%, then he/she must make-up the grade sheet and will not be granted the credit. The second grade earned will be the grade of record for the student’s GPA. Students are required to maintain a 80% cumulative attendance average and 80% average of clock hours achieved verses attempted. GPAs and clock hours will be evaluated monthly. Student’s GPA and clock attendance will be evaluated at the end of each evaluation period (points), and notified of SAP results. If a student does not pass a test with a minimum of 75%, it must be retaken and the higher grade will be the grade of record. A cumulative GPA of 75% must be maintained.

Any time NDBA refers to the contract date, the On-Time Completion Date is used.

16) Students having to repeat a Segment of Basics due to not passing either the week or Basics test-out will have to retake and will be charged $315/weed for full-time and $236/week for part-time.

17) Kit Rentals (optional): Nail Kits = $15; Esthetics Kits = $20; Cosmetology/Hairstyling/Barbering = $30. Outside students will require a $50 rental fee and a deposit for the kit in the amount of the kit cost. If a student fails to return the kit or if the kit has missing items, the student will be charged the full price of the item.

ADDENDA - 04/20: POLICY ON DISTANCE EDUCATION

NDBA’s Policy on Distance Education is in compliance with local, state and federal laws and regulations and follows the Standards and Criteria set forth by NAACAS. Policy VI.02 Curriculum: Policy on Distance Education has been utilized for reference during the assembly of this school policy along with the additional exceptions that were released on April 16, 2020. Evidence of State approval was given from DPOS to utilize distance education. There were no restrictions set forth by the state for units of instruction as there is currently no policy in place at this time. Evidence of this is listed on the approval. Individual students attending the institution that have been quarantined due to the virus must maintain the Stay at Home order their medical documentation given to the school will be maintained in the student’s file.

All interaction between instructor and student will be validated by measureable participation in terms of clock hours earned. All students are evaluated for attendance, participation, practical and theory skills that are exhibited during distance education. The number of hours of instruction designated for each unit must be noted. This list must account for all hours of instruction within the course/program and indicate which of those hours might be offered/earned during distance education. Distance education hours are not to exceed 50% of the program of study. We request that students complete the last 48 hours of training within the building in order to complete an evaluation of information learned during distance education (i.e., practical and written exams along as other graduation requirements).

Assessments will be used to evaluate a student’s GPA. When classes resume within the physical building, students will be re-evaluated to validate their comprehension of all material that was taught during distance education. The distance education component must be identified by all documentation, including transcripts (official or unofficial), that were received during this time frame.

Prior to enrollment or during this time of crisis, students are provided with a disclaimer that any academic achievement earned during distance education may not be accepted for reciprocity or eligible for licensure in other states. A signed and dated copy of this disclosure can be found in each student’s file that was actively participating in distance education. Due to Colorado’s stay-at-home order, during our approval procedure, this correspondence took place via email. Students are asked to sign the email once the stay-at-home is fully lifted and they can resume their education within the building.This policy may be rewritten to accommodate changes that may occur and need to be addressed during this unforeseen crisis.

ADDENDA – PUBLIC HEALTH ORDER – COVID 19- 05/20Public Health Order - Safer at Home Guidelines

Governor Polis issued an executive order on March 11, 2020 declaring a disaster emergency in Colorado due to the presence of COVID-19. All public and private gatherings of any number occurring outside the residence are limited to no more than (10) individuals. Safer at Home still required that vulnerable individuals remain in their homes. People who are sick must stay in their residence at all times except as necessary to seek medical care. New Dimensions Beauty Academy, Inc. shall take the following measures within the workplace/institution to minimize disease transmission in accordance with the CDPHE Guidance. Some adjustments are going to be required of all of us, until things can return to normal. For 24 hours a week T-F 9-330, we will resume classroom interaction. At this time, one instructor will be designated to nine students. Students are required to stay in their designated areas. All students and staff are required to wear a mask at all times. Students will be allowed a fifteen minute break per day-for smokers and non-smokers. This can be divided into two seven and a half minute breaks. If you are stepping out of the building you will be clocked out for this break. Your mask may be removed during smoke break, however, a sanitation station will be set up at the back door and students or staff leaving to go outside for a break will be required to sanitize prior to leaving or entering the building. If ten people are already outside, you will need to return into the building until someone else comes back in, 6 feet distancing is required. Students may bring a camping chair for lunch break purposes- your half hour break will need to be taken when your class is approved to go to lunch. You will be permitted to use the microwave and eat lunch in the parking lot by your vehicle or off campus. Instructors will be clocking students out to comply with restrictions. A sign in/out sheet will be in the basics classroom and by the back door off of the clinic floor. Students will need to provide their own pen for signage. When clients are present, we have more diligent restrictions. After 3:30 on Tuesday, Wednesday, Thursday and Saturday 9-330 will be times when clients services will be available. A lottery will be executed to determine fairly what students can be rotated for client services and/or makeup hours, if needed.

The following COVID 19 guidelines must be followed for person to person interaction:

Clean and disinfect all high touch areas regularly. Staff will be clocking students in and out to avoid congregation and minimize points of contact. Stations will be moved to maintain 6 foot social distancing requirements.

Ensure proper ventilation of classrooms and office spaces. Limit equipment sharing and resources. Basics area has its own separate space with entrance/exit and bathroom- this space will not have open access and will maintain all

social distancing requirements. Discourage all unnecessary travel for students and staff. Inform faculty, staff and students of requirements to self-isolate or

quarantine if they travel. Refer to http://covid19.colorado.gov/isolation-and-quarantine. Post signage for employees, students and customers on good hygiene (i.e., wash hands with soap and water for twenty seconds

Executive Order D 2020 044. Stylists will wear face covering and gloves at all times, or, if wearing gloves is not feasible or appropriate, meticulous

handwashing. Stylists will change gloves and wash hands between every client. Stylist to clean and disinfect all shared equipment and tools between every client. Use contactless payment solutions (credit card). No touch trash cans. Provide customer services by appointment only, do not allow walk-ins or waiting for an appointment. Skin services may not include facials, microdermabrasions, chemical peels, dermaplaning, permanent makeup for lips, face or lip

waxes until restrictions are lifted.
