New Horizons Healthcare Overview� New Horizons Healthcare provides comprehensive senior home care services
throughout the Chicago-land area
� We support our clients’ desire to live independently at home while providing their
family members the peace of mind that their loved one is well taken care of
� We employ compassionate and highly trained caregivers who ensure that our
clients remain safe and healthy at home by providing the following services:

� Medication Reminders � Bathing � Laundry � Transportation
� Exercising � Dressing � Hygiene Assistance � Care Plan Compliance
� Meal Preparation � Companionship � Light Housekeeping � Social Interaction
Through the New Horizons Healthcare Veterans Program, qualified veterans and their surviving spouses can receive from 12 to 22 hrs./week of our services at no cost
Aid and Attendance Overview� Aid and Attendance (A&A) is a tax free non-service
connected disability pension provided by the VA
� Unlike a service connected pension, this pension must be used to pay for un-reimbursed ongoing medical expenses including home care
� Wartime veterans and surviving spouses can receive between 12 and 22 hours per week of home care a week all paid by the VA
� The Aid and Attendance Pension has the following requirements:
� Service Requirement
� Medical Requirement
� Financial Requirement

Wartime veterans with spouses 22 hrs./week
Single wartime veterans 18 hrs./week
Surviving spouse 12 hrs./week
Aid and Attendance Requirements
� 90 days active duty (Not
National Guard/Reserves)
� Served a minimum of one
day during an approved
wartime
� Honorable discharge
� Surviving spouse must
have been married to the
Veteran at the time of
his/her death and NOT
remarried

Service Medical Financial
Applicant must be a veteran or the surviving spouse of a veteran with the following criteria:
� Over the age of 65
� Applicant must be 60% or
more homebound
� Require assistance with at
least two daily living
activities (bathing,
dressing, personal care,
ambulating etc.)
� Medical condition must
be certified by a physician
� Assets typically need to be less than $25,000 for a single person or $50,000 for a couple
� Total assets include all cash accounts, stocks & bonds, IRAs, additional properties
� Home of residence and a car are not considered assets
� Income needs to be less than A&A pension amount
� Income can be reduced by deduction of unreimbursed medical expenses

� Though 1 out of every 3 seniors over the age of 65 is either a veteran or a surviving
spouse of a veteran, only a small fraction of seniors that would qualify take
advantage of this program
� It is a “Catch-22” – you need to have care in place in order to prove to the VA that
you need care
� Paying out of pocket for home care during the application process (between 3-12
months or more) becomes very expensive
� If you fill out one of the 15+ forms required by the VA incorrectly, your application
can be rejected or delayed and the out of pocket expense already spent on home
care is gone
� Also, The VA randomly audits A&A Pension recipients and requires detailed
documentation to verify the recipient has stayed compliant with the benefit
requirements. Non compliant recipients will be back billed by the IRS for benefit
paid out to them

� We provide an interest free loan to families to allow them to get care
services in place during the 3-12 months it takes for their pension
application to get approved by the VA without a penny paid by the
Veteran/Surviving Spouse
� Our team of Accredited VA Benefits Agents will work with you and your
family to complete the application forms for you and submit them to the VA
on your behalf
� Our team of Accredited VA Benefits Agents
will work with you and your family to ensure
appropriate documentation is kept and
adequate communication with the VA occurs
to maintain compliance and pension

A New Horizons Healthcare Case Manager visits Veteran’s or Veteran Spouses’ home to ensure program requirements are met
Documentation and information needed to be gathered is discussed in detail
A customized care care plan is developed that maximizes independence and quality of life

Our Accredited VA Benefits Agent guides Veteran, Spouse and/or Family to gather necessary documentation
We are here to help with all 15+ forms that need to be completed
We will check every form for accuracy and compliance with the program to ensure application will be accepted by the VA
After all documents are received and application is reviewed for accuracy, we submit application to VA on your behalf
We then provide a zero interest loan to the family in order to put home care in place immediately
Not a penny is ever paid by the veteran, surviving spouse or family member!
Our highly trained and compassionate caregivers will assist the veteran or surviving spouse with activities of daily living from 10 up to 22 hours a week
Once VA grants pension, the VA retroactively pays for all care provided during application process
This retroactive payment will be used to pay us back the zero interest loan

� There is certain information that we must have to discuss your situation:
� Date of military service & discharge status of Veteran
� Gross monthly income & type and ownership of assets
� Medical Condition/Assistance with Activities of Daily Living
� Surviving Spouse – marriage license to Veteran
� Once you have this information you should call the office and discuss this
with one of our representatives
� They will review your information to determine what your next step will be
