Health is a gift from god
although patient admits that
he neglects to take good care
of his health. Pt. only seeks
health care when condition
worsens.
Patient says that god has
plans for him and says that
he religiously comply with his
treatment and medications.
Adherence to treatment usually
requires that a person make on
or more lifestyle changes to car
out specific activities that
promote and maintain health.

NUTRITION /
METABOLIC
Patient normal diet has
shellfishes, fish and
sometimes meat, patient
stated that he likes grilled
foods and fried foods. He
seldom drinks water 3 glasses
/day more of soda such as
“sparkle” 3 bottles per day.
Patient is on NPO and only
has TPN and IVF of PNSS
regulated at
Pt wt?? gained or loss?
Most patients with cancer
experience some wt. loss during
their illness. Impaired nutritiona
status may contribute to both
physical and psychological
consequences.
Most IV fluids do not provide
sufficient calories or protein to
meet the body’s daily
requirements. PN solutions can
provide enough calories andnitrogen to meet the pt daily
nutritional needs. (MS Brunner

ELIMINATION Patient stated that he
defecates every other day.
Urinates for about 7 to 8 times
a day and does not experience
any discomfort or pain.
Patient states that he has had
having different defecating
usually 3 times per week, he
also experience discomfort
and compares his stool as
that of chicken food, also he
states that it is somehow
watery. Patient states that heurinates with less volume
and with a color of yellowish
to orange.
ACTIVITY Patient states that his usual
activity were doing carpenter
and fixing houses, his usual
exercise was walking and
stated that he seldom do
strenuous exercise.
Patient state that he no
longer performs exercise and
states that his activity was
limited to listening to radio
because of pain and due to
his condition.
SLEEP Patient states that his hours of
sleep ranges from 8-10
hours/day periods of nap in
the afternoon.
Patient verbalized that he
was having different sleeping
pattern because of the
environment and because of
his pain and frequent taking
of VS. Patient stated that the
longest sleep he had was 1
hour.
Pt. more likely to awaken
because of factors such as
frequent taking vital signs, pain
and noise. Having inadequate
sleep can affect pt. health. The
lack of quality sleep at night
often creates the need for
napping during the day. (MS

COGNITIVE Patient stated that he uses Patient realized that he no

from his wife and after his

his kids.
Patient verbalize that he feels
sad because he cannot
provide for his 2 kids and that
he is not being visited by
them. His only companion
was his nephew.
SEXUALITY Patient has 2 kids and the
separation he no longer have
sexual activities.
Patient stated that up to now
he has no sex activity.
COPING/STRESS Patient verbalized that
whenever he has problem he
seldom thinks about them and
prefers to just forget about it.
Sometimes when the problem
became worst he prays to
god.
Patient verbalized that his
only stress is hos condition
and not seeing his loved
ones. He says that he copes
with this by praying and
listening to the radio.
Patient typically experience
distress related to alterations in
his physical and emotional heal
status, change in his level of dai
functioning, and decreased soci
support or the loss of significan
others. Isolation, loneliness,
sensory changes, financial
problems and disability increase
a person’s anxiety level and like
to cause ineffective coping.
Social support has been
demonstrated to be an effectiv
moderator of life stress (MS
Brunner p. 80)
VALUE and BELIEF Patient states that he is aChristian born again. He also
stated that he seldom goes to
church and only prays
spontaneously.
Patient stated that he praysmore after and states that
god is his only hope and that
he has plans for him.
Ideas, goals, perception andspiritual beliefs influence one’s
choices and decisions, enhance
feeling of connectedness to God
and promote total health of
mind, body, and spirit.
