APPENDICE - Curriculum Vitae

CURRICULUM VITAEMarian Smeulers was born in Eindhoven, the Netherlands on January 30th 1969. After finishing secondary school in 1986, she graduated from Physical Therapy at the Hogeschool van Eindhoven in 1990. She continued her studies with Medical Informa-tion Science at the University of Amsterdam, from which she graduated in 1994. From 1994 until 1996 she lived and worked in Indonesia where she assisted with the set up a local hospital rehabilitation unit. After her return to the Netherlands she began her

career at the Academic Medical Center (AMC) in 1997. She joined the ICT department and worked on the development and implementation of the AMC Electronic Patient Record. Ten years of experience as an intermediary between the hospital departments and the ICT department, resulted in extensive knowledge of healthcare processes as well as the opportunities of information technology. In 2008 it was time for a new challenge and she joined the new established department of Quality and Process Innovation with Piet Bakker. There she was given the opportunity to start a part time PhD program on quality improvement, which resulted in this thesis. Besides her research activities on medication safety and safe handover of care, she was also part of the team that supported the JCI quality accreditation of the AMC in 2012. After her PhD she will remain working to further support the improvement of quality and safety in healthcare. Marian lives together with her partner Erwin and their two children Niels and Laura.

DANKWOORDOnderzoeken en publiceren doe je gelukkig niet alleen, daarom ben ik blij dat ik in dit deel van mijn proefschrift de ruimte heb om iedereen die aan mijn promotie heeft bijgedragen te bedanken.

Mijn promotor Prof. Dr. Piet Bakker – onze eerste kennismaking was in 1998 toen je me de kans gaf een digitale poliklinische diagnoseregistratie te realiseren in het EPD en ook daarna volgden nog vele andere gezamen-lijke EPD projecten. Ik was blij verrast dat je me in 2008 vroeg om voor KPI te gaan werken, een keuze waar ik nooit spijt van heb gehad. Ook gaf je me de kans en de ruimte om naast mijn werk dit promotietraject aan te gaan. Je visie en kennis over de organisatie van de zorg en je vele ideeën over hoe het beter kan, waren een bron van inspiratie. Kortom, dank voor 17 jaar samenwerking en vertrouwen.

Mijn co-promotor dr. Hester Vermeulen – de afgelopen 7 jaar zijn we inten-sief samen opgetrokken. Met jou gedegen achtergrond in EBP en verpleeg-kundig onderzoek, zijn we samen gaan pionieren in het onderzoek naar kwaliteit en veiligheid. Ik bewonder je energie en geduld om altijd weer op zoek te gaan naar manieren om de zorg te verbeteren en daar natuur-lijk gedegen onderzoek aan te koppelen. Dank voor al je tijd, steun en vertrouwen van de afgelopen jaren. Ik heb veel van je geleerd en ik hoop dat we in de toekomst nog vaker samen leuke onderzoeken kunnen doen. Commissieleden – prof. dr. Alle AMC-ers en medeauteurs die op enigerlei wijze hebben meegewerkt aan één of meer van de onderzoeken - alle bijdragen groot of klein zijn onmisbaar en van onschatbare waarde bij de totstandkoming van onder-zoek. Zonder jullie was dit proefschrift er niet geweest. Dank daarvoor!

Astrid en Myra – jullie kennis en ervaring in de wereld van kwalitatief onder-zoek was onmisbaar, ik heb ontzettend veel van jullie geleerd!

Lotte – onze intensieve samenwerking zal ik nooit vergeten en ik ben je

enorm dankbaar dat je ook na je vertrek naar Canada bereid bleef om te helpen met de afronding van onze onderzoeken. Het tijdsverschil had ook voordelen, als ik eind van de middag wat stuurde, had ik het vaak de volgende ochtend al terug .

Catharina – Dank voor je steun en alle handige tips die ik van je kreeg in deze laatste fase van promoveren. Ik denk nog met veel plezier aan Glasgow, het was gezellig.

Els – heel veel dank voor je ondersteuning bij de organisatie en planning van de laatste fase van mijn promotietraject.

Minke en Ella – de apotheek is inmiddels een soort tweede thuis voor me. Dank voor de fijne samenwerking en ik hoop dat we nog veel samen blijven werken om de medicatieveiligheid in het AMC verder te vergroten.

Collega’s en oud collega’s bij KPI - de afgelopen 7 jaar heb ik met veel plezier met jullie samengewerkt. Alle BYO-lunches, borrels, taartmo-menten en het uitje naar Artis waren een welkome en gezellige afleiding. Delphine, Patty (dank voor het nalezen!), Sjaak, Linda, Jolanda, Aleida, Ferry, Stephan, Astrid, Niki, Hanna, Lieke, Jan en ook alle oud collega’s; dank voor de samenwerking en gezelligheid.

Collega’s bij BKV en kernteam JCI –JCI perikelen, ‘mock surveys’ en de echte accreditatie, we hebben samen heel wat meegemaakt als team. Tamara, Ingrid, Erik, Jeanine, Alexander, Margreeth, Agnès en Henk; dank voor de gezelligheid en vele leuke momenten samen.

Jolanda – KPI collega en vriendin, van een rondje buiten tijdens de lunch tot een wandeling naar een Waddeneiland, het is altijd gezellig! Natuurlijk ook dank voor het maken van de foto in dit proefschrift en de ongetwijfeld mooie foto’s die je nog gaat maken op de 14e.

Mijn paranimfen – Nicolette, na een gezellige studietijd samen, was jouw promotie de eerste die ik meemaakte en ook nog als je paranimf. Ik vind het dan ook het erg leuk dat je vandaag naast mij staat en kijk al uit naar je inauguratie binnenkort. Jolanda, onze promotietrajecten lopen bijna gelijk en daarbij delen we onze passie voor medicatieveiligheid. Het was voor mij niet meer dan logisch dat je vandaag naast mij staat.

APPENDICE - Acknowledgements / Dankwoord

Het combineren van promotieonderzoek, werk en een gezin is niet altijd eenvoudig. Lieve familie en vrienden dank voor alle steun, (kinder)opvang, gezelligheid en soms broodnodige afleiding. Ik hoop op nog veel mooie en leuke momenten samen!

Mijn ouders en zus Ineke – dank voor alles. Lieve pa, helaas ben je niet meer bij ons, maar ik weet zeker dat je trots was geweest. Lieve ma, ik hoop nog vaak bij je te komen voor een kopje koffie of thee. Ik ben blij dat Ineke en jij deze bijzondere dag samen met mij meemaken.
