A B O U T T H E A U T H O R

Emma Birnie was born in Amsterdam on 24th November 1988. After secondary school at the Stedelijk Gymnasium Leiden, she travelled to South America, ending up as crew member on asailing yacht to the Galapagos and Tahiti, French Polynesia. During her high school and post-secondary studies, Emma has been always very active in soccer and played at national level. However, she always maintained a passion for becoming a doctor and so enrolled for the study of Medicine at the University of Amsterdam (UvA) in 2008. Infectious diseases have continually played a significant role in her medical education, reflected by the subjects and locations of her studies (South Africa, Gabon, Sierra Leone, Thailand, Ethiopia). During her internship from 2012 to 2013, she discovered a novel Burkholderia pseudomallei sequence type and identified the first melioidosis patient in Gabon, Central Africa; this resulted in multiple awards and grants inclu-ding the Hippocrates Prize and the Snapper Prize. In 2015 she obtained her Honours Masters Degree in Medicine cum laude and started working as a resident at the Internal Department at Ter Gooi Hospital in Hilversum.

Emma was awarded an AMC PhD Scholarship and started her PhD project at the Center for Experimental Molecular Medicine & Center for Tropical and Travel Medicine (Supervisors Prof. Dr. W. Joost Wiersinga and Prof. Dr. Martin P. Grobusch). From 2015 to 2019, she studied the epidemiology and pathophysiology of melioidosis with a special interest in sub-Saharan Africa. Her work on the global burden of melioidosis was recently accepted for publication in the Lancet Infect Dis. She was one of the key initiators and member of the organizing committee for the First WHO African Melioidosis Workshop in Lagos, Nigeria, March 2019. During this workshop, she launched a pilot study ‘In search for melioidosis in Nigeria’, for which she was awarded a research grant from the European Society of Clinical Microbiology and Infectious Diseases (ESCMID). Her interest in other neglected tropical diseases led to a community research project on podoco-niosis in Ethiopia and the start of the foundation Help Tropical Health. Fascinated by the tropics, she will start the Diploma in Tropical Medicine and Hygiene post-graduate course in Liverpool, UK, from September 2019. She lives with her beloved wife Lianne Cremers in Amsterdam.

Dit proefschrift had nooit tot stand kunnen komen zonder jullie hulp. Jullie hebben belangrijke bijdrages geleverd; ieder op zijn eigen manier. Jullie hebben ervoor gezorgd dat ik mijn PhD heb kunnen afronden!

Om te beginnen wil ik mijn promotor en mentor Joost Wiersinga bedanken. Joost, ik leerde je zeven jaar geleden kennen. Ik was nog geneeskunde student en ik zocht een wetenschappelijk stage. Je bleek nog wel een leuk project te hebben zei je; ik mocht voor een spoedcursus naar Thailand en daarna naar Gabon. Dit was mijn kennismaking met het melioidose onderzoek en met Afrika. Jouw onnavolgbare positieve inslag heeft ervoor gezorgd dat ik vertrouwen bleef houden in onderzoek, ondanks de bijbehorende tegenslagen. Je leerde me focussen op de juiste momenten. Ik werd in het diepe gegooid en moest gewoon doen. Wel moest ik leren zorgvuldig te blijven in de presentatie (zowel op papier als in persoon). Ik had nooit gedacht dat ik uiteinde-lijk zoveel plezier zou beleven aan onderzoek doen. Je hebt me heel veel ruimte gegeven en altijd wilde je meedenken over nieuwe mogelijkheden. Het was een eer voor mij om jouw ceremonie-meester te zijn bij jouw oratie, wat een bijzondere dag! De vele tussenbiertjes samen met jou in Gabon, Thailand, Filipijnen, Sri Lanka en Nigeria waren onmisbaar, omdat ik je betrokkenheid voelde. Hopelijk is dit pas het begin van onze samenwerking!

Mijn promotor, Martin Grobusch, was een onvoorwaardelijke steun tijdens mijn PhD. Martin, meerdere keren ben je in Gabon langsgekomen om ervoor te zorgen dat ik het melioidose pro-ject eindelijk kon opstarten en je zorgde ervoor dat ons niets ontbrak! Door jouw toedoen zijn er drie dingen gestart waarvan ik nooit had durven dromen; een PhD, mijn passie voor Afrika, maar bovenal heb je mij 'gekoepeld' aan de liefde van mijn leven.

Tom van de Poll: op afstand heb je mijn PhD meegemaakt en je was niet te beroerd om soms scherp commentaar te geven. Grote waardering heb ik voor je hoe je de Tommies begeleidt en meteen tot de kern kunt komen.

Jasmin en Andy, beiden hebben jullie mij veel geholpen. Jasmin met haar lijstjes tot het bijstaan in de DM3 en Andy programmeerde, kookte en bouwde ook nog even een website tussendoor. Samen maakten we deel uit van de feestlunch met Mariekje, Dorith, Michelle en Lianne.

Michelle, we vulden elkaar uitstekend aan tijdens de voorbereidingen van de oratie van Joost en Joppe! Hessel, samen sporten, fietsen en jouw gezelligheid waren een welkome aanvulling in de laatste fase van mijn PhD! Ik wacht nog steeds op ons tafeltennistoernooi. Joe and Fabrice, I could always ask you questions about epidemiology. Lonneke, je kwam terug voor een Post-doc, wat een stimulans! Jos, jouw tips over bacteriën waren onmisbaar! Tassili, wat wist je veel melioidose projecten op te starten! Jacqueline, jouw eindeloze geduld tijdens het inwerken van mij in het lab zorgde ervoor dat ik niet helemaal in het diepe werd ge-gooid. Joosties; Katja, Tassili, Jacqueline, Duncan, Vanessa, Bas, Harjeet, Floor, Max, Nora, Berke, Tjitske, Alex, Rosan en Angelique jullie tips tijdens onze wekelijkse meetings waren altijd zeer welkom. Bas, we begonnen ongeveer tegelijkertijd aan onze promotie bij Joost en vaak kwam je met slim advies en nieuwe mogelijkheden!

Tommies, dank voor jullie tips in het lab en jullie feedback tijdens onze wekelijkse meeting. Maar bovenal waren de gezamenlijke uitjes en congressen een feestje. Hi Ha tequila!

Joris, je maakte het scoren van coupes tot een hilarische bezigheid! 'Geniet van het moment! ' De dames van de elektronenmicroscopie, Nicole, Anita en Daisy bedankt voor al de mooie plaatjes die jullie gemaakt hebben van mijn lievelingsbacterie Burkholderia pseudomallei. Arie, jouw en-thousiasme over B. pseudomallei bacteriestammen werkt aanstekelijk!

Juanita en Brecht, de redders in nood. Jullie hebben me de wonderen geleerd van de DALY. Zon-der jullie hulp, kennis, inzet en enthousiasme hadden we nooit de DALY voor melioidose kunnen berekenen. Al het harde werken heeft zich uiteindelijk uitbetaald in een hele mooie publicatie! Hopelijk kunnen we nog meer samen werken in de toekomst.

Daan en Frans, ons Nederlandse melioidose project hebben we in een sneltreinvaart af kunnen ronden, zonder jullie en de inzet van de Dutch Melioidosis Study Group was dit nooit gelukt.

Het tropencentrum: Martin, Bram, Kees, Pieter en Michèle, dank voor de wonderen wereld van de tropen. Michèle, vanaf het begin af aan had je vertrouwen in ons Mossy Foot project. Podoco-niosis: de start van een nieuw avontuur!

Jelle, helaas ben ik jou niet vergeten te bedanken. Jouw creatief vermogen om van alles een groots avontuur te maken heeft mij ontzettend geïnspireerd tijdens mijn PhD. Wat bezit jij veel slimmigheidjes! Ons project en de reis in Ethiopië (samen met Jasmyn en Lianne), ons crowd-funding event, en ook nog onze gezamenlijk stichting 'Help Tropical Health' is het begin van iets moois.

Senne, doorzetter, een half jaar heb je geploeterd op qPCRs en gaf je nooit op! Floor, bedankt voor onze gezamenlijke begeleiding van Senne. Dorien, ons avontuur naar Ghana verliep niet helemaal zoals gepland, maar we hebben het toch wel erg naar ons zin gehad, wat een flexibili-teit. Abe, wat een prachtige figuren maak jij, ontzettend bedankt voor de mooie illustraties voor mijn proefschrift.

Jelmer, alle projecten deed je met volle overgave en efficiëntie naast je coschappen; dagenlang artikelen screenen voor ons DALY-project en hierna ook nog 100% inzet voor onze Nederlandse melioidose studie. Het was een waar genoegen om samen te werken, hopelijk blijven we dat in de toekomst doen!

Jurgen, wat een wonder dat jij niet gek geworden bent van al mijn micro-management over de lay-out van dit boekje. Maar wat een ontzettend mooi resultaat!Jill, alle creaties voor de hoogtepunten uit ons leven kwamen door jouw inzet! En de kleur die je dit boekje hebt weten te geven, te gek! Liefste vrienden, deze paragraaf zou eindeloos lang moeten worden. Jullie geplaag, reflectiever-mogen, humor, liefde, oprechte interesse en eindeloze steun in alles wat ik doe hebben ervoor gezorgd dat ik ben wie ik ben. Waar ik ook heen ging en hoeveel plezier ik ook had, jullie zorgden ervoor dat ik altijd terug wilde naar Nederland. De etentjes, feestjes, festivals, weekendjes weg, sporten, vakanties (van surfen tot wintersport) en de eindeloze hoopjes zorgen ervoor dat mijn leven compleet is. Harjiti, I remember the first time when we had a conversation trough Skype. You were interested in doing research on melioidosis, how marvelous! I am really grateful that you joined our group and that you became my all-time melioidosis buddy and friend. Your creative mind and critical (English) reflections really improved my work. Finally, our hard work resulted in our beautiful publication in the Lancet Infect Dis. Hopefully, together, we can put melioidosis on the WHO agenda as a neglected tropical disease.

Dorith, paranimf, zonder jou had ik mijn PhD waarschijnlijk nooit afgerond, of in ieder geval niet binnen de tijd die ervoor stond. Alle levenslessen leerde ik van jou. Gelukkig heb ik jou ook weten te vermaken met het bekende eindeloze lijstje van mijn blunders. Wonder boven wonder, eindigde ons gezamenlijke project toch in een mooie publicatie. Het eeuwige chocola eten en koffiedrinken hield op toen je naar Eindhoven/Maastricht vertrok, voor mij was dit misschien wel het dieptepunt uit mijn PhD historie. Maar ik weet 1 ding heel zeker; je bent nog lang niet van mij af!

De Familie Cremers; Diana, Pieter dank voor jullie oprechte interesse en steun. Jullie waren het die Lianne opvingen toen ik zo ver weg in Nigeria zat. Liefste Lianne, mijn vrouw, levenspartner en paranimf, dit boekje begon en eindigt met jou. Ik ontmoette jou in Gabon tijdens mijn zoektocht. Je geeft mijn leven meer kleur en bij jou vind ik rust, al denkt de hele wereld soms dat wij nooit rust hebben. Ik ken niemand die zo veel oprechte interesse heeft in zijn medemens en zo positief in het leven staat als jij. Als jij erbij bent is alles nog intenser en nog fijner en dan het liefst ook nog in Afrika. Liefste, durfal, ik bewonder je, je bent mijn alles.
