Using iCareplans.com for the care planning process.The iCareplans.com system has the ability to create care plans either in traditional format or in person centered format. Users can select the menu option on the left to enter the desired care planning module.
The system is designed under the premise that care planning should be addressed both from a logistic and holistic perspective. Logistically, the care plan is organized into problems, goals, and approaches as a tool to manage services provided by care-givers. Information from this format should be used to drive care-giver assignment and documentation.
The person-centered care plan allows the user to create the document in a language that everyone understands. It should focus on the resident strengths and be written from the resident’s perspective

The Traditional Care Plan.
The system allows for an unlimited number of problem/need statements per resident. Each problem statement can have an unlimited amount of text.
Each problem/need statement can have an unlimited number of goals and approaches, each with an unlimited amount of text.
The Traditional Care Plan.
Create a problem/need entry by free-typing, or selecting one of several hundred statements from the master library. If a statement is selected from the master library, it may be edited to individualize for the specific resident

Goals and approaches can also be created by free-typing or selecting statements from the master library.
Each approach can be linked to multiple disciplines.
The Person-Centered Care plan.
This module allows the creation of narrative care plan entries into each of several care categories
