Online reporting of Action Points under Health Services Monitoring System:
For every action taken against the “Action Points”, the information is proposed to be captured
through an online reporting system under “Health Services Monitoring System”.
The objective is to get the instant feedback on the action taken and to reduce effort for sending
information through email and to compile.
Supporting documents related to the action taken is also proposed to be captured.
How to access the online system:
URL: http://www.nrhmassam.info/HSMS/
� User id and password provided for “Integrated MIS GIS System” will also be
applicable for Health Services Monitoring System.
� Login using User id of Principal Secretary/ Deputy Commissioner will be
allowed to upload the online reporting.
� Privilege has also been provided to login user id of District Programme
Manager to upload the online reporting.

Step 1: Use URL http://www.nrhmassam.info/HSMS/login.php
Following screen will appear.
Login using the user id and password provided.
Step 2: Following screen will appear after we click on the “Login” button.

Step 3: Select the menu option “Activity Management”. Following menu option will appear
Step 4: To entry “Action Taken” information, click on the “Action Taken Entry Form”. Following screen will
appear.

Step 5: Fill up action taken for every action point.
Step 6: To upload data, browse the file and upload. We can upload PDF file, Word Document, Excel File,
Image (.jpg, .jpeg, .png), etc.

Step 7: To view “Report of the District, click on the “Report” option.
Action point wise detail report will appear as shown below:
To view a document/ photograph, click on the icon.
