User Guidance on Online Reporting

System of “National Programme on

Prevention & Control of Cancer,

Diabetes, CVDs & Stroke (NPCDCS)”

Integrated MIS-GIS System (www.nrhmassam.info) NHM, Assam

Login into the System- Integrated MIS-GIS System ( www.nrhmassam.info ) using the Username and

password provided. Only authorized users have access to the system.

After login into Integrated MIS-GIS System Click on the MIS Menu

Go to MIS->NPCDCS->

There are 3 submenus under NPCDCS

1. MIS->NPCDCS->Reporting Entry Form: To enter the monthly data relating to NPCDCS

2. MIS->NPCDCS->Edit Form: To modify/correct/update the already entered data

3. MIS->NPCDCS->Reports: Various reports are available based on the uploaded data

1. Sub Centre Entry form :To enter the NPCDCS data of Sub Centre go to

MIS->NPCDCS->Reporting Entry Form->Sub Centre (Form 1)

Select District, Health Block, Sub centre from the dropdown list under which you want to enter the data

of the Villages. Select financial year and month for which you want to enter the data then click on the

Go button. After clicking on Go button; it will display the name of the villages already existing in our

system under the selected Sub Centre. So before entering the name of Village, please ensure that village

name is not available in the Select Village list.

It also displays the already uploaded data under the selected Sub Centre of the selected financial year

and month. So before entering the data have a view on the already uploaded data as shown in the next

screenshot

To enter the data, select Village from the dropdown list box for which you want to enter the data under

the selected Sub centre. If the village name for which you want to enter the data under the selected Sub

Centre is not available then only enter the name of the village and enter the required data .Then enter

Date of Reporting, Report signed by (Name), Report signed by (Designation) and click on the Save button

to save the entered data. You will get a confirmation as “Record added successfully”.

Note: If Village name is not selected or Village name is not typed, data will not be saved for that

particular row.

At one time you may enter up to five (5) villages’ data under the selected sub centre. If you want to

enter more than 5 Villages data under the selected Sub Centre, first enter 5 villages’ data and then enter

the rest data.

Displaying already uploaded data of selected financial year and month under the selected Sub centre



2. Primary Health Centre Entry Form: To enter the NPCDCS data of Primary Health Centre go to

MIS->NPCDCS->Reporting Entry Form->Primary Health Centre (Form 2)

Select District, Health Block, Financial year and month for which you want to enter the data and enter

the relevant data and click on the Save button to save the data. You will get a confirmation message as

“Record added successfully”

3. NCD Clinic at CHC entry Form: To enter the NCD Clinic data at CHC go to



MIS->NPCDCS->Reporting Entry Form->NCD Clinic at CHC (Form 3)

Select District,Health Block,CHC , finacial year and month for which you want to enter the data .Then

click on the Save button to save the data. You will get a confirmation message as “Record added

successfully”

4. NCD Clinic at District Hospital entry Form: To enter the NCD Clinic data at District Hospital go to MIS-

>NPCDCS->Reporting Entry Form->NCD Clinic at DH (Form 4)

Select District, , finacial year and month for which you want to enter the data .Then click on the Save

button to save the data. You will get a confirmation message as “Record added successfully”

Edit Form:

1. Sub Centre Edit Form: To modify, edit or correct the uploaded data of Sub Centre go to



MIS->NPCDCS->Edit Form->Sub Centre (Form 1)

Select District, Health Block, Sub centre, financial year and month of which you want to edit the data and click on

the Go button. This will display all the entered data under the selected sub centre, financial year and month.

To modify the particular data click on Edit link. On clicking the Edit link, it will display the data in the text

boxes. Edit the as required and click on the Update button to save the update. You will get a

confirmation message as “Record updated successfully”.

2. Primary Health Centre Edit Form: To modify the uploaded data of Primary Health Centre go to



MIS->NPCDCS->Edit Form->Primary Health Centre (Form 2)

Select District, Health Block, financial year and month of which you want to modify the already uploaded data.

Then click on Go button, It will display the uploaded data, edit the data which you want to change and click on the

Update button to save the changes. You will get a confirmation message as “Record updated successfully”.

3. NCD Clinic at CHC Edit Form: To modify the uploaded data of NCD Clinic at CHC go to

MIS->NPCDCS->Edit Form->NCD Clinic at CHC (Form 3)

Select District, Health Block, CHC, financial year and month of which you want to modify the already uploaded

data. Then click on Go button, It will display the uploaded data, edit the data which you want to change and click

on the Update button to save the changes. You will get a confirmation message as “Record updated successfully”.

4. NCD Clinic at DH Edit Form: To modify the uploaded data of NCD Clinic at DH go to



MIS->NPCDCS->Edit Form->NCD Clinic at DH (Form 4)

Select District, District Hospital, financial year and month of which you want to modify the already

uploaded data. Then click on Go button, It will display the uploaded data, edit the data which you want

to change and click on the Update button to save the changes. You will get a confirmation message as

“Record updated successfully”.

Reports



Sub Centre Reports: To view Sub Centre reports go to MIS->NPCDCS->Reports->Sub Centre (Form 1)

MIS->NPCDCS->Reports->Sub Centre (Form 1) ->Village Wise Details: This report displays details along

with the village name under the selected Health Block or Sub Centre

You may save this report in excel format by Clicking on Export to Excel

MIS->NPCDCS->Reports->Sub Centre (Form 1) -> Sub Centre wise Summary: This report displays sub

centre wise details under the selected district or Health Block



MIS->NPCDCS->Reports->Sub Centre (Form 1) -> District wise Details: This report displays everything in

details under the selected District along with the name of village, health block, district and the uploaded

details

Report of Primary Health Centre: To view Primary Health Centre Report go to

MIS->NPCDCS->Reports->Primary Health Centre (Form 2)

Select District, Health Block, Financial year and month then click on Go button. This will display the data of the

selected District, selected Health Block. If you select only District then it will display all the data of the selected

District of the selected financial year and month. If you select neither Health Block nor district then it will display

all entered data of Assam of the selected financial year and month

Report of Community Health Centre: Go to MIS->NPCDCS->Reports->NCD Clinic at CHC (Form 3)



Select District, Health Block, CHC, Financial year and month then click on Go button. This will display the data of

the selected District, selected Health Block. If you select only District then it will display all the data of the selected

District of the selected financial year and month. If you select District, Health Block then it will display all the data

of the selected Health Block of the selected financial year and month .If you select neither Health Block nor district

then it will display all entered data of Assam of the selected financial year and month.

Report of NCD Clinic at District Hospital: Go to MIS->NPCDCS->Reports->NCD Clinic at DH (Form 4)

Select District, District Hospital, Financial year and month then click on Go button. This will display the data of the

selected District, selected District Hospital. If you select only District then it will display all the data of the selected

District of the selected financial year and month. If you select neither District nor District Hospital then it will

display all entered data of Assam of the selected financial year and month.

Report on Form 5A: This is a compiled report of NCD Clinic at CHC (Form 3) and District Hospital(Form 4)



Go to MIS->NPCDCS->Reports->District NCD Cell (Form 5A)

Select District, financial year and month and click on Go button to view the report

Report on Form 5B: This is a compiled report of Sub Centre (Form 1) and Primary Health Centre (Form 2)

Go to MIS->NPCDCS->Reports->District NCD Cell (Form 5B)

Select District, financial year and month and click on Go button to view the report.
