Latino health care, health care disparities, minority health, tobacco, smoking cessation, cancer screening, minority aging, language access, patient-physician communication, Latin America PROFESSIONAL ACTIVITIES: CLINICAL I have had a continuity practice of general internal medicine at UCSF since 1978. I still follow the first patient I ever saw as an intern in clinic. My practice panel consists of about 200 patients and includes many faculty and staff at UCSF although most patients have multiple chronic diseases and about half are Latino with limited English proficiency. I work as a ward attending on the Medical Service at UCSF for 2 to 4 weeks per year. Since 1981, I have attended a total of 27 months on the medical service at UCSF Medical Center or San Francisco General Hospital. I have attended on the General Medicine Consult Service for a total of about 10 months through 2006. LEADERSHIP Director, Primary Care Internal Medicine Residency Track, UCSF/Parnassus. Coordination of educational program,

evaluation and mentoring of 18 residents per year, director of committee in charge of selecting annual cohort of residents, and all administrative management of 18 internal medicine residents in the primary care track. Maintained close coordination with the Department of Medicine, 1985-1989.

Director of Research, Division of General Internal Medicine. Schedule and coordinate bimonthly research

conferences, biannual research symposium, annual research retreat, annual submission of abstracts to SGIM, practice presentations of accepted abstracts, and lead reports at DGIM retreat, 1991-1999.

Chief, Division of General Internal Medicine, 1999 to present I am responsible for all operations of the largest division

in the Department of Medicine in terms of human resources and the second largest in clinical revenue. About 55 faculty persons are based in DGIM including 11 research non-physicians, approximately 70 staff work in the academic unit and in the clinical practices and the total operating budget is about $20 million annually. DGIM faculty produced about 192 peer-reviewed publications in 2009 and generates about $6 million in direct cost of research grants from the NIH, CDC, private foundations, industry and State agencies. The teaching program includes continuity general medicine for 30 primary care track residents and 45 categorical track residents who spend about 110 practice sessions with DGIM. In addition, we have 2 general internal medicine research or clinician educator fellows and 3 research post-doctoral trainees per year. Approximately 65 third and fourth year medical students rotate through DGIM on the ambulatory services annually and nurse practitioner and pharmacy students participate in selected programs. There are about 80,000 annual patient visits to the three continuity practices and the urgent care clinic.

Director, Medical Effectiveness Research Center for Diverse Populations (MERC). Founded in 1993 as a center to

address minority health, MERC includes 13 faculty persons from seven disciplines and occupies about 6000 square feet of space on the UCSF Laurel Heights campus. Administrative and governing structures have been established and monthly research seminars attract 15 to 20 participants.

Associate Director, Health Care Disparities, Helen Diller Family Comprehensive Cancer Center (CCC). Appointed

in July 2006 to participate in CCC leadership to address all aspects of health care disparities, recruitment of diverse participants to research studies and development of minority investigators.

I attended 3 months on the medical service at San Francisco General Hospital during my general internal medicine fellowship and subsequently one month per year at Moffitt-Long until 1999. Since 1999 when I became chief of DGIM I have attended for 15 days per year. Wards attending service involves daily management of 10 to 15 acutely ill patients with a team of 3 residents and 2 medical students. I see very patient every day and teach about practical management issues, physical examination and patient doctor communication. There are about 6 hours of more formal didactic sessions per week.

Discussant, General Internal Medicine Journal Club, 1983 – 2004. I was the previous director of journal club and an

active participant in reviewing two selected articles per month. Discussant and Coordinator of Ambulatory Care, Morbidity and Mortality Conference, Department of medicine, 2009 to

present; about 35 residents participate in case presentation and discussion with an additional 2 specialty discussants invited to comment on aspects of the case.

health disparities research methods elective offered to the campus community through the Advanced Training in Clinical Research Program directed by the Department of Epidemiology and Biostatistics (Jeff Martin and Stephen Hulley). I direct the course and directly teach a 50% of the classes. Offered yearly and about 15 to 20 students take the course.

Director, CADC and MERC Training, 1993-present; I have directed all training and mentoring activities at MERC and

CADC since their inception. This has involved working with fellows, students, residents and faculty. MERC has the goal to promote minority investigators in the area of health related research and we have worked with trainees form multiple disciplines and ethnic groups. Regular activities include a monthly seminar of work in progress presented on topics relevant to health disparities and minority health. Since 1997 we have had monthly CADC seminars for fellows and faculty funded through the CADC pilot grant program and included three minority supplements to the main grant. Physicians of multiple specialties, nurses, population scientists, psychologists, and sociologists have been part of this training program.

Chief, DGIM 1999 to present; as chief of the division, I meet with and mentor all faculty at least once per year. Although

there is a special emphasis on all assistant professors, I meet all faculty at all levels and provide feedback and career advice at all levels. I mentor individual faculty on their faculty development awards and frequently read and edit manuscripts prior to first submission.

Sabbatical Leave in Argentina during 1992-1993 and 2004-2005. I taught residents and fellows in the general internal

medicine program at the main university hospital in Buenos Aires during both sabbaticals. The program was directed by Raul Mejia, MD and has been active since 1989. I will also give periodic lectures, participate in research seminars and lectured at the annual meeting of the Sociedad Argentina de Medicina Interna General.

TEACHING NARRATIVE

I am preceptor for residents in the General Medicine Practices at Parnassus one session per week. Each session lasts about 4-5 hours and usually has 4 to 10 residents seeing their own patients and each patient is presented and discussed with a faculty preceptor. Each session is preceded by a 30-minute didactic session and I teach several each year. I teach on the inpatient medical service for two weeks per year. This requires about 3 to 4 hours of informal bedside teaching per day including preparation time. I work closely with residents to manage and coordinate patient care, teach students and communicate with consultants and outside clinicians. I direct a 10-week course for the clinical research-training program that is held annually in the spring. I give invited lectures in multiple settings within UCSF on several clinical topics and on health care disparities issues. Finally, I directly mentor 55 DGIM faculty and meet with them at least annually and about 10 additional fellows and faculty outside the DGIM through my role at MERC.

The major goal of this center is to foster the development of diverse investigators who will conduct research with older ethnic/racial minority persons, develop and implement strategies to diversify the investigator workforce conducting research on the health of older minority persons, develop and implement strategies for recruiting and retaining minority group members in research dealing with the health of minority elderly through community-based network, and conduct methodological studies on measures commonly used in older minorities. 9R01DA024877-06 (Pérez-Stable, PI) 09/30/07-08/31/2012 NIH/National Institute on Drug Abuse/Fogarty International Center Tobacco Control Research and Training in South America. The overall purpose is to develop and implement two tobacco control interventions in Argentina. The first is a community0-based intervention through schools to prevent smoking initiation and promote cessation among Argentinean adolescents in the Northwestern provinces of Tucuman and Jujuy. The second project will evaluate the efficacy of a randomized intervention with physicians to teach them how to help their patients quit smoking with the goal of increasing quit attempts and cessation among their patients at 12 months.
