ULTAIRE™ AKP: TREATABLE CASES

What is Ultaire™ AKP?Ultaire™ AKP is an innovative, high-performance polymer for removable partial dentures (RPDs) that provides a comfortable, lightweight, biocompatible and aesthetically pleasing alternative to traditional metal frames.

Ultaire™ AKP RPDs have similar mechanical properties to corticle bone, are tooth- and tissue- supported and may help minimize bone loss. They are digitally designed and milled from a Dentivera™ milling disc.

When can it be used? Removable partial dentures made from Ultaire™ AKP are ideally suited to Kennedy Classifications 1 and 3.

Stability and retention are optimized when the distal extension has tooth-borne characteristics.

Using Ultaire™ AKP for Classes 2 and 4 should be left to the discretion of the clinician.

IMPRESSIONS A traditional VPS (Vinyl Polysiloxane) Impression Material is recommended. Consult your approved Ultaire™ AKP lab to determine if a custom tray is necessary for each case.

If you currently use an intraoral scanner for digital impressions, consult your lab to determine if it is possible to submit the case digitally.

AESTHETIC CONSIDERATION One of the many advantages of Ultaire™ AKP is the ability to stain the clasps for improved aesthetic appearance. Stained clasps blend in with the abutment teeth so they are less noticeable than metal clasps. When you submit the case to your lab, simply indicate a VITA® classic shade for the abutment tooth and they will match the clasp(s) as closely as possible. It is recommended to take the shade of the tooth closest to the gingival margin. Please note: Ultaire™ AKP is a high-performance polymer and can be stained, but does not exhibit the same characteristics as ceramic or other materials as related to color.

CHECKING THE FIT (Frame try-in and final delivery)

FRAME Once you have received the frame from the lab, it will be ready for the patient to try-in. Due to the digital CAD/CAM manufacturing of the Ultaire™ AKP frame, minimal adjustments should be necessary, but if needed, please follow instructions below.

• During the try-in phase, if any discrepancies are noted, coat the guide planes, saddle, and any other areas which may require adjustment with a small amount of occlusal spray or indicating paste.

• Dry the patient’s mouth using cotton and/or air.

• Carefully place the RPD into the patient’s mouth and instruct the patient to close. Ask the patient if there are any sore spots.

• Gently remove the partial and determine where markings from the occlusal spray or indicating paste transferred to the RPD.

! Before making adjustments to the prosthesis, it is recommended to shave a small amount of material from a sample card. If you do not have a sample card of Ultaire™ AKP please request one from your lab. Use a tapered, cross-cut carbide bur on low speed to shave with small shaving strokes where markings were transferred on the frame. Continue to shave and smooth until the fit is optimal.

Removing too much material will weaken the Ultaire™ AKP RPD and could lead to early failure. Caution should be used when removing excess material.

How to prepare teeth for Ultaire™ AKPWhen prescribing Ultaire™ AKP for an RPD framework, treat the case in a similar fashion as you would for a metal RPD frame. Due to the properties of the material and the ability of the material to be tooth-borne, occlusal preps should be completed prior to impressions.

To provide stable vertical support for the RPD, the preparation for the occlusal rest must be at least 1.0 mm wide, ideally half of an #8 round bur 2.30 mm, and at least 2.0 mm wide to allow for the correct material thickness in Ultaire™ AKP.

Continue to treat the case as a conventional metal partial and make an impression.

Make sure the marginal ridge is removed from the rest seat and there is a gradual radius (waterfall) in this area. This will allow for optimal design of the Ultaire™ AKP rest and correct material thickness.

SPEECH AND OCCLUSION • Check speech by having the patient repeat their syllables.

If speech is not optimal, make modifications to the acrylic denture base by re-contouring with a standard carbide bur. Use a fine rubber point and a goat hair or microfiber wheel to prevent plaque accumulation to a rough surface. Do not adjust the Ultaire™ AKP frame.

• Occlusal problems can be detected using occlusal paper or spray to obtain markings. Adjust denture teeth that show markings for optimal occlusion and function.

POLISHING To polish, use an acrylic, fine rubber point then use an acrylic polishing paste with a goats hair or microfiber wheel. A smooth surface will prevent plaque accumulation.

! Do not use a rag wheel on a lathe.

OCCLUSAL RESTS To ensure that the rests are seated correctly, ask the patient to bite down on occlusal paper and do lateral and protrusive excursions with their mouth. Determine if the rest has interference with the occlusion by looking at the markings. Adjust where necessary with a cross-cut carbide bur or fine rubber point on low speed with small shaving strokes where markings were transferred on the frame. Continue to shave and smooth until the fit is optimal.

Occlusal rests must be at least 1.0 mm thick for strength and durability.

CLASP MODIFICATION • Unlike a cobalt chrome frame, Ultaire™ AKP will not relax

or loosen over time. If a clasp is too tight or putting undue pressure on an abutment tooth, carefully remove the frame from the patient’s mouth.

• Apply occlusal spray or indicating paste on the inside of the clasp.

• Return the frame to the patient’s mouth. Ask the patient to close their mouth gently.

• Remove the frame to determine any markings and modify as needed with a tapered cross-cut carbide bur on low speed with small shaving strokes. It is recommended to only make a few strokes before testing the prosthesis again.

Use caution when modifying a clasp as they cannot be tightened. Removing too much material may result in a loss of retention and or compromise the strength of the clasp and could result in the frame needing to be remade.

! Adjusting clasps with a pliers is not recommended.

CLEANING AND CARE• It is recommended that patients follow standard RPD

protocol for care and cleaning of their Ultaire™ AKP partial dentures.

• Solvay Dental 360™ recommends using a soft brush, denture cleaner, regular toothpaste, and storing Ultaire™ AKP partial dentures in fresh tap water when not in use.

• Keep out of reach of children and pets.

• Do not bend or flex.

CLASP REPAIR It is possible to repair a clasp on an Ultaire™ AKP RPD.

• Ultaire™ AKP does not behave like metal or acetyl. The material balances rigidity and toughness, making it more resilient than metal, but more rigid than acetyl.

• If it is necessary to repair a clasp, follow the standard repair procedure of taking a pickup impression of the partial in the patient’s mouth and forwarding the partial in the impression to the lab.

ADDING TEETH It is possible to add a tooth to an existing Ultaire™ AKP RPD as long as there is sufficient lingual or buccal plating in the edentulous space.

This procedure should be conducted by your dental lab. Take a pickup impression of the RPD and send it to your lab for processing. (See Figures 1-4)

Design ConsiderationsAll Ultaire™ AKP qualified labs have gone through a comprehensive training on how to optimize an RPD design to utilize the unique material properties of Ultaire™ AKP. Your lab can provide you with digital images of your case design for review.

Labs will use specific design techniques to create an RPD with retentive clasps. They will not design a double akers, Y/I/T Bar or clasps that will go over the embrasures due to the material properties and minimal thickness. If you have any questions, please request design images and discuss them with your lab.

FRAME

The Ultaire™ AKP frame will not be similar in size to a cobalt chrome frame, because of the material properties of Ultaire™ AKP.

The clasps will be shorter and thicker, and the plate will be a little bulkier than a cobalt chrome but not as thick as an acrylic.

Clasps are shorter because the material has the ability to engage up to 0.5 mm of undercut as opposed to 0.3 mm advised for cobalt chrome. This is due to the slight flex in the material. Having a shorter clasp can be very aesthetically beneficial.

LINGUAL BAR

It is also important to note that when prescribing a lower frame, there is a minimum height from the lingual sulcus of 7.0 mm and a 3.0 mm clearance of the gingiva should be incorporated to improve the overall oral health of the patient. Therefore, if there is minimal space lingually, it is recommended to have a lingual plate.

This case has enough room for a lingual bar with sufficient clearance from the gingival margins. At least 3.0 mm is suggested, but left to the discretion of the clinician.

This case does not have enough space above the lingual bar, therefore a lingual plate is advised.

A lingual plate can be useful in the future if it is necessary to add more teeth to the RPD.

Since Ultaire™ AKP RPDs are designed using CAD software and milled to very tight tolerances, they will fit very closely to the model. Therefore, it is important that the impression or intra-oral scan be as accurate as possible to limit any potential adjustments. If the impression or scan is accurate, it is possible that the Ultaire™ AKP RPD will not need to be adjusted.
