Patient Chief Complaint: “My front tooth is turning dark and I have a dull aching pain now and then.”
Medical History:
White male with a BP 125/82 Pulse 66
Patient is taking the following medications: Aspirin 81mg 1x day and Lipitor 20 mg 1/day
Previous history of surgery for broken foot 1982
Patient allergies: Penicillin
Patient also wears glasses
Remaining medical history non-significant findings
Social History:
Patient is a social worker
No history drug use
Pt drinks a glass of wine every night
Family history of Diabetes
Patient was born in the United States
No other significant medical history
Dental History:
Patient came in for pain in the front tooth and darkness of the tooth
Patient has not been to the dentist for over five years
No history of extractions or trauma
Last radiographs about six years ago
Patient gums bleed slightly when brushing, and tissue is inflamed
Patient is overall satisfied with her teeth except for the front tooth and has no other significant dental findings
