Looking for day time involvement for your senior loved
one?Realizing that most seniors who live alone tend to stay at home
because of transportation challenges or half day programs; we have
created the answer to these issues:
We offer a full day program with transportation to Medicare
recipientsMonday-Friday 8:30-5:30Special events the 1st and 3rd
SaturdayTransitionsAt TRANSITIONS, we realize that many seniors
live alone or with a loved one who works during the day (thus
worrying about their senior loved one while they work); are they
eating? Is it too warm, too hot in the home? Did they fall? Are
they hurt or experiencing a medical trauma?...TRANSITIONS gives you
the peace of mind you need knowing that your loved ones are cared
for while you are working. There is no longer a need to leave them
home alone.TransitionsThis program is for Seniors who live alone or
with a loved one and their families. There are a myriad of day
programs albeit most are day and many do not provide
transportation. TRANSITIONS provides structured full day programs
that stimulate the cognitive process as well as engage our seniors.
