When late Prof. Rashid Chaudhry was in charge of Fountain House I had the opportunity to witness the progress achieved there, especially in response to the many disasters that have hit Pakistan in recent years. As a founder member of this world body in the mid eighties I have watched its progress with great interest. At a time when there is increasing evidence to suggest that both biological and social factors play an important part in the care of people with long term illnesses it is very appropriate that the organizers have selected to focus on recent developments in these particular areas. During the last two decades there had been significant developments in this area in the Asia Pacific region and this is another important reason why the planned conference would be of great significance.

It gives me great pleasure to visit Lahore to participate in the World Association of Psychosocial Rehabilitation Asian-Pacific conference 2013. Mental health rehabilitation is undergoing something of a renaissance after many years as the unfashionable arm of psychiatric services. Investment in new community based models of care had left those with the most complex and longer term mental health problems marginalised and institutionalised. Increasingly, professionals, service users, carers and those involved in planning services are recognising the importance of investment in high quality, whole systems of care to support people to achieve their potential. This means living as fulfilling a life as possible, as independently as possible, even when a person has multiple problems and their needs are great. These whole systems work across multiple boundaries and are often provided through collaborative and creative partnerships between statutory and non-statutory services. This is an exciting time to be a rehabilitation practitioner and I am very much looking forward to the meeting in Lahore which I am sure will provide a rich forum for discussion and sharing of ideas.

I’m very glad to be joining other zones and associations to wish for a very successful and unique event. By a quick view of the history and geography of this region, we find that it was constantly subjected to wars, revolutions, great political stress, Tsunamis, earthquake, flooding and other natural disasters. These elements result in rise of the number of mental illnesses and this fact strongly emphasises the importance of providing a better and more suitable psychiatric services and rehabilitation to people. Organizing events, such as this conference, greatly helps this important goal.The name of Fountain House will always remind us of Prof. Haroon Rashid Chaudhry, his father and what both of them accomplished for the psychiatry in the region. It gives me great pleasure to write a few words for the 3rd Asia Pacific conference on Psychosocial Rehabilitation which will be held 1st – 3rd November 2013 in Lahore, Pakistan.The congress theme “Recent Advances in Rehabilitation, Biological and Social Psychiatry” is most appropriate in view of the current situation around the world especially in Asia. The knowledge of both biological and social psychiatry is now increasing and it would be better to integrate rather than separate them for the benefit of the holistic approach in the treatment of various types of mental disorders. The need to provide rehabilitation and care for people going through emotional distress is also crucial in improving the mental health in our stressful societies. This need is increasing as our societies become more developed, sophisticated and competitive. Other challenges that we are facing are the rapid social changes that have come with our rapid economic growth in the Asia Pacific region.I hope that these challenges will be dealt with by the experts in their respective fields of biological psychiatry, social psychiatry and psychosocial rehabilitation. British Pakistani Psychiatrists Association is privileged and proud to be one of the co-sponsors of this meeting. Rehabilitation Psychiatry has come a long way over the last three decades, when the focus of treatment was illness and psychiatry and patients were confined to institutions in big asylums in cities. Patients and carers, including staff working in Mental Health Services, were stigmatised and marginalised, resources were scarce and research, non-existent. Professionals had low expectations from their patients and the families had lost hope of seeing their loved ones having a quality life, outside the shackles of drugs and Institutions.Over recent years, there has been a sea change in our understanding of mental health, human behaviour and our attitude towards mental illness. Professionals have worked hard in understanding and educating the society and reducing the stigma attached to mental illness. We have started looking at patients holistically, beyond illness, as individuals who have the capacity to cope with their illness and, at the same time, with the support of multi-professionals and carers, decide about their future and independence. To have a meaningful and satisfying life, as defined by the person himself or herself, whether or not there are on-going or recurring symptoms or problems.Rehabilitation, defined by the World Health Organization, as the application of measures aimed at reducing the impact of disabling and handicapping conditions and enabling disabled people to achieve social integration, is now progressing hand in hand with the Recovery modelwhich is a ‘personal process, way of life or attitude, involving the growth of new meaning and purpose beyond the effects of mental illness’. Recovery is about discovering – or re-discovering – a sense of personal identity, separate from illness or disability. WARP, since its inception in 1983, has been in the forefront of encouraging the sea change of attitudes, by disseminating the principles and practices of psychosocial rehabilitation, both in the developed and under developed world and on behalf of the British pakistaní Psychiatrist Association, I wish the president, Dr Afzal Javed and the conference organising committee, every success.

The challenge identified this year is being engaged with the local community while being aware of trends in Global Psychiatry, and in particular, the state of mental health in Pakistan. Over the last several decades, incidence and recognition of mental health problems in Pakistan has escalated. There is an increase in violence and violent crime, terrorism, and the insoluble effects of the War on Terror on societal behaviour. We are increasingly exposed to violence and its portrayal on the media, and the traumatized effects on families and children are palpable. We have to do our part for future generations, and we have to build resilience in the best ways possible.Building Resilience includes controlling TV and gaming time, internet addiction, exposure to online media and bringing in greater exposure to positive news, positive messages and solutions on the media. Overexposure to negative news events can harmfully impact sensitive children and adolescents. Global temperature change we know has been caused directly by human activity, leading to greater harm for our future generations. With your participation we will be looking forward learning from recommendations and possible implementation of projects that will be of relevance to Asian Psychiatry in this fast moving technological Age.

While we bask in the glory of biopsychiatry in the twenty first century, let us not forget the contributions of social psychiatry in the second half of the twentieth century. It was the works of likes of Karen Horney, Harry Stack Sullivan, Erik Erikson and Maxwell Jones that linked the human psyche with the cultural, anthropological, and societal context and consciousness. The huge influence of the therapeutic community on rehabilitation of the mentally ill particularly those suffering from mental and behavioural consequences of substance abuse, is a testimony of the relevance and impact of social psychiatry. In Pakistan, late Professor Rasheed Chaudhry was the pioneer in bringing social psychiatry to Pakistan in 1960s and 1970s by setting visionary projects for the rehabilitation of mentally ill in Lahore and Farooqabad, rooted in the principles of social psychiatry. The public mental health movement led by Professor Malik H Mubbashar in 10980s and 1990s added further impetus to highlighting the significance of community and family in psychiatry. College of Physicians & Surgeons, Pakistan’s fellowship and membership residency programmes in the field of Psychiatry has continued to emphasise the psychosocial aspects of health in general and of mental health in particular. The Faculty of Psychiatry, of CPSP has encouraged its trainees to reach out to excellent facilities at Fountain House Lahore and had approached the late Professor Haroon Rasheed Chaudhry, (the moving spirit behind this and many other social psychiatry projects nationally and internationally) to recognize it as a centre of excellence for training. His premature and tragic demise left a huge gap in this and many other visionary projects that he had initiated. The efforts of Dr Afzal Javed (a son of soil, who has done Pakistani psychiatry proud by his innumerable accomplishments and contributions to social psychiatry) ably supported by the energy and enthusiasm of A/Prof Nasar Sayeed have assumed their spiritual idols’ mantle. To bring the 3rd Asia Pacific Conference on Psychosocial Rehabilitation and the Eighth International Psychiatric Conference to Lahore Pakistan is a tribute to the blessed souls of Professor Rasheed Chaudhry and Haroon Rasheed Chaudhry. Through this scientific activity, we can emphasise the all important need to seek jobs for the mentally ill, and bring into focus the significance of social psychiatry themes and evidence-based risk factors of adverse life events, unemployment, disasters, homelessness, migration, and social adversity. It is indeed a pleasure to give this message on the occasion of 3rd Asian-Pacific Conference on Psychosocial Rehabilitation. I congratulate the organizers for holding this mega event on such an important area of psychiatry and pay warmest greeting to all the participants especially the international delegates. Psychosocial intervention/ Rehabilitation is important during management of all psychiatric conditions especially those patients suffering from severe and persistent mental illness. This component of management not only decreases psychiatric disability but also lead to restoration of community functioning and well-being of an individual. The goal is to help disabled individuals to develop the emotional, social, motor and intellectual skills needed to live, learn and work in the community with the least amount of professional support i.e. to make the sufferer an earning member of the family and useful member of the society.Enabling persons with persistent and serious mental illness to live a normal life in the community causes is indeed a shift away from a focus on an illness model towards a model of functional disability. In my opinion this will be very helpful to encourage people to invest in mental health and by enlarge reducing the stigma attached to mental illness. The revised International Classification of Functioning, Disability and Health (ICF) includes a change from negative descriptions of impairments, disabilities and handicaps to neutral descriptions of body structure and function, activities and participation. A further change has been the inclusion of a section on environmental factors as part of the classification. This is in recognition of the importance of the role of environmental factors in either facilitating functioning or creating barriers for people with disabilities. Environmental factors interact with a given health condition to create a disability or restore functioning, depending on whether the environmental factor is a facilitator or a barrier.This conference will enhance our skill by incorporating psychosocial intervention to defeat disease by Holistic approach to manage chronic psychiatric cases with multidisciplinary team involving caregivers.

In our part of the world persons with psychiatric disabilities report improper and forced treatments, including restraints, drugs, isolation, treatment from faith healers and trauma due to stigma related to psychiatric disorders. Traditionally in our society medication and psychotherapy were the two major treatment approaches, with little attention given to supporting and facilitating daily functioning and social interaction. Therapeutic interventions often had little impact on daily living, socialization and work opportunities.
