Score at the light-colored dashed fold lines. Trim at the edges of the brown color, and Fold on the horizontal scored line, gluing the front to the back. Fold at the vertical scored lines, forming a spine for the booklet. Attach a photograph of the player where indicated, and sign in the white space below the picture.
