Cause of fireThe fire started due to pyrotechnics It ignited foam on both sides of the drummer's alcove at the back of the stageThe foam was in two layers, with highly flammable urethane foam over polyethylene foam, the latter being difficult to ignite but releasing much more heat once ignited by the less dense urethane.

Cause of DeathAlthough four exits were there, most people tried to get out of the front door through which they had come, resulting in a bottleneck.The east exit door was reported to be closed and for use only by band members.By the time 2 minutes were over, the place was filled with black smoke.

Drawbacks in management462 people were in attendance, even though the club's official licensed capacity was 404

National Institute of Standards and Technology (NIST) concluded that afire sprinkler systemwould have contained the fire long enough to give everyone time to get out safely.
