IMPORTER: Coby Electronics Corp., of Lake Success, N.Y.

DESCRIPTION: The recall involves Coby DVD/CD/MP3 players with product numbers TF-DVD 1020 and TF-DVD 8501. "Coby" is printed on the front cover and the product number is on the bottom of the unit. The serial numbers on the recalled rechargeable batteries are printed on a label on the batteries in table of note 1.

MANUFACTURED IN: China

HAZARD: The rechargeable batteries can overheat, posing a fire hazard to consumers.

INCIDENTS/INJURIES: Coby Electronics has received 22 reports of the battery overheating. Eighteen additional incidents of the battery overheating in the TF-DVD 1020 model, 17 of which resulted in property damage ranging from minor up to $9,650. No additional incidents have been reported for the TF-DVD 8501 model.

In the affected product, in the event of removing the input plug from the socket outlet while the UPS is still energised, backfeed voltage may be present on the pins of the plug or the UPS input mains connector pins.

CONSUMERS ACTION

Cabac advises that any product purchased from 2006 until mid 2009 should be sent back immediately to the point of purchase or any electrical wholesaler. The product will then be forwarded to Cabac for inspection and modification to correct the issue if it exists, at no extra cost. Any product with a large blue dot on the back panel does not need to be returned as these products have already had the modifications implemented.

DESCRIPTION: This recall involves the AYC or HBAYC model chain set for use in a gymnasium or similar recreational facility with one of the following Metalux commercial light fixtures: F-Bay HB/2HB; F-Bay 2HBHD; F-Bay HBE; F-Bay HBHT; F-Bay HBI; F-Bay HBL; F-Bay 2HE Series 2HE; Arctic Bay AB; or Arctic Bay ABI.

HAZARD: An “S” hook on the chain connecting the fixture to the ceiling can straighten when subjected to high impact forces, resulting in the light fixture falling. This poses an injury hazard to consumers.

INCIDENTS/INJURIES: The firm has received three reports of light fixtures having one chain detached or the fixture falling. No injuries have been reported.

The SKU number can be found on the underside of the adapter unit.


HAZARD: Faulty wiring can cause the connector tips to heat and melt the plastic encasing the connector tips, posing a burn hazard to consumers.

INCIDENTS/INJURIES: The firm has received 518 incidents of the connector tips heating, 53 of which resulted in the melting of the plastic casings. Eight consumers have reported a finger tip or hand burn. No reports of medical attention were received.

STRANGULATION

The product poses a risk of strangulation due to the presence of a cord longer than 220 mm which can extend to 570 mm under a force of 25 N. Considering the length of the cord and the presence of a knot at one end, a slip knot could be formed while playing with the toy, and thus children under 36 months runs the risk of strangulation while playing. The product does not comply with the Toys Directive and with the relevant European standard EN 71.

HAZARD: The drop side hardware can break or fail, allowing the drop side to detach from the crib. When the drop side detaches, a hazardous gap is created between the drop side and the crib mattress in which infants and toddlers can become wedged or entrapped, posing a risk of suffocation and strangulation. In addition, children can fall from the cribs when the drop side detaches or fails to lock.

INCIDENTS/INJURIES: CPSC and LaJobi have received a total of 99 reports of drop side incidents, including hardware breakage and drop side detachment. There were two incidents in which children became entrapped in the gap created by the detached drop side. Both children were freed by their caregivers. There were six reports of children falling due to drop side failure, including one report of a mild concussion.

HAZARD: The swimsuits have halter straps that were manufactured too short causing the plastic ring located at the neck of the swimsuit to press against the child’s throat and obstruct the airway. This poses a strangulation hazard to the child.

Upper and side trim are in light brown leather or artificial leather, while the tongue, side trims and instep padding are in mustard-coloured textile. The sole is rubber, anti-slip, in two-tone light brown. The insole is yellow; internal lining is in a lighter colour. A code is written inside in gold lettering: P99-08E-576 41, probably the article code and size.


CHEMICAL

The product poses a chemical risk because the shoes contain 3.49 mg/kg dimethylfumarate (DMF) in the sachet.

Dimethylfumarate is a substance which is strongly sensitising on contact with the skin, and consumer products containing dimethylfumarate are banned according to Commission Decision 2009/251/EC.

Withdrawal from the market, recall from consumers, seizure and destruction of the products ordered by the authorities.

CHEMICAL

The product poses a chemical risk because the inner leather lining of the shoes and the silica gel sachets contain dimethylfumarate (DMF). The amount of the DMF in the lining varies between 2.2 to 2.8 mg/kg. In the sachets, the amount of DMF varies between 2.1 to 19 mg/kg. In the sachets containing grey or brown small balls, the amount of the DMF is of around 400 mg/kg.

On the sides are a series of perforations within a triangle of imitation leather revealing another colour. Packed in a light brown cardboard box with statements and designs in beige colour. The brand `Jiada` features on all sides of the box and a stylised flower motif also appears in the lower right-hand corner of the longer sides.

The product poses a chemical risk because it contains 0.35 mg/kg dimethylfumarate (DMF) in the sachets.

CHEMICAL

The product poses a chemical risk because it contains prohibited azo dyes, which release from the lining 107 mg/kg of 4-amino azobenzene.
