What a dazzling development we have been living through in the past few years!

Computed Tomography (CT) was regarded as a standard radiological application

in the mid 90’s. It was common sense that there wasn’t much left to improve

in CT, since the introduction of spiral CT in 1988.

All of a sudden, in 1998, a breathtaking series of clinical and technological

innovations was started with Multislice CT. And that was just the first spark. New

applications like Cardiac CT, CT-Colonography and low dose lung assessment

A round, well-defined isodense lesionwas depicted in the region of theenlarged sella in the native scan.A strong peripheral enhancementpattern of the lesion was clearlyshown 50 seconds after intravenousadministration of contrast medium [1].The peripheral enhancement was betterdelineated after six minutes delay andthe central part of the lesion remainedisodensed and unenhanced [2].

The CT diagnosis of pituitary tumor wasconfirmed by surgery and pathologicaldiagnosis.

Case 2:Diagnosis of Osteosarcoma with Pulmonary Metastases

A 14-year-old boy with osteosarcoma ofthe right arm and pulmonary metastasesunderwent surgical resection of thetumor and a total replacement of theupper humerus with prosthesis [1]in September 2001. He was put on aregimen of chemotherapy after thesurgery. A low dose, non-contrast spiralCT scan of the lungs was performed forfollow-up.

The CT examination of the thoraxshowed multiple metastases in bothlungs [2–5]. A significant portion of thenodules is small and lies fairly close tothe pleural surface. However many ofthem lie deep within the parenchyma,particularly those in the right lower lobe.There are also several lesions wrappingaround the trachea and main bronchi[3–5].

In spite of the low dose techniqueand the significant amount of metallicprosthesis in the vicinity of the lungs,the examination yields excellentdiagnostic information and 3D results.

A 72-year-old male patient sufferingfrom an infrarenal abdominal aorticaneurysm was treated with anendovascular aortic stent-graft.A follow-up abdominal CTA wasperformed one year after theimplantation.

Results and Comments

The CTA images demonstrated a normalinfrarenal aortic stent extending into thedistal part of both common iliac arteries.No contrast enhancement was showninside the aneurysm, which excludedthe endoleakage. Neither thrombi norstenosis could be detected in the stentlumen. Both renal arteries were wellopacified and both kidneys werenormally perfused.

The aortic branches are remarkablywell demonstrated in the secondaryreformation by using 2 x 2.5 mm slicecollimation and 1.5 mm reconstructionincrement. The MIP images, especially,present very high quality.

For the follow-up studies of aortic stent-grafts, CTA with the dual slice CT scannerSOMATOM Emotion Duo, has proven itsworth in our hospital.

Case 4:Follow-up Examination with CT-Colonography

This is a 60-year-old male patientwith a family history of colon cancer.He had a conventional colonoscopicexamination a year ago. A polyp wasfound during the previous colonoscopy.A sample was taken which proved to bea benign lesion.

When due for a follow-up the patientopted for a CT examination forCT-Colonography instead. He hadfound the conventional procedurea very uncomfortable experience.

The patient underwent bowel cleansingand a low-fiber diet for two days priorto the examination. The bowels wereinflated with air and a non-contrast,spiral acquisition was performed for thewhole abdomen.

The examination revealed a polyp-likelesion, which had been discovered inthe previous conventional procedure,in the descending colon. The lesion hada dark low-density center which wasprobably due to residual scarring afterthe endoscopic biopsy. No other polypswere found, which had been theintention of this follow-up examination.

Hence, the patient was saved the traumaand discomfort of going through asecond conventional colonoscopy.

Imagine a perfect combination ofvolume coverage, thin slices, andtop image quality; wouldn’t that trulybe progress in CT scanning? Well,outstanding technological developmentshave greatly enhanced the scope ofall CT related clinical applications andat the same time changed the wayradiologists think about CT imaging.Today’s Siemens SOMATOM MultisliceCT systems are able to acquire 2, 6, 10or even 16 slices simultaneously, coupledwith rotation times down to 0.4 s.This results in temporal resolution ofdown to 105 ms and isotropic resolutionof 0.5 mm.

The image qualityis sustained during the completemultislice scan pitch range. In addition,compared to sequence scanning,SureView can reduce the dose ofradiation up to 20%. CARE Solutions – Minimizing the Dose

Our ambition to develop CT technologyeven further is not solely concerned withthe examination method. Making CTscanning safer for both, users andpatients is just as important for theSiemens engineers. Therefore,minimizing the radiation dose is onemajor effort we are concerned with.And we are successful at it: radiationexposure in a lung CT today is lowerthan it was in a conventional lung X-rayabout 10 years ago.
