1 INTRODUCTION

e NII to I-Iinltley Point B Power Station tviewed the current plant status stating

that R4/TA8 has a boiler tube leak and the station is preparing to shut the unit down and that R3ITA7 was at nominal full load.

The minutes of the start up meeting were agreed without correction. said that any issues arising would be raised under the appropriate item.

3 ACTIONS FROM THE 2005 R4 START UP MEETING

Two sets of actions were discussed; those which were required for R4 start-up and the outstanding rcview meeting actions. -s stated that by virtue of the fact that permission to start u R4 had been granted these actions had been satisfactorily completed. dconcurred. The outstanding actions were reviewed as follows:

4.0 STATION DIRECTOR'S OVERVIEW

m g a v e his view on the stations performance since the last start up meeting in part~cular notlng what had gone well but also any areas for improvement.

He summarised the positive areas as follows:

Outage safety performance -no LTAs Contractor safety performance -no LTAs for 987 days but still areas for improvement (for both Station and Contractors). Plant was returned from Outage safely and to plan The Station was now allowed to recruit more people and Operations in particular had been increased (recognizing the additional training burden). System Health had almost completed its recruitment. A new permanent outage organization was being formed. This would replace the current system of secondment from different departments. Work management - scope stability/schedule adherence -best in fleet. Nuclear Safety Cul t~~re Awareness workshops -41 now held for all staff & contractors and delivered by the management team. Very positive response from attendees. Human Performance Tools -Workshop held for fuel route team in the use of error prevention tools. Now being rolled out to the rest of the station. Cast Iron replacement -Huge achievement to replace all the cast iron pipework whilst remaining on load with no reportable events1LTAs. No Reportable Events since the last meeting. The number of Task Observations has significantly increased however it is recognized that the quality of observations needs to be improved. There has been a major improvement in the CAP process and its effectiveness e.g. screening meeting. Significant improvement had been made in CCR conduct of operations.

Me then sumlnarised areas for improvement:

TA8 governor vlv returned from outage with stiction problems when in the fully open position, which had affected output. Repair of Gas Bypass Plant GI230 provided a number of lessons to be learnt Condensate instability problcms are still not fully understood. CMEC system diesel spill -24000 litres lost into a trench. All the oil has now been accounted for. Delivery of the C02 plant improven~ent programme had been slow. Material condition of plant - still a long way to go. Additional funding had been requested for Station improvements. Boiler tube leak -no permanent solution devised for economiser tube leak plugs (noting cunent R4 leak). Sccurity - general issues placing a heavy burden on security. Environmeutal Technical Specifications -large work load to implement. Training accreditation - large work load to mcet training accreditation deadline of June 2007. Lub. Oil Coolcr issue - needs to be revisitcd as further leaks have occurrcd

4.1 Plans for R3 Outage

With respect to the supplied brief, said that he welcomed the review of data and anal sis. However what portant was how this information was wed. & next raised the issue of SLOs (Site Liason Officers). He stated that the SLO role w Id be set for their responsibilities hat SLO responsibilities were managed durmg station reorganisation. ted that he would be reviewing SLO effectiveness during the Outage.

5.0 FEEDBACK FROM NII TOUR

ummarised the observations of the plant tour on the previous day. 'The tour concentrated on the Turbine Hall & RCA, revisiting areas visited during the start up mecting in November 2005. He noted his disappointment that in a number of cases no improvement was seen sincc the last mccting. He sited several examples:

Leak management deficicncics - water close to electrical cables, in the turbine hall causing a hazard.

Cables in pond area still loose although there has been somc improvement. Ncwspapers and loose kcys found in a C2 area. This was not acceptable. Flask Bay -material condition is poor, no change from previous meeting and poor working environment. Problems found with spalling concrete in the Turbine Hall probably due to corrosion of re-enforcing bars. Not an immediate problem but will be one in the long-term. No action seems to have been taken. One person found working without appropriate PPE. A gas cylinder was found in the Gas Bypass Plant tied to a column with rope and a person was sitting next to it, but had not taken any corrective action. Material Conditions in certain areas had improved but there was still scope for significant improvement.

6.0 SAFETY MANAGEMENT REVIEW

6.1 Nuclear Safety

o t e d that there were a number of events caused by human performance issues 1.e. operator errors and asked what was being put in place to address the problem. TIIreplied that an action plar, had been put in place for the fuel route teams -o owmg fuelling events. This programme reinforces the use of the 10 error prevention tools i.e. pre-job briefs, peer checking etc. The programme is now to be rolled out to all other teams. In addition, Nuclear Safety Culture Awareness courses have been held for all staff. Management has reinforced expectations to all staff and PJBIRisk assessments were being used for all non-standard operations. He asked if OEF had been looked at as part of the training. r e p l i e d thpt it had, and in particular the incident with the Heysham power station charge machme was used as a case study.

6.2 Industrial Safety

A contractor was found working on a roof without proper safety equipment or procedures The same contracting Company had been found lifting a portacabin without any supervision by BE staff or proper lifting procedure. Checks on another contractor's welding equipment revealed faults which could have given someone an electric shock.

6.3 Radiological Safety

He also asked for instances where ALARP assessments have been used to try and limit dose. -stated that the main doses are from vessel entry. For the recent cell entries, it was decided to clean the cell externally as far as it was possible, and to undertake extensivc task rehearsals. He agreed to include data on trending for future meetings.

6.4 Emergency Arrangements

The Technical and Safety Annual Report findings will be fed back to the Station Director. Control of Contractor issues & role of SLOs was an area for concern. The Training Assist has followed up shoi-tfalls in regard to duly authorized persons -SRD had provided assistance during this process. Following Station Reorganization there was unease amongst System Health staff. This needed to be addressed. Assessment Branch main area of work is focused on the assessment of PSR2 submissions.
