UNTRACEABLE PRACTITIONERS, 1937

WE have been requested by the authorities of the General MedicalCouncil to publish the following list of registered medical practitionerswho had not, when the list was sent to press, replied to letters writtento them on behalf of the Council under Section 14 of the Medical Act,1858, in order to ascertain whether their addresses on the Medical Registerare still correct. The date given after each name is the date of originalregistration ; the qualification given is the principal qualificationregistered by the practitioner. The letters (E), (S), or (I) indicatewhether the practitioner was originally registered in England, Scotland,or Ireland ; and the expression (Col.) or (For.) indicates that the

practitioner is registered in the Colonial List or in the Foreign list in theRegister. Any practitioner, wherever resident, who finds his or her namein the list should write without delay to the Registrar, General MedicalCouncil, 44, Hallam-street, Portland-place, London, W.1, unless he orshe was originally registered in Scotland. Practitioners originallyregistered in Scotland should write directly to the Registrar of theBranch Council for Scotland, 44, Queen-street, Edinburgh, 2. Thename of every practitioner in the list from whom a further communi-cation is not received at a very early date will be omitted from thenext issue of the Register in accordance with the provisions ofSection 14 of the Act of 1858.

PANEL AND CONTRACT PRACTICE

The Post-graduate CoursesTHE new arrangements for post-graduate courses

for insurance practitioners are not meeting withunqualified approval. The National Conference of

Friendly Societies and some insurance committeesare taking exception to the entirely medical adminis-tration of the scheme. The National Association ofInsurance Committees also want to know why therepresentatives of insured persons are to have noshare in the administration. It appears that on

Sept. 8th a regulation was published making it clearthat the cost is to be met from the unclaimed proceedsof the sale of health insurance stamps, and thereforewill be paid partly by insured persons and partly bytheir employers. The London committee, whilewelcoming the scheme, have recorded their regret atthe exclusion of insurance committees from itsadministration.

A Standard Fine ? ?

The London medical benefit subcommittee reportthat the Minister has withheld the sum of E5, witha view to a corresponding deduction from the remu-neration of the insurance practitioner concerned, ineach of six cases. Comparison of the sins of thevarious practitioners shows that one had failed to

provide adequate attention for an insured person ;the insurance committee required the doctor to paythe bill of the other practitioner who was called in,and recommended the withholding of £10. The secondhad unwisely tried to help the financially embarrassedwife of an insured man in hospital by signing twocertificates without seeing the patient ; E2 was recom-mended but E5 was withheld. Case No. 3 was oneof extravagant prescribing, and the amount withheldwas that which had been recommended. The otherthree cases had not been before the insurance com-mittee and each related to failure to return to theD.M.O. within the time specified certain formsR.M.2.

Three Medical Service Cases

The London medical service subcommittee did notmeet from May until September and even then hadonly three cases to consider. One was disposed of sum-marily by informing the complainant that his state-ment disclosed no prima-facie grounds of complaint.Another was the all too familiar, though fortunatelynow less common than formerly, issue of certificatesirregularly.A man, suffering from endocarditis, declared on the

funds of his society on Feb. 20th. He then went awayinto the country and it was not until March llth that hesubmitted another certificate, again an initial certificate,which was issued by a doctor in the Isle of Wight. The

society declined to pay benefit for the intervening periodin the absence of medical evidence of incapacity. Theinsured person wrote to the first doctor, who had not seenhim since Feb. 20th, telling him of the difficulty andasking for certificates, which he led the doctor to thinkhad been requested by the society. The doctor was of

the opinion that the man had been continuously incapaci-tated-and in fact he remained incapacitated until

September-so he issued insurance certificates datedFeb. 27th, March 6th and 13th, and a final certificatedated March 16th, this last because he understood thatas from that date the medical care of the patient had beenassumed by the practitioner in the Isle of Wight. Thus he

signed five statements which were not true-viz., that onfour dates he had examined the patient and that onMarch 16th the patient was in his opinion no longerincapable of work. The society did not accuse the practi-tioner of any intention to mislead them, but the insurancecommittee had no option but to find that the practitionerhad committed a breach of the medical certificationrules.

It is a curious thing that medical men should sooften be content to sign printed statements whichthey would undoubtedly decline to write in theirown hand, and it cannot be too strongly urged thatthe printed health insurance certificates containstatements of fact and should not be signed unlessthose statements are of fact.The remaining case was an unusual one.On June 28th the patient, a woman of about 37, con-

sulted the practitioner’s assistant, complaining of pain inher right leg. Liniment was prescribed, rest in bed

advised, and in the absence of a further report it wasassumed that the condition-which was a diffuse tender-ness over the shin-had cleared up. The father, who wasthe complainant, bitterly attacked the assistant’s handlingof the case, but before the subcommittee, although allegingthat the patient suffered intense pain from June 28thonwards, he admitted that no attempt was made to securefurther medical attention until July 1st. On the latterdate the assistant visited the patient by request andfound her suffering some pain in the same leg and with atemperature of between 99° and 100° F. Having regardto the patient’s nervousness he endeavoured not to frightenher, but he discussed with her and her mother the desira-bility of hospital treatment, since he suspected osteo-

myelitis. The patient was removed to hospital wheretwo X ray photographs on successive days gave no indi-cation of a disease of the bone ; and in fact it was notuntil ’the fourth day after admission that a definite diag-nosis of osteomyelitis was made and an operation performed.The patient died on July 8th. The responsible practi-tioner said that in over thirty years’ experience he hadnot known this disease to originate in a patient of morethan about 15 years of age whereas this insured personwas 37. He thought that on June 28th there could havebeen no signs, and even on July lst almost no symptomsindicative of osteomyelitis were to be found.

In this view the medical members of the subcommitteeconcurred, and the subcommittee’s report concludedwith an expression of regret that the father shouldhave pressed the case so strongly upon insufficientgrounds.

RoYAL BERKSHIRE HOSPITAL.—In response to thecentenary appeal for ;EI 50,000 made by this hospital over:f:66,OOO has now been received. The committee hope toobtain half of the required sum before the end of the year.
