    ABSTRACTB a c k g r o u n d : The interactions betwe en ins ulin and physical activity are varied and complex. Control of blood glucose

    levels relies heavily on the balance between insulin and opposing counterregulatory endocrine hormones that bind totheir respective receptors in muscle, adipose, and liver tissues. Other metabolic factors--such as fuel use and availabil-ity, exercise intensity and durati on, train ing status, and visceral fat levels --also affect physical activity done by indiv id-uals with ins ulin resistance and type 2 diabetes mellitus DM).

    O b j e c t i v e s : The purpose of this article was to discuss how certain variables interact with acute sessions of physicalactivity and more extended exercise training, focusing special attention on their relationship to both exercise andresting glycemia as it relates to type 2 DM preve ntion and control. Also discussed are strategies that indivi duals withtype 2 DM can use to engage in appropriate physical training to control DM and prevent other insulin-related healthproblems.

    M e t h o d s : The content of this article was based on an electronic search of the literature using Pu bMed a nd the selectionof key search terms alone an d in combination, including exercise, aerobic training, resistance exercise, insulin , hyperglycem ia,diabetes, diabetes prevention, and humans . Articles that app eared in relevant, combine d searches of these terms were exam-ined back to the ye ar 2000. A seconda ry search of articles related to type 2 DM a nd exercise was con ducte d via referencesfoun d in these cited studies that m et the search criteria.

    Results: Current research suggests that type 2 DM can be prevented and controlled with increased physical activity.With respect to the possibili ty of reversal of a prediabetes state, both aerobic and resistance training may pla y imp orta ntroles in these processes. Altho ugh ind ivid uals with DM are usu ally instruc ted to exercise to improv e control of their dis-ease, their special metabolic situation can lead them into hypoglycemic or hyperglycemic conditions. When recom-mended guidelines are followed, however, physical activities can be done safely and effectively. In addition, abnormalinsulin action in the body is associated with not only prediabetes an d DM but also with a host of other insulin-relatedhealth conditio ns, includ ing cardiovascular disease.

    I N T R O D U C T I O NThe interactions among insulin and insulin action, counter-regulatory hormone release, blood glucose regulation, andphysical activity are varied and complex. Control of bloodglucose levels relies on the balanced release of insulin andother redun dant endocrine hormones; the proper function-ing of hormone receptors found particularly in insulin-sensitive muscle, adipose tissues, and the liver; and othermetabolic factors such as fuel use a nd availability.1 3 More-over, individ uals with diabetes mellitus DM) are usuallyinstruc ted by their health care provider s to exercise, but is itsafe and effective for these ind ivid uals to do so? In addition,abnormal insulin action in the body is associated not only
