T he professional competency of Asian orthodontists in treating pa-

tients with Class III malocclusion is one of the “Asian wisdoms” which has been overlooked in the past. It has been recognized that the

majority of Class III malocclusion cases are found in the Asian population.

Since orthodontic knowledge and formal education was first developed in

the Western world, not surprisingly most Asian orthodontists believe in us-

ing this Western knowledge to treat these patients.

The Asian wisdom in treating patients with Class III malocclusion has been

continuously developed and changed by several groups of Asian orthodon-

tists. Now, with advances in technology and communication, this valuable

knowledge and wisdom has been widely accepted throughout the world.

Complications in Class lll Treatment Diagnosis and Re-treatment

Lecture is given in English.

The common complications of Class lll treatment include:

improper extraction treatment, unnecessary early or-

thopedic correction, relapse due to late mandibular growth,

over treatment by using temporary anchorage devices

(TADs), and inappropriate orthognathic surgery for patients

with an orthognathic profile and Class lll malocclusion.

Improper extraction often causes lingual dumping and gingi-

val recession of lower incisors. Sometimes improper lower

single arch extraction can lead to the absence of antagonist

for upper second molars.

Relapse of Class lll early treatment due to late mandibular

growth is a common phenomenon. Early orthopedic treat-

ment and early interceptive treatment will be carefully ana-

lyzed, to see if the results from these types of early treat-

ment can sustain in long-term follow-ups.

Nowadays due to the advancement of TADs, many

types of treatment involve the use of TADs. Proper

diagnosis should be made to evaluate the appropri-

ateness of these aggressive devices for mild Class

lll cases. Various applications of TADs on Class lll

cases will be systematically reviewed, followed by

specific recommendations on case selections. Tra-

ditionally surgical correction is the first and primary

treatment option for severe Class III malocclusion.

With the available technology of passive self-ligating

brackets and TADs, the treatment of difficult Class

III malocclusion is now much easier and possible

without relatively more invasive surgeries. For pa-

tients with severe Class III malocclusion and an ac-

ceptable profile, orthodontic camouflage treatment

usually is preferred than orthognathic surgeries.

All the complications described above will be exam-

ined in this lecture. Extensive long-term follow up

cases documented in the literature review, togeth-

er with the speaker’s own cases, will also be pre-

sented as evidence. Specific clinical considerations

and treatment options in dealing with these com-

mon complications will be provided followed by the

analysis.

Total Arch Distalization with TADs

Lecture is given in English.

Mini-implant anchorage has been applied widely in or-

thodontic treatment for anchorage control in recent

years. Maximal retraction without anchorage loss is the

primary objective while utilizing mini-implants. Total arch

distalization after maximal retraction was noticed in some

protrusion cases, which exceeds the original expectation. If

total arch distalization is feasible, flared incisors can then

be repositioned to an ideal position without extraction. Non-

extraction approach with miniscrew anchorage can not only

be successfully applied in upper arch distalization for Class

II correction, but its application in lower arch distalization

for Class III correction has also produced satisfactory re-

sults. Severe Class I crowding cases are also good candi-

dates for total arch distalization. Nevertheless, proper diag-

nosis should be carefully performed prior to proceeding with

treatment modalities incorporating mini-implants. Various

anchorage setups in various malocclusions for total arch dis-

talization and the key to the successful total arch distaliza-

tion- exodentitional position of the miniscrews will be dem-

onstrated in the presentation. The limiting factors of this

treatment approach will be also discussed.

Johnny Joung-Lin, Liaw completed his orthodontic training at National Taiwan Uni-

versity Hospital in 1994 and received his master degree in Chang Gung University.

He is now in private practice since 2002 after 8-year visiting staff in Shin-Kong

Memorial Hospital. He keeps on lecturing a clinical orthodontic course over 15 years

and he is also a clinical director in National Taiwan University Hospital. And he is now

the president of the Taiwan Association of Orthodontists.

CIass III malocclusion combined with impaction presents

a significant challenge to orthodontists and often re-

quire collaboration with oral surgeons. Several critical con-

siderations involve: 1. extraction or not, if yes, which one? 2.

surgical exposure; 3. mechanic force design; 4. soft tissue

management in the impacted area; 5. profile change.

This lecture aims to present an effective protocol to address

these challenging but common issues in Class III treatment

by orthodontic bone screws.

Learning objectives:

1. Outline the thinking process for Class III treatment

planning.

2. Understand the design of force systems to treat

complicated impacted teeth and malocclusion.

3. Acquire ability to apply this innovative approach to

practice

Dr. Chris H. Chang,

Dr. Chris Chang received his PhD in bone physiology

and Certificate in Orthodontics from Indiana Uni-

versity in 1996. Dr. Chang is a Diplomate of Amer-

ican Board of Orthodontics(ABO). He is author

of iAOI workbook, ABO Case Reports, 3D iBooks

Ortho, Jobsology and publisher of International

Journal of Orthodontics and Implantology(iJOI).

Dr. Chang is frequently invited worldwide to

lecture on a wide range of topics, including im-

paction treatment, OrthoBoneScrews, implant-

orthodontic combined treatment and Jobs’ ef-

fective presentations. In addition to teaching and

private practice, he also founded Newton’s A, Inc.

and Beethoven Orthodontic Group in Taiwan and

produces a podcast series, Podcast Encyclopedia

in Orthodontics, and the first 3D interactive or-

thodontic ebook, 3D iBooks Ortho, as innovative

E-learning tools. He has been actively involved in

the design of orthodontic bone screws and appli-

cation on impaction treatment. His latest focus

is implant and orthodontic combined treatment.

Some Practical Orthodontic Mechanics I Share with Other Class III Gurus

Lecture is given in English.

This lecture will summarize the ideas, design, planning and application

of various orthodontic mechanics, which Dr. Rungsi Thavarungkul has

learned and shared throughout the years, with several famous colleagues.

They have proven their theories and beliefs by successfully non-surgical-

ly treating many patients with severe Class III malocclusion. This valuable

knowledge has been growing its acceptance steadily throughout the world.

He will also present the application of these various mechanics in the Thai

patient population.

Dr. Rungsi Thavarungkul

Rungsi Thavarungkul received a master degree in orthodontics from Chu-

lalongkorn University in 1990 and has been in private practice ever since.

He is also well-known for providing his own animation in many orthodontic

books as well as presentations which enabled him to successfully shared

and communicated his orthondontic knowledge and experience in a crea-

tive way.

WeCare WeShare Journey

WeCare just celebrated our 5thyear anniversary in August 2012 and announced our

next step milestone toward “WeCare Ortho-Implant Solution Center”. The intention

is for WeCare WeShare (WeCare Academic Team led by Dr. Pintippa Bunyaratavej & Dr. Rungsi

Thavarungkul) to have a clear vision in developing and driving the WeCare team.

In 2009 WeCare WeShare started our first study club for a team of 20 dentists,

mostly specialists in various disciplines. We realize the fact that most specialists are com-

petence, knowledgeable and skillful in their own disciplines while may not be so well around

in other disciplines. The first goal is to have a common place for WeCare team to regularly

meet and communicate with one another to strengthen team professional understanding.

To this day, WeCare team is steadily growing. We believe that positive and open communica-

tion is one of major keys to team success. We are moving step-by-step toward our future

challenge which is The Excellence in Interdisciplinary Clinical Dentistry.
